One of the most challenging decisions is to assess the preterm infant's transition from tube feeding to oral feeding. Thus, we require a reliable tool for determining the time to start oral feeding. This study aimed to measure the interrater and intrarater reliability of the Preterm Infant Oral Feeding Readiness Assessment scale (PIOFRA). Materials and Methods:This study was an observational, cross-sectional study. The study participants were preterm infants who had been hospitalized in the Neonatal Intensive Care Unit of Shariati Hospital affiliated to Tehran University of Medical Sciences, between December 2017 and February 2018. The inclusion criteria were absence of neurological and gastrointestinal disorders or major congenital anomalies, Apgar score 3 or more in the first 1 minute, and 5 or more in the first 5 minutes. The exclusion criteria included family's unwillingness to participate in the study, infant's death, or a sudden change that affects neonates' nutritional status, like cerebral hemorrhage or intestinal problems. Results:The interrater and intrarater reliability of the total PIOFRA scale was good Intraclass Correlation Coefficients (ICC˃0.75). The interrater and intrarater reliability of most items were good and excellent, with weighted kappa more than 0.50, with the exception of lip posture and especially stress sign, with weighted kappa less than 0.40. Conclusion:Generally, most items of the PIOFRA scale had acceptable interrater and intrarater reliability. Also, the interrater and intrarater reliabilities of the total POFRAs score was good.

Correlation of facial anthropometry data of late preterm newborns and oral feeding readiness

Objective: To correlate nine facial anthropometric measurements with the readiness for oral feeding of late preterm newborns using an orogastric tube. Methods: Observational study, carried out in two institutions in Maceió, Alagoas. Fifty-two newborns participated. A single measuring of nine facial measurements and daily measuring of the interface area for fixing the tube was performed. For readiness assessment, the Premature Oral Feeding Readiness Assessment Scale was used. Results: An average readiness of 28.81 (± 3.18) was observed in the first evaluation and 30.65 (± 3.23) in the second. Most facial measurements are correlated with weight. There was a positive and slight correlation between glabella-subnasale distance and readiness. No correlation was observed between the area of the tube fixation interface and facial measurements. Conclusion: It is concluded that the glabella-subnasale measurement is positively correlated with the readiness for oral feeding in late preterm newborns who used an orogastric tube for feeding.

Background: Prematurity is a risk factor for eating disorders. However, late preterm newborns are generally cared for in a rooming-in unit, with no different management from term newborns. Hyperbilirubinemia is considered physiological, but can be potentially neurotoxic at high levels. Phototherapy is the most widely used treatment, but it can lead to adverse events, such as circadian rhythm changes. Preterm infants are at greater risk of neurotoxicity due to hyperbilirubinemia and feeding difficulties are a risk factor for increased serum bilirubin levels. In view of this, it is necessary to evaluate the breastfeeding performance of late preterm infants in order to provide data that will enable appropriate management in the face of possible difficulties and guarantee that preterm infants undergoing phototherapy will have an effective feed and adequate intake. Methods: Controlled cross-sectional study. Conducted in a public maternity hospital, the sample was based on convenience and consisted of 60 mother/late preterm newborn dyads, assisted in the Joint Lodging, divided into two groups. One group comprised of 30 mother/newborn dyads in which the newborns had hyperbilirubinemia that required phototherapy (case group) and another group with 30 dyads in which the newborns did not require phototherapy (control group). Data characterizing the sample was obtained through anamnesis and medical/hospital records. Breastfeeding performance was assessed using the Breastfeeding Observation Form proposed by UNICEF. The analyses were carried out using Jamovi (version 2.4.1). The significance level of 5% was adopted for all analyses. The study was approved by the Research Ethics Committee. All participants/guardians signed the Free and Informed Registration Form. Results: Newborns in the case group had a higher occurrence of "fair" and "poor" scores in all the protocol categories. Conclusion: The study concluded that late preterm newborns undergoing phototherapy have more difficulties in breastfeeding compared to late preterm newborns who do not require the treatment, and therefore need careful and individualized attention in the management of breastfeeding.
