The introduction of early complementary feeding (ECF) is determined by different factors depending on when it occurs. The objective of this study was to analyze factors associated with the introduction of ECF in two different moments of the infant's life: from zero to three and from four to five months of age. A cohort with 3,306 dyads studied in the BRISA survey in São Luis/MA in 2010 was used. Questionnaires were applied at birth and at follow-up when the infants were 15 to 36 months of age of women with more than 20 weeks of gestational age, residing in this municipality. A multivariate model of multinomial logistic regression was used to verify associations between independent variables and ECF at 0 to 3 months and at 4 to 5 months of age. A hierarchical analysis model was used to select variables for confounding adjustment. Variables with a P-value o0.05 were considered significant. For ECF introduced between 0-3 months, the variables ''use of pacifier'', ''maternal paid activity'', ''smoking'', and ''postpartum pregnancy'' were identified as risk factors. The variables ''use of pacifier'' and ''maternal paid activity'' remained associated as a risk for ECF introduced from 4-5 months. The variable 'mother without partner' (RR=1.26 and P=0.04) represented a risk factor for ECF only for the 4-5 months period. Although each period presented specific risk factors, the use of pacifier and maternal professional activity were associated in the two periods studied, indicating their importance for the introduction of ECF.

Objetivo: Descrever a vivência da manutenção da prática do aleitamento materno exclusivo segundo a percepção de mães, quando do retorno ao trabalho. Metodologia: Trata-se de estudo exploratório-descritivo, qualitativo, realizado em uma Instituição de Ensino Superior de Fortaleza-Ceará com nove mulheres, selecionadas por meio do método de amostragem bola de neve, que vivenciaram ou estavam vivenciando a experiência do aleitamento enquanto trabalhadoras formais. Os dados foram coletados em novembro de 2016, por meio de entrevista semiestruturada e analisados pela técnica de análise temática, proposta por Minayo. Resultados: Da análise das entrevistas emergiram três categorias que abrangeram o “Conhecimento das mães sobre a amamentação”; “Sentimentos maternos no retorno ao trabalho” e “Dificuldades para manutenção da lactação exclusiva no retorno ao trabalho”. Conclusão: As mães mostraram conhecimento sobre o aleitamento exclusivo alinhado às recomendações dos organismos de saúde, sendo grande parte das informações fornecida antes da alta da maternidade pelos profissionais de saúde. O retorno ao trabalho se apresentou como fator estressor para a manutenção do aleitamento exclusivo devido à separação física da criança, da longa distância entre o local de trabalho e a residência, o não cumprimento do horário da amamentação pelo empregador, as longas jornadas de trabalho e a falta de espaço adequado para a ordenha ou amamentação. Torna-se necessário que sejam desenvolvidas estratégias intersetoriais para garantir que as mães trabalhadoras que optarem pela amamentação dos filhos, principalmente em caráter exclusivo, tenham esta prática facilitada.

Objective: To evaluate the effectiveness of play strategies to break the pacifier-sucking habit and induce self-correction of the anterior open bite in the primary dentition. Methods: Data collection took place at the Pediatric Dentistry clinic of a public education institution. Three children, aged between 3 and 4 years old, using a pacifier and presenting with a 2 to 5 mm anterior open bite, participated in the research. Pediatric dental guidance was imparted to educate the family and to provide play strategies for the child by means of an illustrated book about the pacifier fairy. Results: After the first consultation, two children quit their pacifier habit. Subsequent consultations were devoted to positive reinforcement and follow-up of the self-correction of the anterior open bite. The third child required four visits to stop the pacifier habit, but other habits persisted and the open bite was only corrected after intervention by a multidisciplinary team. Conclusions: After receiving orientation from the professionals, the parents/guardians cooperated in stimulating the child, and the play activities were effective in encouraging the child to stop sucking on the pacifier, permitting self-correction of the anterior open bite. Bottle feeding, tongue interposition and mouth breathing may hinder the spontaneous correction of the anterior open bite after the pacifier habit is abandoned, requiring multidisciplinary intervention.
