Background Compared with those without dementia, older patients with dementia admitted to acute care settings are at higher risk for triad combination of polypharmacy (PP), potentially inappropriate medication (PIM), and drug-drug interaction (DDI), which may consequently result in detrimental health. The aims of this research were to assess risk factors associated with triad combination of PP, PIM and DDI among hospitalized older patients with dementia, and to assess prevalence and characteristics of PP, PIM and DDI in this population. Methods In this retrospective cross-sectional study, 416 older inpatients diagnosed with dementia and referred for specialist geriatric consultation at a tertiary hospital in Brisbane, Australia during 2006-2016 were enrolled. Patients were categorized into two groups according to their exposure to the combination of PP, PIM and DDI: 'triad combination' and 'non-triad combination'. Data were collected using the interRAI Acute Care (AC) assessment instrument. Independent risk factors of exposure to the triad combination were evaluated using bivariate and multivariate logistic regression analyses. Results Overall, 181 (43.5%) were classified as triad combination group. The majority of the population took at least 1 PIM (56%) or experienced at least one potential DDI (76%). Over 75% of the participants were exposed to polypharmacy. The most common prescribed PIMs were antipsychotics, followed by benzodiazepines. The independent risk factors of the triad combination were the presence of atrial fibrillation diagnosis and higher medications use in cardiac therapy, psycholeptics and psychoanaleptics. Conclusions The exposure to triad combination of PP, PIM and DDI are common among people with dementia as a result of their vulnerable conditions and the greater risks of adverse events

RESUMO Objetivos Determinar a incidência de delirium em idosos internados em um hospital terciário e verificar sua distribuição de acordo com variáveis preditoras. Métodos Foram incluídos idosos que se internaram via emergência de abril a setembro de 2018. A amostra foi caracterizada e foi verificada a diferença da incidência de delirium , em até cinco dias de internação, em relação às variáveis preditoras (teste qui-quadrado, 5%). Resultados A amostra foi composta por 300 participantes predominantemente do sexo masculino (52%), com idade entre 60 e 69 anos (50,7%), eutróficos (49,3%) e com três ou mais doenças anteriores à internação (42%). A incidência de delirium foi de 12% (IC95% 9-16). Foi observada diferença estatisticamente significativa em relação ao número de doenças anteriores à internação (p = 0,025), à saída do leito durante a internação (p < 0,001) e ao uso de sonda vesical (p < 0,001). Conclusão O reconhecimento desse transtorno é fundamental na rotina de emergência dos hospitais, onde são atendidos pacientes com múltiplas doenças crônicas, pois o delirium é comumente subdiagnosticado, levando a um aumento nas taxas de morbimortalidade.

Objective: To evaluate medication adherence and associated socioeconomic factors in elderly Brazilians. Methodology: This observational study was conducted with 159 elderly retired in an outpatient clinic in the city of São Paulo. Treatment adherence was assessed with the questions from the Morisky Green Levine Medication Adherence Questionnaire, and medications were classified using the Anatomical Therapeutic Chemical system. Statistical tests and adjusted Poisson regression models were used to analyze variables. Results: The study population was mostly female (67.5%), had an average age of, and took an average of 6.5 medications per day. The most commonly used drugs were agents acting on the renin-angiotensin system (67.9%), statins (62.3%), antithrombotic agents (48.4%), and biguanides (37.1%) for the treatment of hypertension (76.7%), dyslipidemia (54.1%), and diabetes (47.8%). The rate of adherence was below 60% in the groups of participants that were analyzed except for the high household income category, which had a rate of 75.8%. Conclusion: Medication adherence among the elderly was low in all categories except for the high household income category, a relevant finding that will help to understand medication adherence patterns in elderly Brazilians.
