Aim: To evaluate the efficacy of different kinematics in removal of root canal filling materials.Methodology: A total of 60 human freshly extracted mature permanent maxillary central incisors were selected. Access cavities were prepared and the teeth were cleaned and shaped then obturated. Teeth were randomly divided into three groups according to removal technique (manual H-file, ProTaper NEXT file and Reciproc file), then split longitudinally and photographed. The images were transferred to a computer, and total canal space and the remaining filling material were quantified. The ratio of the remaining filling material to the total root canal space was computed with the aid of AutoCAD software. The mean percentages of the remaining filling material and removal time were analysed using the Kruskal-Wallis, Friedman's test and one-way ANOVA test. Results:The mean percentage of the remaining filling material was significantly higher in group I than both II and III, While there is no statistically significant difference between group II and III. Regarding comparison between the root levels, the apical level showed the statistically significantly highest mean percentage of remaining endodontic filling material, followed by the middle and the coronal levels. The time required to remove filling material was significantly shorter in group III followed by II and I. Conclusion:Total elimination of root canal filling is a challenge especially in the apical part. No statistically significant difference was found between the two kinematics regarding the efficiency in removing obturation materials, while reciprocation was faster than rotation in removal time.

Influence of solvent use on apical extrusion during removal of Resilon™ from root canals

Background/Aim. During retreatment, filling material and debris may extrude and trigger an inflammatory reaction of periapical tissues. Resilon? has not been investigated in terms of solvent use and influence on apical extrusion during retreatment. This study evaluated the amount of apically extruded debris during Resilon? removal using ProTaper, Twisted File and Hedstrom instruments, with and without solvent. Methods. In total, 72 extracted teeth with single canal were used. Canals were prepared with ProTaper Universal (F2) and filled with Resilon? and RealSeal sealer, before being assigned randomly to 6 groups (n=12 each). Retreatment in groups 1, 2 and 3 was done with ProTaper, Twisted File or Hedstrom instruments, without solvent; in groups 4, 5 and 6 the same instruments were used with chloroform. Apically extruded debris was collected in a simulated periapical environment and assessed visually. Additionally, the time required for retreatment was recorded. Data were analysed statistically using Mann-Whitney U test, with a significance level of 0.05. Results. There were no significant differences in apical extrusion debris between groups (p > 0.05) regarding solvent use. Rotary instruments, without solvent, were associated with significantly less debris extrusion when compared with hand files (p < 0.05). When solvent was used, rotary instruments caused higher degree of extruded material, that was similar to the results of hand instruments. Conclusion. Within the limitations of this study, the use of rotary instruments without solvent resulted in a lower degree extrusion of Resilon ? material compared to hand instruments, while greater caution is advised in the presence of solvent when rotary instruments are used to remove this material.

The Impact of Citric Acid Solution on Hydraulic Calcium Silicate-Based Sealers and Root Dentin: A Preliminary Assessment

Hydraulic calcium silicate-based (HCS) sealers have recently gained tremendous popularity due to their unique properties. However, their removal during endodontic retreatment is challenging. The solvent, which could chemically deteriorate the material, would be highly desirable for endodontic retreatment procedures. This preliminary study assessed the interplay and dissolving capability of 10% and 20% citric acid, compared to 17% EDTA, on commonly used HCS sealers (AH Plus Bioceramic Sealer, Bio-C Sealer, BioRoot RCS, TotalFill BC Sealer), and evaluated the potential impact of these solutions on root dentin structure. The interaction between tested sealers and irrigating solutions was photographed, and solubility-related mass changes were determined. The surface morphology of treated filling materials and dentin was evaluated using a scanning electron microscope (SEM). One-way analysis of variance (ANOVA) along with Tukey’s test were used to detect the statistically significant differences among groups at the confidence level of 0.95. Intense gas release was observed during the interaction of HCS materials and citric acid, with no evidently visible “bubbling” after the immersion in EDTA. The mass loss of HCS sealers equally confirmed the significantly higher dissolving characteristics of 10% and 20% citric acid solutions compared to EDTA. The surface structural changes, associated with pore and crack formation, were mainly seen for HCS sealers exposed to citric acid. Meanwhile, no severe erosion was detected for dentin after root canal preparation with 10% and 20% citric acid solutions. These findings demonstrate that citric acid has the potential to dissolve HCS sealers with minimal or no negative impact on root dentin, suggesting citric acid as a solvent for HCS sealers in endodontic retreatment procedures.
