The efficacy of whitening toothpastes is questionable and controversial. Clinicians, patients and researchers have expressed concern with whitening toothpastes due to the risk of wearing the dental structure and the potential for disappointment if the advertised cosmetic results are not achieved. Objective: This study compared the whitening performance of toothpastes with different whitening technologies after initial and continued use. Material and Methods: Ninety bovine incisors were stained using a concentrated solution of black tea. They were randomly distributed into 6 groups, according to the toothpaste whitening technology: activated charcoal (B&W), blue covarine (WAD), hydrogen peroxide (LWA), microbeads (Oral B 3D White Perfection – 3DW) and optimized abrasives (XW4D). They were compared to a traditional toothpaste without a whitening agent (TA – control). Specimens underwent a brushing machine with controlled pressure, time and temperature. A calibrated examiner measured the color using a VITA-Classical scale before the first brushing cycle (T0), after the first brushing cycle (TI), and after a brushing cycle that simulates continuous use (TCU). Whitening performance was evaluated by the difference of shades (ΔSGU) between T0–TI and T0–TCU timepoints, using the Kruskal-Wallis and Dunn's non-parametric test. The Wilcoxon test was used to evaluate the cumulative effect (α=0.05). Results: Statistically significant differences were observed between toothpastes in both TI and TCU (p<0.05). The time of use also had a significant effect (p<0.05). Conclusion: Only WAD and 3DW showed whitening performance after the first use (TI). The greatest whitening performance after continuous use was obtained by WAD, followed by LWA and 3DW. The use of conventional toothpaste (TA) promotes no tooth whitening. Clinical relevance: Microbead abrasives (3DW) and blue covarine (WAD) were the active technology tested that presented the best global tooth whitening performance.

Resumo Introdução O blue covarine é um agente branqueador que promove mudanças ópticas na superfície dos dentes, associado aos abrasivos dos dentifrícios, colaborando para o branqueamento dentário. Objetivo Determinar as alterações de cor, rugosidade e massa do esmalte bovino, após escovação simulada com dentifrícios branqueadores contendo blue covarine nos tempos 6, 12 e 24 meses. Material e método 80 corpos de prova (CP) foram divididos em 8 grupos (n = 10): grupo controle (GC-água) e 7 grupos teste (GT1-Colgate Total 12, GT2-Oral-B 3D White Perfection, GT3-Colgate Luminous Instant White, GT4-Close Up White Diamond Attraction, GT5-Close Up White-Glacier Fresh, GT6-Sorriso Xtreme White, GT7-Colgate Luminous White Advanced). Os CP foram escurecidos com café e submetidos à escovação simulada por 6, 12 e 24 meses. As avaliações de cor, rugosidade e massa foram realizadas após cada período de escovação. Resultado Nenhum dentifrício teste promoveu alterações significativas na rugosidade e massa dos CP, contudo, quando comparados ao GC todos promoveram branqueamento significativo. A comparação dos grupos teste não demonstrou diferença significativa na variação de cor entre GT1, GT2, GT3, GT5, GT6 e GT7 nos tempos estudados. O GT4 apresentou comportamento variável de acordo com o tempo, não diferindo estatisticamente de GT3 aos 6, 12 e 24 meses, de GT6 aos 6 e 12 meses e de GT7 aos 12 meses. Conclusão O efeito branqueador dos dentifrícios contendo blue covarine , após 2 anos de escovação, parece estar relacionado principalmente com a sua associação aos agentes branqueadores mecânicos, que promoveram o polimento das superfícies dentárias.

Objective: This randomized clinical trial evaluated the efficacy and safety of a blue-covarine whitening toothpaste on tooth bleaching. Subjects and methods: Seventy-five subjects with shade mean C1 or darker were randomized into three groups (n = 25): CT-conventional toothpaste, WT-whitening toothpaste, and CP10-10% carbamide peroxide. Subjects from CT and WT brushed their teeth twice/day for 2 weeks. CP10 used bleaching gel for 4 hours/night for 2 weeks. Tooth shade and CIELab parameters were measured at baseline, after the first application, 2-and 4-week. ΔE* ab and ΔE 00 were calculated. Subjects' perception about tooth color appearance, tooth sensitivity (TS), and gingival irritation (GI) were also assessed. Results: At all evaluation periods, it was not observed differences between WT and CT considering tooth shade (P > 0.7) or CIELab parameters (P > 0.3). At 2-and 4-week, ΔE* ab and ΔE 00 were higher for CP10 than WT or CT (P = 0.001). WT and CT reported major dissatisfaction with tooth color appearance than CP10 (P = 0.001). At 1-and 2-week, WT experienced GI and TS similar to CT, both lower than CP10 (P < 0.01). Conclusion: There were no significant differences in the whitening efficacy between whitening and conventional toothpastes. Neither of the dentifrices was as effective as at-home bleaching. Clinical significance: The silica-based toothpaste containing blue covarine evaluated in this study did not give significant tooth whitening benefits over 2 weeks of use. K E Y W O R D S clinical trial, tooth bleaching, tooth discolouration, toothpastes 1 | INTRODUCTION Tooth bleaching is still the most requested treatment in dental offices because it has been considered an effective and conservative way for tooth whitening. 1,2 However, the side effects frequently related to athome or in-office bleaching techniques are the gingival irritation and/or tooth sensitivity. 3,4 To promote the same benefits achieved with these techniques with lower incidence of adverse effects, several companies have developed and improved the over-the-counter (OTC) at-home bleaching products to make the treatment more accessible and to meet patients expectations. 5-8 The OTC products usually contain low levels of hydrogen peroxide (2%-6%) and include gels, rinses, gum, toothpastes, whitening strips, or paint-on films. 9-11 The action mechanisms of the OTC products are based on the bleaching of intrinsic stains using oxidizing agents to break down the organic molecules of pigments present in the teeth, and the control and removal of extrinsic stains through the abrasive agents. 12-14 Whitening toothpastes, which represent more than 50% of the OTC products, often contain optimized abrasives that may be associated to enzymes, surfactants, hydrogen peroxide (limited to about 1 wt%),
