OBJECTIVE: To characterize complementary feeding and to analyze the influence of individual and contextual factors on dietary practices of low birth weight infants. METHODS: This cross-sectional study included 2,370 low birth weight infants aged 6 to 12 months included in the Breastfeeding Prevalence Survey in Brazilian Municipalities (2008), which covered the 26 state capitals, the Federal District and 37 municipalities. Dietary practices were assessed using two indicators: I) dietary diversity, characterized by the consumption of five food groups: meat, beans, vegetables, fruit and milk; II) consumption of ultra-processed foods, characterized by the ingestion of at least one of the following foods on the day prior to the survey: soda, or processed juice, or cookie, cracker and crisps. The covariates of interest were the socioeconomic characteristics of infants, mothers and health services. The contextual factor was the “municipal prevalence of child undernutrition.” The individualized effect of the study factors on outcomes was assessed by multilevel Poisson regression. RESULTS: Approximately 59% of infants consumed ultra-processed foods, while 29% had diverse feeding. Mothers living in municipalities with child undernutrition prevalence below 10%, with higher education and working outside the home were more likely to offer dietary diversity. Consumption of ultra-processed foods was higher among infants living in municipalities with child undernutrition prevalence below 10%, whose mothers were younger and multiparous. CONCLUSIONS: The low prevalence of diverse feeding combined with the high prevalence of ultra-processed food consumption characterizes the low quality of feeding of low birth weight Brazilian infants. Individual and contextual factors impact the feeding quality of this population, suggesting the need for effective strategies to increase the consumption of fresh and minimally processed foods and decrease the consumption of ultra-processed foods by this vulnerable population.

Este estudo objetivou conhecer o perfil epidemiológico de puérperas e neonatos atendidos em um hospital de referência. Trata-se de um estudo transversal quantitativo realizado em um hospital público, onde foram analisados o cartão de gestante e os prontuários das puérperas e do recém-nascido. Participaram 457 puérperas, destas, 51,4% tinham idade entre 17 e 26 anos, 85,4% eram casadas/ união estável, 39,6% eram primigestas e 64,6% tiveram parto normal; e 461 neonatos, destes, 84,2% eram a termo e tiveram Índice de Apgar maior que 7 e 8 no 1º (92,2%) e no 5º (94,8%) minuto de vida. Baixo peso ao nascer e prematuridade foram constatados em 11,3% e 9,1% dos neonatos, respectivamente. A pesquisa permitiu traçar o perfil epidemiológico de puérperas e de seus neonatos, possibilitando a compreensão da interação dos fatores de risco e a reorganização de estratégias de serviços mais efetiva.AbstractThis study had the objective of to trace the epidemiological profile of puerperae and neonates treated in a referral hospital. A quantitative cross-sectional study was carried out in a public hospital, where the pregnant woman’s card, the medical records of the puerperae and the newborn were analyzed. 457 postpartum women participated, 51.4% were between 17 and 26 years of age, 85.4% were married / stable, 39,6% were primigravidae and 64.6% had normal births; and 461 neonates, of whom 84.2% were at term and had an Apgar score greater than 7 and 8 in the 1st (92.2%) and in the 5th (94.8%) minute of life. Low birth weight and prematurity were found in 11.3% and 9.1% of neonates, respectively. The research allowed us to trace the epidemiological profile of puerperal women and their newborns, enabling the understanding of the interaction of risk factors and the reorganization of more effective service strategies.

The birth weight of newborn is an indicator that reflects maternal health conditions and the healthcare quality delivered during pregnancy. The aim of this study is to analyze the association between maternal consumption of cigarette during pregnancy and Low Birth Weight (LBW) in the Northeast of Brazil. Case-control study with the group of cases composed of 402 mothers with newborns with a birth weight <2,500 grams, and control composed of 1,210 mothers with newborns with weight ≥2,500 grams. Data were collected based on self-reported information by mothers and complemented with hospital records. Simple linear regression and logistic regression models were conducted. Majority of women were brown/ black, married, of low income and almost half of them had less than nine years of education. The consumption of 10 or more cigarettes a day was associated with LBW. For each cigarette added to the daily cigarette consumption there was a reduction of 18.6 grams in birth weight. Among mothers who smoked more than ten cigarettes per day, the mean reduction in birth weight was 283.1 grams. This study reinforces that LBW is associated with the maternal consumption of cigarettes.
