Precision non-implantable neuromodulation therapies: a perspective for the depressed brain

Abstract: Current first-line treatments for major depressive disorder (MDD) include pharmacotherapy and cognitive-behavioral therapy. However, one-third of depressed patients do not achieve remission after multiple medication trials, and psychotherapy can be costly and time-consuming. Although nonimplantable neuromodulation (NIN) techniques such as transcranial magnetic stimulation, transcranial direct current stimulation, electroconvulsive therapy, and magnetic seizure therapy are gaining momentum for treating MDD, the… Show more

“…Non-invasive methods include electroconvulsive therapy (ECT), repetitive transcranial magnetic stimulation, and transcranial direct current stimulation. 16 As depicted in Figure 1 , invasive techniques include vagus nerve stimulation (VNS), deep brain stimulation (DBS), and epidural cortical stimulation (ECS). 17 While the efficacy of ECT has been demonstrated since its early days, some patients still do not achieve remission and may present cognitive complaints, despite the refinement of the technique in terms of effectiveness and safety.…”

“…In addition, dietary capsaicin confers the prevention of depression via regulation of the monoamine transmitter by improving the gut microbiota dysbiosis (12)(13)(14). However, non-invasive brain stimulation (NIBS) is one of the fastest-growing fields in the treatment for major depression (15), and it refers to a set of techniques used to modulate brain activity using non-implantable methods, including electroconvulsive therapy and transcranial magnetic stimulation (TMS) (16,17).…”
