Patients with serious mental illness have a higher risk of hepatitis C virus (HCV) infection but suboptimal HCV care. The current study aimed to facilitate HCV treatment uptake by implementing an integrated outreach care model. Multidisciplinary outreach screening followed by HCV reflex testing and onsite treatment for schizophrenia patients was accomplished through the coordination of nongovernmental organizations, remote specialists, and local care providers. The objective was microelimination effectiveness, defined as the multiplication of the rates of anti‐HCV antibodies screening, accurate HCV RNA diagnosis, treatment allocation, treatment completion, and sustained virological response (SVR12; no detectable HCV RNA throughout 12 weeks in the post‐treatment follow‐up period). A total of 1478 of the 2300 (64.3%) psychiatric patients received HCV mass screening. Seventy‐three (4.9%) individuals were seropositive for anti‐HCV antibodies. Of the 73 anti‐HCV seropositive patients, all (100%) received HCV reflex testing, and 29 (37.7%) patients had HCV viremia. Eight patients (34.8%) had advanced liver disease, including 3 with liver cirrhosis and 2 with newly diagnosed hepatocellular carcinoma. Twenty‐three of the 24 (95.8%) patients who stayed in the healthcare system received and completed 8 weeks of glecaprevir/pibrentasvir treatment and post‐treatment follow‐up without significant DDIs or adverse events. The SVR12 rate was 100%. The microelimination effectiveness in the current study was 61.6%. Individuals with serious mental illness are underserved and suffer from diagnostic delays. This patient‐centered and integrated outreach program facilitated HCV care in this marginalized population.

Neste trabalho foram realizadas entrevistas sobre o conhecimento acerca da hepatite C com pacientes e acompanhantes em atendimento nos ambulatórios do Cemed, localizado na Universidade Anhanguera Uniderp, em Campo Grande, Mato Grosso do Sul, no período de abril a agosto de 2021. Foi realizado um levantamento bibliográfico nas plataformas PubMed, LILACS, portal de periódicos da Capes, Scielo e Cochrane, construindo-se um roteiro para a entrevista, contendo 5 questões, com total de 14 perguntas, abrangendo agente etiológico, transmissão, sintomas, diagnóstico, prevenção e tratamento da doença. O objetivo do trabalho foi verificar e identificar o nível de conhecimento desses participantes sobre a hepatite C. Os resultados mostraram que há carência de informação da população, sobretudo no que se refere ao patógeno relacionado, ao órgão afetado e às formas de contaminação, além de confusão em relação à hepatite A sobre os meios de transmissão. Em razão disso, fica evidente a necessidade de medidas educativas, de forma a prevenir a patologia, diminuir a morbimortalidade e os custos com a saúde pública decorrentes de complicações advindas da doença.

Educational innovations in the approach of outpatients to improve health literacy

Objective: To propose new approaches to improve patients’ health literacy to the treatment for functional dyspepsia and gastritis. Methods: This is a prospective study with a pre- and post-educational intervention. Patients had their first consultation with the physician and answered the pre-educational intervention questionnaire. Guidance texts and videos were sent to the patients for 3 weeks after the consultation. Finally, patients return to the consultation with the physician and answered to the second questionnaire after the educational intervention. Results: The sample included 50 patients, 35 female patients (70%). The average age was 55.7 years (15.3 SD). In the post-intervention questionnaire, 82% of patients improved their clinical condition with the treatment, 44% recognized that dietary changes are important for symptoms improvement, 98% of patients understood the videos, 74% of patients believe that the educational intervention could be used for other specialties and 90% of patients would recommend this method for other specialties. Conclusion: Patient education is an important strategy for a better understanding of their disease and, possibly, improves adherence to their treatment. Besides that, this study used educational tools through guidance texts and educational videos that can be used in clinical practice. This is an easily, accessible and inexpensive tool for health literacy.
