Background/objectivesIn Brazil, the increase in obesity rates has been accompanied by increased consumption of ultra-processed food (UPF). The objective of this paper was to evaluate body mass index (BMI) and body fat percentage (%BF) trajectories in adolescents over a 3-year follow-up according to the frequency of UPF consumption.Subjects/methodsData of three consecutive years (2010, 2011, and 2012) were obtained from the Adolescent Nutritional Assessment Longitudinal Study (ELANA) that aimed to assess changes in anthropometric indicators of nutritional status, and 1035 adolescents enrolled in the 1st year of high school from six schools (four private and two public) in the metropolitan area of Rio de Janeiro, Brazil were included. At three follow-ups, they had their weights and heights measured. Body composition was measured at the first and second follow-ups. Mixed linear regression models were used to estimate BMI and %BF trajectories based on quartiles of UPF intake, adjusting for type of school, sex, physical activity, and underreporting.ResultsCompared to their counterparts in the 1st quartile, adolescents in the 4th quartile of UPF consumption had a lower daily intake of fruits, cooked vegetables, and raw vegetables and a higher intake of total sugar and physical activity levels (p < 0.001). There was an inverse association between UPF consumption and BMI both at baseline and at follow-up. Values for %BF followed the same trend. Adolescents in the 4th quartile had the greatest level of physical activity and lowest total energy intake.ConclusionThis study confirmed that greater intake of UPF is a marker of an unhealthy diet, but did not support the hypothesis of a high rate of change in BMI associated with greater UPF consumption, even after adjusting for physical activity.

Introduction: A valid food frequency questionnaire (FFQ) is a priority for epidemiological studies. We aimed to perform a relative validity of an FFQ for Portuguese adolescents. Methods: A semi-quantitative Portuguese FFQ (SQ-PortFood-FQ) was developed and the relative validity performed using the multiple-pass 24-h recall. Eighty adolescents (aged 10–17 years) completed both questionnaires to measure energy intake, macro- and specific micronutrient intake. We used the following methods for the relative validity: (1) Spearman’s correlation coefficient (r); (2) cross-classification into the same third (agreement) and into the opposite third (disagreement); (3) weighted κ-coefficient (κW); (4) and the Bland-Altman method. Results: We obtained acceptable correlation coefficients between questionnaires for energy (r =0.59; p < 0.001), protein (r =0.60; p< 0.001), fat (r = 0.55; p < 0.001), carbohydrates (r =0.51; p < 0.001) and sodium (r =0.51; p < 0.001). Tertile agreement for energy, protein, fat, carbohydrates and vitamin C intakes classified correctly more than 50% and grossly misclassified less than 10% of participants. Energy and protein produced moderate agreement between methods with a κW >0.40. The SQ-PortFood-FQ overestimated all analysed data when compared to the 24-h recall, although those estimations were <10% for energy, macronutrients and cholesterol. The 95% limits of agreement showed that all the variables (energy, protein, fat, carbohydrates, fibre, calcium, sodium, iron, saturated fatty acids, monounsaturated fatty acids and cholesterol) were within the desirable limits of agreement of 50–200%, with the exception of vitamin C (76–296%) and polyunsaturated fatty acids (PUFA) (75–232%). Discussion/Conclusion: The SQ-PortFood-FQ showed acceptable relative validity at the individual level for energy, protein, fat, carbohydrates, fibre, calcium, sodium, iron, saturated fatty acids, monounsaturated fatty acids and cholesterol. Non-significant relationships were found for vitamin C and PUFA. Our results are in line with previous studies carried out amongst adolescents. As far as we know this is the first FFQ validated for Portuguese adolescents.

Estudos sobre padrões alimentares dependem da validade dos métodos, como o questionário de frequência alimentar (QFA), para estimar consumo alimentar de longo prazo, que pode ser superestimado e o método precisa ser validado. Questiona-se se estudos de validação aninhados a estudos com desenho amostral complexo (DAC) deveriam ignorar a dependência dos dados. Este estudo objetiva validar o QFA aplicado a adolescentes escolares e estimar os fatores de calibração em um estudo com amostragem complexa. A validade do QFA é testada frente a registros alimentares (RA) aplicados a 85 adolescentes em um estudo com DAC, com estimação dos fatores de calibração (λ) para energia, nutrientes e grupos alimentares (GA). Coeficiente de correlação intraclasse foi calculado; diferenças médias foram comparadas via modelagem, e foram realizadas análises de concordância. As médias entre os métodos diferiram para energia e nutrientes (p<0,05), mas quatro GA foram similares. Limites de concordância não se mostraram aceitáveis para os nutrientes. Coeficientes de correlação de concordância de fibras, zinco e ácidos graxos, ajustados pela energia, variaram de 0,16 a 0,22 e para os GA variaram de 0,07 a 0,36. Os fatores de atenuação corroboraram as diferenças (λ<0,40). O QFA apresentou validade relativa, sobretudo para os GA e λ aproximou as estimativas do QFA ao RA, sendo relevante quando nutrientes são usados em análises epidemiológicas. O ajuste das análises pelo DAC confirmou que a estrutura de dependência dos dados deve ser considerada, mesmo em estudos de validação, devendo-se evitar tratar tais dados como se fossem oriundos de amostra aleatória simples.
