Níveis de engagement em profissionais da atenção primária à saúde: estudo comparativo em dois municípios brasileirosNiveles de engagement en profesionales de la atención primaria a la salud: estudio comparativo en dos municipios brasileños AbstrAct Objective: To evaluate the levels of engagement of Primary Health Care professionals in two municipalities. Methods: A descriptive study was carried out with 238 health professionals who work in the Primary Care services of two municipalities in the state of São Paulo. The engagement dimensions of the professionals were investigated through the Utrecht Work Engagement Scale. Results: Female predominance (82.8%), age under 40 years (52.9%). Median working time in primary care of three (municipality A), four years (municipality B). Significant difference in levels of engagement in both municipalities. Community health agents presented mean scores in all dimensions. Significant difference in levels of engagement according to professional category in all dimensions. Conclusion and implications for practice: Municipal health professionals with 100.0% coverage by the Family Health Strategy tend to have higher levels of engagement. Nurses presented greater engagement; community health workers reached lower levels than other professionals did. Engagement is an indicator that contributes to the evaluation of the workforce in the Primary Health Care services and can be used to direct strategies that improve the levels of dedication, absorption and vigor of the professionals, benefiting the organization of the primary care services. Objetivo: Avaliar os níveis de engagement de profissionais da atenção primária em saúde de dois municípios. Métodos: Estudo descritivo realizado com 238 profissionais de saúde que atuam nos serviços de Atenção Primária de dois municípios do estado de São Paulo. As dimensões de engagement dos profissionais foram investigadas através da Utrecht Work Engagement Scale. Resultados: Predomínio do sexo feminino (82,8%), idade inferior a 40 anos (52,9%). Tempo mediano de atuação na atenção primária de três (município A), quatro anos (município B). Diferença significante dos níveis de engagement nos dois municípios. Agentes comunitários de saúde apresentaram escores médios em todas dimensões. Diferença dos níveis de engagement segundo categoria profissional significante em todas dimensões. Conclusão e implicações para a prática: Profissionais de município com 100,0% de cobertura pela Estratégia Saúde da Família tendem a ter maiores níveis de engagement. Enfermeiros apresentaram engagement maior; Agentes comunitários de saúde alcançaram níveis mais baixos que os demais profissionais. O engagement é um indicador que contribui para a avaliação da força de trabalho nos serviços de Atenção Primária à Saúde e pode ser empregado para direcionar estratégias que melhorem os níveis de dedicação, absorção e vigor dos profissionais, beneficiando a organização dos serviços de atenção primária. Resumo: A atenção primária à saúde (APS) concebida como coordenadora do cuidado e ordenadora da rede regionalizada de atenção à saúde (RAS) é um dos condicionantes da dinâmica regional da saúde. O objetivo do artigo é identificar as dimensões política, de estrutura e de organização da APS em diferentes regiões do Brasil; assume-se que estas dimensões podem explicar, se não o todo, pelo menos parte importante do funcionamento da APS em cenário regional. Foram realizadas 84 entrevistas com atores-chave em cinco regiões de saúde. Essas regiões foram selecionadas com base na diversidade de situações socioeconômicas, territoriais e de organização do sistema de saúde. Apesar da heterogeneidade das RAS, notam-se traços comuns. Na dimensão da política, observou-se fragilidade na cooperação intergovernamental e no protagonismo da esfera estadual, além da incapacidade da Comissão Intergestores Bipartite em se configurar como espaço de planejamento e pactuação. Na dimensão estrutura ficou clara a insuficiência de condições que assegurem minimamente a execução de funções essenciais da APS. Pontos críticos são escassez, má distribuição e problemas de qualificação de recursos humanos, além do subfinanciamento das ações. Na dimensão organização são visíveis as dificuldades para se romper a fragmentação dos serviços. A APS não consegue assumir seu papel de coordenadora do cuidado, e observa-se a ausência de um modus operandi capaz de atender às necessidades dos usuários considerando as especificidades de cada região. A superação dos constrangimentos identificados é central para o fortalecimento do próprio SUS como sistema público, universal, equânime e integral.

This document expresses the position of the Primary Health Care Research Network (PHC Network) of the Brazilian Association of Collective Health (Abrasco) in a context of threats to the principles and guidelines of the Brazilian Unified Health System (SUS) and the Family Health Strategy (FHS), aggravated in the last two years with the democratic rupture in the Country. The document is signed by the group of researchers of the PHC Network and contains propositions to compose a strategic political agenda for the SUS, main object of the debate held during the XII Brazilian Congress of Collective Health in Rio de Janeiro in July 2018. In this year of 2018 we celebrated 30 years of SUS, the culminating moment of the Brazilian Sanitary Reform Movement and one of the milestones in the redemocratization process of the Country. The SUS was created by the Citizen Constitution of 1988, which established 'Health as a right of all and duty of the State'. From this milestone, Brazil has established health as a fundamental right, through a universal public system and the interaction between social and economic policies 1. This year, we also celebrate the 40th anniversary of the Declaration of the International Conference on Primary Health Care held in 1978 in the city of Alma Ata, Kazakhstan, in the former Union of the Soviet Socialist Republics (USSR), under the auspices of the World Health Organization (WHO) 2. Since Alma Ata, and its slogan of 'Health for all by the year 2000', many initiatives for the implementation of PHC have been undertaken worldwide with different conceptions and approaches: from very selective proposals of minimum baskets to populations in extreme poverty, 'poor medicine for the poor', to integral PHC as the basis of universal public health systems. These approaches have produced different results on the organization of health systems, the right to health and citizenship. Selective PHC corresponds to a conception of residual citizenship, and the integral PHC of the universal systems corresponds to full citizenship 3. To mark the 40th anniversary of the Alma Ata Declaration, WHO will hold the Global Conference on PHC in Astana, capital of Kazakhstan, in October 2018. At that time, the Astana Declaration will be signed, whose preliminary versions indicate a setback in the defense of integral PHC 4. The versions in consultation caused concern to the scientific community in Abrascão 2018, by proposing the Universal Health Coverage without emphasizing the relevance of integral PHC and universal health systems in guaranteeing the full right of citizenship and equitable access to health services according to need. Among the critical points of the proposal, there is little emphasis on guaranteeing the universal human right to health and the segmentation inherent in Universal Health Coverage, to be achieved through insurance
