RESUMO: Objetivou-se identificar fatores associados ao tabagismo em usuários da estratégia saúde da família. Estudo epidemiológico transversal, descritivo e analítico. A amostra por conveniência compreendeu 100 indivíduos usuários de uma Unidade Saúde da Família de um município da região centro sul do Paraná. Houve predomínio das mulheres (n=63; 63%), com média de idade de 49,5 anos, cor branca (n=82; 82%), com ensino fundamental completo (n=21; 21%) e casados (n=49; 49%). O consumo médio diário de cigarro foi de 17,7 cigarros/ dia e 56 (56%) participantes apresentavam grau de dependência muito elevado ou elevado à nicotina. Constatouse associação entre a média de tempo de tabagismo e idade (p=0,001), estado civil viúvo (p=0,035), colesterol elevado (p=0,035), uso de anti-hipertensivo (p=0,031) e grau de dependência a nicotina muito elevada (p=0,008). A identificação dos fatores associados ao tabagismo possibilita à equipe de saúde buscar e planejar estratégias de prevenção, tratamento e acompanhamento dos tabagistas. FATORES ASSOCIADOS AO TABAGISMO EM USUÁRIOS FACTORS ASSOCIATED WITH SMOKING AMONG USERS OF THE FAMILY HEALTH STRATEGY ABSTRACT:The aim was to identify factors associated with smoking among users of the Family Health Strategy. A cross-sectional, descriptive and analytic epidemiological study. The sample by convenience encompassed 100 individuals who were users of a Family Health Unit from a city in the center-south region of the Brazilian State of Paraná. There was a predominance of women (n=63; 63%), with a mean age of 49.5 years, Caucasian (n=82; 82%), who had completed junior high school (n=21; 21%) and who were married (n=49; 49%). Mean daily cigarette consumption was 17.7 cigarettes/day and 56 (56%) participants had a very high or high degree of dependence on nicotine. An association was observed between mean smoking duration and age (p=0.001), marital status -widowed (p=0.035), high cholesterol (p=0.035), use of antihypertensives (p=0.031) and very high degree of dependence on nicotine (p=0.008). The identification of the factors associated with smoking makes it possible for the health team to seek and plan prevention strategies, treatment and monitoring of smokers.

ABSTRACT:Objective: To describe the implantation of the Surveillance System for Noncommunicable Diseases (NCDs) in the Unified Health System (Sistema Único de Saúde) and the challenges in maintaining it. Methods: A literature review was carried out the information contained in federal government directives between 2003 and 2015 was consulted. Results: A comprehensive risk and protection factor surveillance system was implemented. It is capable of producing information and providing evidence to monitor changes in the health behavior of the population. Among the advances cited are the organization of epidemiological surveys, such as the Surveillance System for Risk Factors and Protection for NCD (Sistema de Vigilância de Fatores de Risco e Proteção para DCNT -Vigitel), the National School Health Survey (Pesquisa Nacional de Saúde do Escolar -PeNSE), and the National Health Survey (Pesquisa Nacional de Saude) from 2013, which enabled the most extensive health diagnosis of the Brazilian population. In 2011, the NCD National Plan 2011-2022 established targets for reducing risk factors and NCD mortality. Conclusion: The information gathered from the NCD surveillance system can support the implementation of sectoral and intersectorial strategies, which will result in the implementation of the Brazilian Strategic Action Plan for the prevention and control of NCDs, as well as the monitoring and evaluation of their results periodically. Finally, it can be a very important tool to help Brazil achieve the goals proposed by the 2030 Agenda for Sustainable Development and the Global Plan to Tackling NCDs.

Diverging incidence trends of oral tongue cancer compared to other head and neck cancers in young adults in France

While head and neck cancer incidence decreased worldwide due to reduced tobacco and alcohol consumption, oral tongue cancer (OTC) incidence has been reported to be increasing in several countries. Our study examines the incidence trends of OTC in France from 1990 to 2018, globally and by age; and compares the incidence trends with the evolution of the incidence of other human papilloma virus‐unrelated head and neck squamous cell carcinoma, that is, cancers of the remaining subsites of the oral cavity (RSOCC) and laryngeal cancers for the period 1990 to 2018. World age‐standardized incidence rates of oral tongue cancers (C02), cancers of the remaining subsites of the oral cavity (RSOCC, C03‐06) and laryngeal cancers (C32) were estimated using the French National Network of Cancer Registries for the period 1990 to 2018. Trends in national incidence rates were estimated from a mixed‐effect Poisson model including age and year effects using penalized splines and a district‐random effect. In women aged 30 and 40, a significant increase in OTC incidence was observed, while ROSCC showed a nonsignificant incidence decrease. In young men aged 25, a marginally significant increase of OTC incidence years was observed, while incidence rates of RSOCC significantly declined. The results suggest a tendency towards diverging incidence trends for OTC compared to RSOCC and laryngeal cancer in young adults. The observed trends may reflect changes in underlying exposures or emerging exposures not yet identified, and stress the need to further investigate the etiology of oral tongue cancers.
