Biological Enhancements for Anterior Cruciate Ligament Reconstruction

Abstract: The anterior cruciate ligament (ACL) is mostly responsible for providing knee stability. ACL injury has a marked effect on daily activities, causing pain, dysfunction, and elevated healthcare costs. ACL reconstruction (ACLR) is the standard treatment for this injury. However, despite good results, ACLR is associated with a significant rate of failure. In this context, the mechanical and biological causes must be considered. From a biological perspective, the ACLR depends on the osseointegration of the graft in… Show more

“…The anterior cruciate ligament (ACL) ensures proper stabilization of the knee and is a passive limiter of excessive translation of the anterior tibia. ACL injury causes pain, which significantly reduces the patient’s quality of life [ 58 ]. Despite many methods of treatment, surgical reconstruction of the ACL has remained the gold standard in the treatment of patients with ACL trauma for many years [ 59 ].…”

“…The above‐mentioned process is what usually occurs in “tendon to tendon” healing (Thomopoulos et al, 2015). In contrast, when “tendon to bone” healing occurs (as observed after the reconstruction of the anterior cruciate ligament [ACL]), an increased density of Sharpey‐like fibers is observed, and a disorganized collagen structure is produced, binding the tendon to the bone (C. B. G. Leite & Demange, 2019; Thomopoulos et al, 2015). In this setting, osteoclasts are also recruited to the injured site, triggering bone resorption and improving tendon attachment (Zelzer et al, 2014).…”

“…Besides the attempts for an optimal recuperation of the joint function, the knee healing process is not always satisfactory. Thus, different therapeutic approaches have been suggested to improve knee healing capacity, including biological and biophysical interventions (C. B. G. Leite & Demange, 2019). Among the different strategies, Hyperbaric oxygen therapy (HBOT) arises as a new potential biophysical adjunctive therapy for knee injuries.…”

“…Биологическую несостоятельность трансплантата ПКС можно подозревать у пациентов с рецидивирующей нестабильностью в коленном суставе при условии исключения других причин (до 17 % случаев) [42]. Биологическая интеграция трансплантата протекает через стадии асептического воспаления, включая этап реваскуляризации и ремоделирования его структуры [43].…”
