Objective: To understand how the nursing staff perceives the care provided to people in situations of psychiatric urgencies and emergencies in the Mobile Emergency Care Service (SAMU – Serviço de Atendimento Móvel de Urgência). Method: Descriptive and qualitative study conducted in the Northeast region of Brazil with 34 of the SAMU nursing workers. Data were obtained by semi-structured interviews and processed by the Thematic Analysis. Results: The analysis of interviews allowed the identification of three categories: mechanical practice, need for qualification and (de)humanization of care. The results showed that the care offered to users in psychiatric urgency or emergency situations is based on mechanistic and specific actions. Final considerations: Nursing workers perceive that the care for people in situations of psychiatric urgency and emergency in SAMU is mainly based on physical and chemical containment measures, performing a little resolute and dehumanized care and raising the need for professional qualification.

RESUMO Objetivo: analisar os atendimentos de natureza psiquiátrica, realizados pelo serviço pré-hospitalar móvel de urgência. Método: estudo retrospectivo com análise dos registros de atendimentos de natureza psiquiátrica, realizados pelo Serviço de Atendimento Móvel de Urgência de uma capital do Nordeste brasileiro no ano de 2014. Resultados: foram realizados 38.317 atendimentos pelo Serviço de Atendimento Móvel de Urgência, sendo que 1.088 (2,8%) eram psiquiátricos. A amostra foi composta, em sua maioria, por usuários do sexo masculino (64,8%), na faixa etária de 20 a 59 anos (81,4%), com média de idade de 35 anos. Houve predomínio de quadros de agitação e/ou agressividade (65,7%) e 8,0% dos usuários encontravam-se sobre suspeita de uso de bebidas alcoólicas. As Unidades de Suporte Básico foram acionadas em 96,8% das ocorrências e 91,6% dos usuários foram encaminhados para o hospital psiquiátrico. Ressalta-se a ocorrência de tentativas e ideações suicidas em 7,6% dos atendimentos, com maioria do sexo feminino (54,9%). Observou-se associação significativa entre o tempo de atendimento e as variáveis: motivo do atendimento (p=0,003), realização de procedimentos (p=0,000) e uso de medicação (p=0,000). Conclusão: o estudo mostrou elevado número de atendimentos de natureza psiquiátrica realizado pelo Serviço de Atendimento Móvel de Urgência, evidenciando sua importância como um dos componentes da Rede de Atenção Psicossocial. Entretanto, o destino dos usuários ainda continua sendo o hospital psiquiátrico.

The present study is an experience report that analyzes the perception of the medical students of Unigranrio, Barra da Tijuca, regarding the performance of the psychiatrist, in the field of specialty practice in primary care and in the psychiatric emergency service, and in relation to the insertion of the non-teaching of medical education, as well as basic themes in mental health, highlighting the feelings about the contact and approach of the patient with mental disorders and self-perception of the medical student's mental health, allowing reflections on the problem of existing non-teaching doctor. It carried out an evaluation in order to bring contributions to mental health education, detecting the presence of erroneous concepts, stereotypes and perceptions related to the subject in the formation of the student, at the same time as it discussed the need of forms of intervention to overcome challenges that involve obstacles and methodologies of mental health teaching in the medical course. The project leads to reflections on teachers and students, the possibility of mobilization at improving the future formation of the doctor, in a view of the magnification and relevance of psychiatry teaching. The construction of the project made it possible to highlight a different perspective for insertion and expansion of mental health to medical students, and for analysis of the Psychiatric Reform process, as well as questions about the need for changes in the medical curriculum formation, about concepts, sensations, misconceptions, prejudices, fears and anxieties related to the care of the psychiatric patient.
