Abstract

Intervenções em saúde pública e seu impacto nas desigualdades sociais em saúde Introdução A avaliação de políticas sociais, em geral, e das intervenções em saúde pública, em especial, é complexa e envolve diferentes parâmetros. Fala-se em estudar a eficácia, a efetividade e a eficiência dos programas de saúde, confrontar custos e benefícios, avaliar processos e resultados em condições reais e em condições ideais. Espera-se que as intervenções em saúde pública sejam efetivas, isto é, que promovam melhores condições de vida e reduzam a carga de doenças. As intervenções na área também devem ser avaliadas quanto a seu possível impacto, favorável ou não, sobre as políticas de promoção de equidade entre os estratos sociais.Em 2000, César Victora et al. (2009) formularam a hipótese da equidade inversa, que pode afetar as iniciativas de promoção da saúde, e forneceram uma importante diretriz para a avaliação dos programas. Segundo sua formulação original, quando as intervenções de saúde pública são implantadas, elas atingem inicialmente os estratos sociais de melhor condição econômica e só posteriormente se expandem aos mais pobres. Com isso, melhorias globais nos índices de saúde seriam obtidas de modo associado ao incremento da desigualdade na experiência de doença entre os estratos sociais.Medidas de boa qualidade contribuem para reduzir a carga global de doença na população, mas podem ter como efeito indesejado o aumento da desigualVol27n1.indd 161
