Clinicopathological features and prognosis of patients with gastric neuroendocrine tumors: A population‐based study

BackgroundDespite its rarity, studies have shown the incidence of gastric neuroendocrine tumors (G‐NETs) is increasing. This study investigated the risk factors affecting the survival of G‐NETs patients and their prognosis over time.MethodA retrospective analysis of 506 G‐NETs patients who underwent surgery for nonmetastatic disease from the Surveillance, Epidemiology and End Result database from 1988 to 2011 was conducted. Multivariate Cox regression analyses identified the prognostic factors affecting overall survival (OS) and disease‐specific survival (DSS). Three‐year conditional survival (COS3 and CDS3) estimates at “x” year after treatment were calculated as follows: COS3 = OS(x + 3)/OS(x) and CDS3 = DSS(x + 3)/DSS(x).ResultsThe 1‐, 3‐, and 5‐year OS rates of all patients after surgery were 90.2%, 77.3%, and 68.8%, respectively. The 1‐, 3‐, and 5‐year DSS rates after surgery were 93.9%, 84.5%, and 80.9%, respectively. In the multivariate analysis, age, tumor grade, and T stage were independent prognostic factors of OS and DSS (all P < 0.05). With 1‐, 3‐, and 5‐year survivorship, the COS3 improved by +5.2 (82.2%), +7.2 (84.4%), and +8.5 (85.5%), respectively, and the CDS3 improved by +4.4 (89.4%), +9.1 (94.1%), and +12.5 (97.5%), respectively. Notably, the CDS3 improved dramatically among patients with advanced stage disease (eg, N0 stage: 93.0%‐98.9%, Δ5.9% vs N1 stage: 52.0%‐95.7%, Δ43.7%).ConclusionFor G‐NETs patients, age, tumor grade, T stage, and N stage were the clinicopathological factors significantly associated with prognosis. There were excellent outcomes for most G‐NETs patients, with a CDS3 of greater than 90% across all independent prognostic factors after 5 years of survival.

Východiská: Nádory vychádzajúce z neuroendokrinných buniek sú definované ako epitelové neoplázie s predominantne neuroendokrinnou diferenciáciou. Ide o samostatnú skupinu ná-dorov s charakteristickou histologickou stavbou a funkčnými vlastnosťami vyskytujúcimi sa na rôznych miestach v organizme, najmä v oblasti gastrointestinálneho systému (67 %) a pľúc (25 %). Aj keď ide obyčajne o pomaly rastúce nádory s indolentným priebehom, takmer všetky z nich majú malígny potenciál. Väčšina z nich má schopnosť tvoriť aktívne hormonálne látky. Spektrum klinických príznakov je rôznorodé, mnohé sú typické pre konkrétnu lokalitu neuroendokrinného nádoru. Aj keď tieto nádory poznáme > 130 rokov, koncept týkajúci sa klasifikácie stále nie je jednotný. Cieľ: Prehľadová práca poskytuje v úvodnej časti ucelený pohľad na neuroendokrinný systém človeka a nádory z neho vychádzajúce od ich objavenia až po v súčasnosti platnú terminológiu a klasifikácie. V druhej časti je pozornosť upriamená na nádory z endokrinných buniek gastrointestinálneho traktu, na ich klinickú symp tomatológiu, makroskopický a mikroskopický obraz s dôrazom na ich klasifikáciu, dia gnostické kritériá pri určovaní gradingu a TNM (tumour, node, metastasis) stagingu nádorov a na špecifiká v jednotlivých lokalitách zažívacieho traktu. Záver: Nádory z neuroendokrinných buniek predstavujú zriedkavé ochorenia, ktoré sa môžu klinicky prejaviť typickým obrazom karcinoidného syndrómu. Väčšia časť nádorov je však asymp tomatických, čo spolu s ich obyčajne malými rozmermi a lokalitou v oblastiach čreva ťažko dostupných endoskopickému vyšetreniu často vedie k dia gnostike v pokročilom štádiu ochorenia. Základom úspešnej dia gnostiky a liečby nádorov vychádzajúcich z neuroendokrinných buniek je myslieť na ne v rámci diferenciálne-dia gnostického postupu s exaktnou histopatologickou klasifikáciou, gradingom a stagingom podľa presne stanovených kritérií a najnovších klasifikácií a odporúčaní. V literatúre aj v odbornej verejnosti sa často používajú pojmy karcinoid, neuroendokrinný nádor, neuroendokrinní tumor či neuroendokrinný karcinóm synonymicky, no z hľadiska názvoslovia a klasifikácie je potrebné si osvojiť správnu terminológiu. BackgroundGastroesophageal Junction neuroendocrine neoplasms (GEJ‐NENs) are rare and heterogeneous tumors. We aim to analyze the clinicopathlogical features and prognostic factors of GEJ‐NENs and to compare the outcome of GEJ‐NENs with other gastric NENs.MethodsA total of 297 GEJ‐NENs patients were enrolled from 10 Chinese hospitals and 3152 gastric NENs patients, including 274 GEJ‐NENs, were retrieved from Surveillance, Epidemiology, and End Results (SEER) database.ResultsThe clinical characteristics of GEJ‐NENs among different races were different. All Chinese patients had GEJ‐NENs of grade 3, with 67.7% of poorly differentiated NEC and 32.3% of MANEC. In SEER database, 70.8% of white, 62.5% of black, and 87.5% of AP patients had poorly differentiated/undifferentiated tumors. In Cox multivariate analysis, NEC/MANEC (HR 2.09, 95%CI 1.24‐3.56; P = 0.006), lymph node metastasis (HR 3.52, 95%CI 1.68‐7.34; P = 0.001), and distant metastases (HR 3.90, 95%CI 2.50‐6.08; P < 0.001) are independent predictors of overall survival. Surgical resection showed a median OS improvement of 13.1‐73.3 months (HR 0.21, 95% CI 0.14‐0.33, P < 0.001). Adjuvant therapy did not improve survival for postoperative GEJ‐NEN patients (P = 0.141). GEJ‐NENs were larger, higher grade, more distant metastasis, and worse prognosis than other gastric NENs.Conclusion GEJ‐NENs were mostly poorly differentiated carcinomas, and all of Chinese patients were NEC/MANEC. The outcome of MANEC was preferable to NECs. Both lymph nodes metastasis and distant disease were independent predictors of prognosis. Surgical resection can improve survival, but postoperative adjuvant therapy had no additional benefit. GEJ‐NENs have worse survival than other gastric NENs.
