Study of carotid disease in patients with peripheral artery disease

Abstract: the study of the carotid arteries by duplex scan examination is of paramount importance in the evaluation of patients with symptomatic peripheral arterial disease, and should be systematically conducted in the study of such patients.

“…A Brazilian study found a prevalence of 84% of carotid stenosis in patients who already had peripheral arterial disease. In this same sample, carotid stenosis greater than 50% was present in 40% of patients and stenosis greater than 70% in 17% of them 22 . The treatment of extracranial carotid stenosis is usually composed of two strategies: clinical or surgical.…”

“…Most patients with extracranial carotid artery stenosis are asymptomatic and have their diagnosis through the identification of lesions on imaging tests such as vascular echography with Doppler 22 . Indications for treatment are made after imaging of the carotid bifurcation, which may be (1) for the neurologically symptomatic patient or (2) for the neurologically asymptomatic patient.…”

“…In other work, age, duration of diabetes, smoking habits, high blood pressure and an HbA1c > 7% were significant predictors of PAD, while obesity did not correlate with PAD [21]. Whereas, in a study by Bez and Navarro [10], of the patients with PAD investigated, 86% had hypertension, 66% exposure to tobacco smoke, 47% diabetes, 65% dyslipidemia, 24% coronary artery disease, 16% renal failure, and 60% a family history of cardiovascular disease [10]. It seems worth underlining at this point that in trying to explain such a high incidence of hypertension in patients with PAD (70% in our study of patients with normal creatinine levels) in comparison with other populations, the potential coexistence of renal artery stenosis (RAS) should be taken into account [14] because, in an analysis by Endo et al [22] of 410 Japanese patients with PAD, 22.9% had RAS > 50% and 11.0% had RAS > 75%.…”

“…They may overlap (polyvascular disease, multisite atherosclerosis), in which case the risk of cardiovascular event and mortality increases significantly [4,5,[9][10][11][12][13][14]. Sometimes, the respective types of vascular disease prevalence demonstrate a family association, which suggests that they may be related to composite domestic factors; not only genetic, but also, for example, active and passive smoking habits, diet, overeating, low physical activity, stress, alcohol abuse, etc.…”

“…Son muchos los estudios que relacionan la estenosis carotidea con determinados factores de riesgo como la hipertensión arterial, la diabetes, la dislipemia y el hábito de fumar (6)(7)(8)(9)(10)(11). Desde hace años se conoce la relación entre la presión arterial y el riesgo de desarrollo de ECA, de manera que por cada aumento de la presión arterial sistólica en 20 mm de Hg se duplica el riesgo de desarrollar esta patología (6).…”
