OBJECTIVE:To assess the quality of nursing care provided in inhalation, peripheral venipuncture, and administration of intra-muscular medication procedures in the context of professional training of nursing assistants. METHODS:A cross-sectional study was carried out in two phases among nursing staff (nurse assistants as well as unlicensed nursing personnel), in three hospitals in the state of Bahia, Northeastern Brazil, in October and December 2001 and 2002. Data was collected by means of direct observation of task performance and analysis of the median values of performance. A convenience sample was stratified according to professional category and work unit. RESULTS:There was a quality improvement or maintenance while performing the procedure of inhalation, with an overall median score equal to or above 70%. Median scores were reduced for peripheral venipuncture and administration of intra-muscular medication. Analysis of the differences in the procedures indicated that performance improved regarding basic procedural steps but worsened regarding the interaction with the patient. As to the nursing teams, those in Hospital 1, where there were better working conditions, had the best performance, and those in Hospital 2, where there were the worst working conditions, showed the worst performance. CONCLUSIONS:Educational processes among nurse assistants implemented per se in poor working conditions are not able to bring about quality improvement of professional performance.

RESUMO: Este trabalho teve por objetivos avaliar reações emocionais e sintomas referidos porfuncionários da Escola de Enfermagem da USP que receberam a vacina dT na região ventro-glútea e investigaro conhecimento e a aceitação da população quanto ao local de aplicação. Dos 38clientes, fo i necessário reiniciar o esquema vacinal em 50%; 13,1% receberam a2a dose, 2,6% a terceira eo restante o reforço . Dos 29 localizados para a entrevista, 22 manifestaram-se favoravelmente à utilização desta região em futuras injeções; 3 escolheriam tanto a região ventro-glútea, como a dorso-glútea, 1 prefere a dorso glútea e o outro a deltóide. Quanto aos demais, 1 utilizaria a região ventro-glútea, dependendo do aplicador, e a última continua indiferente ao local de aplicação.ABSTRACT: The aim ofthisstudy isto estimatethe emotional reactions and syr lptons told by staff members of scool of nursing of the Universidade de São Paulo, as they received the ventrogluteal application of the dT vaccine. It also intended to verify the population's knowledge about the site of the application and the aprovai levei showed by them. Doses of 0.5 ml of the vaccine were applied in 38 patients, 10 men and 28 woman. In 50% ofthe cases, itwas necessary to begin again the vaccinal scheme; 13.1 % and 2.6 were inoculated with the second and therd doses, respectively. From 38 vacinated, 29 were interviewed. Twenty two of them were favorable to the use of the ventrogluteal side in future injections; 3 would choose both the ventrog luteal and the dorsogluteal sites. One ofthem still prefers the dorsogluteal region and another, the deltoide site. With regard to the last two persons interviewed , one mentioned she would use the ventrogluteal site again, depending on the applyer and anotherdoesn't have any preference co ncerning the aplication site, even after this experience.

The object of this study is the organization of activities in the vaccination program in Ribeirão Preto, São Paulo, during the process of municipalization of health services, analyzing changes and estimating results. The methodology included a survey performed during supervisory visits and questionnaires with 20 nurses and 39 vaccination workers in all the vaccination rooms at the municipal primary health care unit in this city, a survey of storage conditions for immunobiological products at the municipal level and a questionnaire for the section head, and interviews with five health professionals responsible for vaccination at the State, regional, and municipal levels. It followed that since implementation of the so-called Integrated Health Actions proposal there has been an expansion of primary health care services, including vaccination services; there has also been a technical and administrative centralization of decisions involving vaccination activities, including training of human resources by the Municipal Epidemiological Surveillance division. The study points out that although this activity is dealt with carefully by those responsible for the State, municipal, and district levels, the same is not true at the local level.
