PURPOSE Early integration of pediatric palliative care (PPC) for children with cancer is critical to improving the quality of life of both the patient and family. Understanding physician perceptions of palliative care and perceived barriers to early integration is necessary to develop PPC in Brazil. METHODS The Assessing Doctors' Attitudes on Palliative Treatment survey was modified for use in Brazil. The survey was open from January 2022 to June 2022 and distributed to physicians of all specialties from participating institutions who treat children with cancer. Statistical analysis was complemented by qualitative analysis of open-ended responses. RESULTS A total of 272 respondents participated. Most respondents reported access to PPC experts for consultation (77.2%) and 34.5% indicated previous palliative care training. Physician knowledge of PPC was generally aligned with WHO guidance (median alignment, 93.0%; range, 80.5%-98.2%). However, about half (53.3%) felt comfortable addressing physical needs of patients receiving PPC, 35.3% addressing emotional needs, 25.8% addressing spiritual needs, and 33.5% addressing grief and bereavement needs. Most respondents (65.4%) felt palliative care should be involved from diagnosis, but only 10.3% stated that this occurred in their setting. The most important barriers identified were physician discomfort (89.0%), limited physician knowledge (88.6%), and lack of home-based services (83.8%). CONCLUSION Despite a strong understanding of the role of palliative care, physicians in Brazil reported low confidence delivering PPC to children with cancer. Additionally, physicians generally believed that PPC should be integrated earlier in the disease trajectory of children with cancer. This work will direct educational and capacity building initiatives to ensure greater access to high-quality PPC for children with cancer in Brazil to address patient and family suffering.

Introdução: O manejo da dor na paliação pela equipe interdisciplinar é considerado um grande desafio, visto que o conhecimento sobre o manuseio destes pacientes é pouco relatado. Deste modo, uma assistência interdisciplinar paliativa buscará proporcionar o alívio da dor, assim um conhecimento inadequado ocasionará uma diminuição da qualidade de vida dos pacientes. Neste sentido, o estudo buscará analisar as evidências científicas sobre o conhecimento da equipe interdisciplinar em relação ao manejo da dor nos cuidados paliativos. Métodos: Revisão integrativa da literatura dos anos de 2017 a 2022 com os descritores (“knowledge”) AND (“palliative care”) AND (“pain management”) AND (“interdisciplinary team”) nas bases de dados Web of Science, Scopus, Embase e Wiley Online Library via periódicos capes, contendo uma amostra de 7 estudos. Resultados: Na literatura foram destacadas que há o déficit de conhecimento da equipe interdisciplinar quando associado ao manejo da dor na paliação, ocasionando em uma oferta de assistência inadequada. Além disso, foram apresentadas tecnologias educacionais utilizadas para otimização deste conhecimento pela equipe de saúde sobre o manejo da dor na paliação e as dificuldades enfrentadas por estes profissionais durante a assistência aos pacientes paliativos. Considerações Finais: O déficit de conhecimento da equipe interdisciplinar apresentou-se em diversos estudos e este sendo considerado uma grande barreira para a oferta da assistência qualificada ao paciente em paliação.No entanto, o fato da utilização de estratégias que auxiliam na otimização do conhecimento apresenta-se de grande importância e assim contribuindo para o alívio da dor nos cuidados paliativos de maneira eficaz.

Objective: to analyze the strategies undertaken by nurses at Hospital do Câncer IV to act in accordance with the World Health Organization guidelines. Method: a historical and qualitative study carried out at Hospital do Câncer IV, a reference and exclusive Palliative Care unit belonging to the National Cancer Institute. Data collection included written documents and six oral statements and was carried out from August 2020 to August 2022. The study findings were organized and analyzed in accordance with the historical method, which included triangulation of the written and oral sources and the context in which they were produced. Results: based on the references of St. Christopher's Hospice and with the support of the Unit's Humanization Group, the strategies undertaken by the nurses and evidenced in this research were as follows: expansion of the Hospital Chaplaincy Project and the Silence Room; investments in graduate courses; professional training and technical-scientific update courses, both at the unit and at other institutions; participation in events as organizers, speakers and listeners; and performance in the Nursing Residency Program. Conclusion: nurses undertook effective strategies in the Oncology Nursing field through the consolidation and dissemination of specialized knowledge in Oncology Nursing in Brazil.
