Comparative Study of the Use of HPA Lanolin and Breast Milk for Treating Pain Associated with Nipple Trauma

Abstract: Objective To compare two different treatments—the use of highly purified anhydrous (HPA) lanolin and expressed breast milk—for women with pain and nipple trauma during the breastfeeding process. Method A total of 180 puerperal women were randomly assigned to 2 groups: one was treated with HPA lanolin and the other with their own expressed breast milk. “…So, lanolin did not have a strong influence in the healing of damaged nipples, in comparison with human milk. Our results differ from the results of Mariani Neto et al (2018) who reported a lower level of pain and better healing of damaged nipples in the group that used HPA lanolin in comparison with human milk during 7 days of treatment.…”

“…However, due to a small sample sizes, most researchers’ studies have not properly answer the question regarding the most effective treatment. Lanolin has had a curative influence on painful and damaged nipples (Bourdillon et al, 2020; Mariani Neto et al, 2018) and has often been recommended by experts. The healing properties of human milk in the treatment of painful and damaged nipples are widely known (Witkowska-Zimny et al, 2019).…”

“…Lanolin was associated with less nipple pain and damage at 49 h after treatment commenced, with the effect persisting for up to 7 days. 171 This corroborated the findings of a 2010 German RCT of 84 lactating mothers with painful and damaged nipples, who had significantly reduced pain with breastfeeding and significantly higher healing rates of nipple trauma within 3 days of commencing lanolin treatment, with effects still evident at 14 days. 172…”

“…9 There are various interventions for management of nipple pain such as breastfeeding education, applying compresses, dressings, ointments or tea bags. 10 Evidence suggests that utilization of some drops of expressed breast milk can be a facilitator of moist and healing nipple tissue among breastfeeding women. Expressed breast milk is a treatment for nipple pain as it works as a barrier that avoids losing the natural moistness of deeper skin layers.…”
