Objective: Reflect on how the new coronavirus pandemic triggered or accentuated the fear of childbirth in pregnant women and affected childbirth care practices. Methods: Reflective analysis of women's pregnancy and childbirth experiences during the current pandemic, supported by the latest scientific evidence and recommendations on the topic. Results: Pregnancy and childbirth are life-changing events for women, but during the new coronavirus pandemic, fear and uncertainty have taken on an unprecedented dimension in the negative way that many pregnant women have anticipated and experienced childbirth. Final considerations: The current period has accentuated a chronic problem: a paternalistic system of health institutions in the approach to childbirth, dense with additional levels of fear in pregnant women. In this context, addressing the fear of childbirth means not giving up the promotion of safe and positive birth experiences for women.
Objective To compare two different treatments—the use of highly purified anhydrous (HPA) lanolin and expressed breast milk—for women with pain and nipple trauma during the breastfeeding process. Method A total of 180 puerperal women were randomly assigned to 2 groups: one was treated with HPA lanolin and the other with their own expressed breast milk. All of the participants received the same breastfeeding technique instructions and therapeutic care standard. Three assessments were performed: at the time of inclusion in the study (after randomization); after 48 hours; and after 7 days. At each interval, data was collected in relation to pain and trauma. A numerical/verbal category scale was used for the pain variable, and the nipple trauma score for the trauma variable. The results were subjected to statistical analysis using the chi-squared test, the Fisher exact test, the student t-test, and the Kolmogorov-Smirnov test. Generalized estimating equations were calculated using the STATA 12 statistical software package (StataCorp LLC, College Station, TX, USA) and IBM SPSS Statistics for Windows, Version 20.0 (IBM Corp, Armonk, NY, USA). Results There was pain improvement from the second to the third assessment in the group that used HPA lanolin, while the pain remained unchanged between these two periods (p < 0.001) in the breast milk group. In terms of trauma, improvement was identified in its extension and depth from the first to the third assessment, and it was higher in the HPA lanolin group than in the breast milk group (p = 0.025). Conclusion The treatment of pain and nipple trauma with HPA lanolin achieved better results than the one with breast milk, based on a 7-day treatment period.
Objective: To characterize domestic violence in pregnancy. Method: Cross-sectional, exploratory and analytical study of domestic violence with 385 women who attended a public maternity. The Chi-square test of Pearson and Fisher exact test were used to verify associations and considering significant results p<0.05. Data of the sociodemographic characteristics of women, partners and family members and items of "Abuse Assessment Screen-AAS" were collected. Results: Domestic violence compromised 36.9% of women at some point in life and 34.6% during pregnancy. Prevalence rates were due to psychological (97.1%), physical (48.7%) and sexual (4.9%) violence and the partner was the main aggressor. The following variables were significantly associated with domestic violence: protestant religion (p=0.0022), lack of planning of pregnancy (p=0.0196), low family income (p=0.0215) and partner drinking habit (p=0,0002). Conclusion: Domestic violence should be systematically investigated during pregnancy, with special attention to protestant pregnant women, women who did not plan their pregnancy and women whose partners are alcoholics. ResumoObjetivo: Caracterizar violência doméstica na gravidez. Método: Estudo transversal, exploratório e analítico da violência doméstica com 385 mulheres atendidas em maternidade pública. Testes de Qui-Quadrado de Pearson e Exato de Fisher foram utilizados para verificar associações e considerados significantes resultados p<0,05. Dados das características sociodemográficas das mulheres, parceiros e familiares e itens do "Abuse Assessment Screen-AAS" foram coletados. Resultados: A violência doméstica acometeu 36,9% das mulheres em algum momento da vida e 34,6% na gravidez. As prevalências foram para violência psicológica (97,1%), física (48,7%) e sexual (4,9%) e oparceiro foi o principal agente. Houve associação significante da violência doméstica com religião protestante (p=0,0022), ausência de planejamento da gravidez (p=0,0196), baixa renda familiar (p=0,0215) e hábito do etilismo do parceiro (p=0,0002). Conclusão: A violência doméstica deve ser investigada sistematicamente na gravidez, com atenção especial nas grávidas protestantes, sem planejamento da gravidez e as mulheres cujos parceiros são etilistas.
Objective: to compare the axillar temperatures of newborns that are put immediately after birth in skin-to-skin contact under the Top Maternal device, as compared to those in a radiant heat crib. Methods: comparatives observational study of the case-control type about temperature of 60 babies born at the Obstetric Center and Normal Delivery Center of a public hospital of the municipality of Sao Paulo, being them: 29 receiving assistance in heated crib and 31 in skin-to skin contact, shielded by a cotton tissue placed on mother's thorax, called Top Maternal. Results: the temperature of the babies of the skin-to-skin contact group presented higher values in a larger share of the time measures verified, as compared to those that were placed in radiant heat crib, independently from the place of birth. Differences between the two groups were not statistically significant. Conclusion: the study contributes to generate new knowledge, supporting the idea of keeping babies with their mothers immediately after birth protected with the Maternal Top, without harming their wellbeing, as it keeps the axillar temperature in recommendable levels.
