Sepsis is a life-threatening disease with a high number of mortality in premature infants. Premature infants have immature immune systems, with less pool neutrophils and imperfect ability to destroy pathogen. Neutrophil function is supported by lymphocyte's ability to form antibody or specific cell-surface receptors for particular antigens. This underlies the use of neutrophil-to-lymphocyte ratio (NLR) as an inflammation marker to detect and assess the severity of sepsis. C-reactive protein (CRP) is known as an acute phase reactant. Neutrophil-tolymphocyte ratio is an easier, fast, and inexpensive method when compared to CRP. The aim of this study was to evaluate the correlation between NLR and CRP in detecting early-onset sepsis (EOS) in premature infants. A cross-sectional study was conducted on 53 premature infants born and hospitalized in a hospital in Indonesia who were recruited during the period of April to October 2018. Blood was sampled from the umbilical cord at birth for laboratory examination. The NLR was determined as the ratio of neutrophil to lymphocyte count. The Tollner scoring system was used to identify sepsis. Mann Whitney-U test and Spearman Correlation test were computed for the statistical analysis. neutrophil-to-lymphocyte ratio which results showed a strong positive correlation with CRP (r=0.702, 0=0.001) in premature infants with EOS. Leukocyte count was lower in infants with EOS than those without EOS group (median; IQR, x10 3 : 8.9 (6.3-13.8) vs 12.5 (10.1-16.1); p=0.016). Neutrophil-to-lymphocyte ratio and CRP tended to be lower in EOS group when compared to that of the non-EOS group. In conclusion, NLR has a strong positive correlation with CRP in premature infants with EOS. Neutrophil-to-lymphocyte ratio (NLR) diajukan untuk petanda inflamasi, mendeteksi dan menentukan derajat sepsis, sedangkan C-reactive protein (CRP) telah diketahui sebagai protein fase akut. Neutrophil-to-lymphocyte ratio adalah pemeriksaan yang lebih mudah, cepat, murah dibanding dengan CRP. Yenidoğan sepsisi tanı ve tedavideki son yıllarda olan gelişmelere rağmen halen önemli bir morbidite ve mortalite nedenidir. Yardımcı tanı yöntemleri olarak çeşitli hematolojik parametreler kullanılmaktadır. Özellikle trombositle ilgili parametrelerin enfeksiyonların klinik seyrinde ve tedavi etkinliğinin değerlendirilmesinde kullanılmasıyla ilgili çalışmalar yapılmaktadır. Bu çalışmada, geç neonatal sepsis tanısında trombosit parametrelerinden trombosit sayısı (PLT), ortalama trombosit hacmi (MPV), MPV/trombosit oranı(MPR)ile trombosit/ lenfosit oranı (PLR), nötrofil /lenfosit oranı (NLR)düzeylerinin araştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Yenidoğan yoğun bakım ünitesinde Ocak 2014-Haziran 2016 tarihleri arasında, klinik sepsis tanısı alan veya kan kültüründe pozitiflik saptanan 48 yenidoğan çalışmaya dahil edildi. Tedavi öncesinde alınan tam kan sayımı sonuçlarından PLT, MPV, MPR ve PLR, NLR parametreleri değerlendirildi. Kontrol grubu başka nedenlerden dolayı kan alınmış 60 sağlıklı yenidoğanlardan oluşturuldu. BULGULAR: Gruplar arasında PLT, MPV, MPR, NLR bakımından istatiksel olarak (p<0.05) anlamlı fark saptandı. Sepsis grubunda MPR ile platelet ve PLR arasında negatif yönlü korelasyon bulundu. Sepsisi ayırt etmede MPR ve NLR ölçümleri anlamlı bulunurken, MPV ve PLR değerlerinde istatistiksel olarak anlamlı bir fark bulunmadı. SONUÇ: Çalışma sonucunda sepsis grubunda MPV, MPR değerlerinde yükseklik saptanmışken PLR değerinde ise istatiksel olarak fark bulunmamıştır. Trombosit parametrelerinin farklı mikroorganizmalara karşı yanıtlarını tanımlamak için geniş örneklemlerle, çok merkezli, prospektif çalışmalara ihtiyaç vardır.

Background Late-onset infection is the most common serious complication associated with hospital care for newborn infants. Because confirming the diagnosis by microbiological culture typically takes 24 to 48 hours, the serum level of the inflammatory marker C-reactive protein (CRP) measured as part of the initial investigation is used as an adjunctive rapid test to guide management in infants with suspected late-onset infection. Objectives To determine the diagnostic accuracy of serum CRP measurement in detecting late-onset infection in newborn infants. Search methods We searched electronic databases (MEDLINE, Embase, and Science Citation Index to September 2017), conference proceedings, previous reviews, and the reference lists of retrieved articles. Selection criteria We included cohort and cross-sectional studies evaluating the diagnostic accuracy of serum CRP levels for the detection of late-onset infection (occurring more than 72 hours after birth) in newborn infants. Data collection and analysis Two review authors independently assessed eligibility for inclusion, evaluated the methodological quality of included studies, and extracted data to estimate diagnostic accuracy using hierarchical summary receiver operating characteristic (SROC) models. We assessed heterogeneity by examining variability of study estimates and overlap of the 95% confidence interval (CI) in forest plots of sensitivity and specificity. Main results The search identified 20 studies (1615 infants). Most were small, single-centre, prospective cohort studies conducted in neonatal units in high-or middle-income countries since the late 1990s. Risk of bias in the included studies was generally low with independent assessment of index and reference tests. Most studies used a prespecified serum CRP threshold level as the definition of a 'positive' index test (typical cutoff level between 5 mg/L and 10 mg/L) and the culture of a pathogenic microorganism from blood as the reference standard.
