Background: Anti-M antibody alloantibody is a fairly common, naturally occurring cold-type antibody.e mostly of the IgM class but may also occur as IgG or IgM with IgG component. It is considered clinically insignificant as it reacts at temperatures below 37°C. IgM class anti-M antibody may be responsible for interference with the standard ABO blood grouping in patients. Material and methods:In this study we focus on the experience of the Reference Laboratory of the Regional Blood Transfusion Center Katowice in detection, identification as well as interference of anti-M alloantibody with ABO grouping. Between January 1st 2022 and June 30th, 2022, the Reference Laboratory performed 1800 consulting and blood grouping tests. The study test pool was divided into 2 groups by age: 4 months-18 years and above 18 years of age. 137 cases of anti-M antibodyalloantibody from the MNS blood group system were detected and identified. In cases of ABO blood grouping-problems, additional procedures were applied such as blood grouping with conventional test tube technique (CTT) at 37°C, use of standard M negative O, A1, B red blood cells and treatment of sera with 2-mercaptoethanol (2-ME) agglutanation test. Results: Anti-M antibody interference with ABO blood grouping was more common for individuals under 18 years of age than in the older patients. Anti-M antibody occured approximately twice as frequently in the IgM class than in the IgG/IgG + IgM class, regardless of age/study group. Conclusions: Anti-M antibody was causing unexpected positive reactions of the study serum with standard red blood cells and the reason of complications in interpretation of results and delay in their the issue/release.

Streszczenie Wstęp: Alloprzeciwciała anty-M są dość często występującymi naturalnymi przeciwciałami typu zimnego. Zazwyczaj są klasy IgM, ale występują także jako przeciwciała klasy IgG lub IgM z komponentą IgG. Najczęściej nie wykazują aktywności w temperaturze 37°C i w takim przypadku są traktowane jako nieistotne klinicznie. Przeciwciała anty-M klasy IgM mogą być przyczyną odchyleń od klasycznego schematu oznaczania grupy krwi w układzie ABO u pacjentów. Materiał i metody: W niniejszej pracy przeanalizowano doświadczenia własne Pracowni Konsultacyjnej Regionalnego Centrum Krwiodawstwa i Krwiolecznictwa (RCKiK) w Katowicach związane z wykrywaniem i identyfikacją alloprzeciwciał anty-M oraz problemem z oznaczeniem grupy krwi wynikającym z ich obecności. Od 01.01.2022 roku do 30.06.2022 roku wykonano 1800 badań konsultacyjnych oraz badań grup krwi. Analizowaną pulę badań podzielono na dwie grupy w zależności od wieku pacjenta: od 4. miesiąca życia do 18. roku życia i powyżej 18. roku życia. W 137 przypadkach wykryto i zidentyfikowano alloprzeciwciała anty-M z układu MNS. W przypadkach, w których wystąpiły problemy z oznaczeniem grupy krwi, zastosowano dodatkowe procedury, takie jak: oznaczenie grupy krwi techniką probówkową z zachowaniem temperatury 37°C, użycie krwinek wzorcowych O, A1, B M-ujemnych, traktowanie surowicy badanej odczynnikiem 2-ME. Wyniki: Przeciwciała anty-M stanowiły większy problem przy oznaczaniu grupy krwi u osób poniżej 18. roku życia niż u starszych pacjentów. Występowały około 2 razy częściej klasy IgM niż klasy IgG/IgG+IgM niezależnie od wieku grupy badanej. Wnioski: Przeciwciała anty-M wywoływały nieoczekiwane reakcje dodatnie surowicy badanej z krwinkami wzorcowymi, utrudniając interpretację otrzymanych wyników i opóźniając wydanie wyniku grupy krwi. Background: By analyzing the detection rate of anti-M antibody in patients with the MNS blood group system in the Hunan area, we aimed to explore its clinical significance and blood transfusion strategy. Methods:We retrospectively analyzed the clinical data of patients who had been confirmed to contain anti-M antibodies through serological methods such as the saline tube method and cassette anti-human globulin method.Results: Irregular antibody screening tests had been applied to 94,452 patients, from which 652 results were positive. Among those positive patients, 93 cases were positive for anti-M antibodies, accounting for 14.26% of the positive rate of irregular antibodies; 11 cases had a blood transfusion history, accounting for 11.8%; 59 cases had a pregnancy history, accounting for 63.4%; and 2 cases had a transplant history, accounting for 2.2%. The patients with anti-M antibodies included 23 pregnant woman, accounting for 24.7%, and 19 tumor patients, accounting for 20.4%. A total of 66 cases were immunoglobulin M (IgM) + immunoglobulin G (IgG) class, accounting for 71.0%, 26 cases were IgM class, accounting for 28.0%, and 1 case was IgG class, accounting for 1.0%. Conclusions:The detection rates of anti-M antibody in the Hunan area and unexpected antibodies in literature reports are mainly related to a pregnancy history, and the type of antibody is predominantly IgM + IgG class. The clinical significance of anti-M antibody cannot be ignored, and three media should be used for cross-matching of blood wherever possible to ensure the safety of blood transfusion.
