The feasibility of dual antiplatelet therapy as early as possible in patients with ST-segment elevation acute coronary syndrome, where percutaneous coronary intervention is recommended, has been proven: it improves treatment outcomes by reducing the risk of adverse ischemic events, including stent thrombosis and myocardial infarction.This article provides a detailed analysis of the evidence data and current recommendations on the validity and timing of dual antiplatelet therapy for acute coronary syndrome. The emphasis is made on the controversy regarding the early dual antiplatelet therapy in non-ST-segment elevation acute coronary syndrome. The rationale for using dual antiplatelet therapy only after coronary angiography and determining the revascularization strategy is described, which should increase the accessibility of coronary artery bypass graft surgery for patients.
Цель. Вопрос выбора оптимальной стратегии реваскуляризации для пациентов с острым коронарным синдромом без подъ-ема сегмента St (ОКСбпSt) и многососудистым поражением (МП) коронарного русла остается открытым. целью работы явилась оценка результатов лечения данной группы больных в зависимости от стратегии реваскуляризации.Материалы и методы. В исследование в рамках проспективного одноцентрового регистра за 2012-2015 годы было вклю-чено 362 пациента с ОКСбпSt и МП коронарного русла. В зависимости от реализованной стратегии реваскуляризации пациенты были разделены на три группы: поэтапное ЧКВ (ЧКВ -ЧКВ) выполнено 266 (73,4 %) пациентам, коронарное шунтирование (КШ) -72 больным (19,9 %), первый этап -ЧКВ и второй этап -КШ (ЧКВ -КШ) реализованы 24 (6,6 %) пациентам. Конечными точками исследования стали такие неблагоприятные кардиоваскулярные события, как смерть, инфаркт миокарда (ИМ), острое нарушение мозгового кровообращения/транзиторная ишемическая атака (ОНМК/тИА), повторная реваскуляризация целевого сосуда, кровоте-чение по шкале BARC (Bleeding Academic Research Consortium). Конечные точки оценивались в период от индексной госпитализа-ции по поводу ОКСбпSt до окончания госпитального периода при выполнении второго этапа реваскуляризации (ЧКВ или КШ) или единственного этапа реваскуляризации (КШ).Результаты. Максимальные значения шкалы GRACE, достигающие 145,81±18,61 балла, были отмечены в группах ЧКВ -ЧКВ и ЧКВ -КШ, тогда как наибольшая выраженность коронарного атеросклероза по шкале SYNtAX (до 28,75±10,47 балла) в группах ЧКВ -КШ и КШ. Показатель летальности колебался от 2,8 % в группе КШ до 8,3 % в группе ЧКВ -ЧКВ (р=0,5). Частота ИМ от 1,4 % в группе КШ до 8,3 % в группе ЧКВ -КШ (р=0,5). Встречаемость геморрагических осложнений 3-5-го типа по шкале BARC составила от 1,1 % в группе ЧКВ -ЧКВ до 37,5 % в группе ЧКВ -КШ (р=0,003).Заключение. Отсутствие четких рекомендаций для выбора оптимальной стратегии и сроков реваскуляризации среди паци-ентов с ОКСбпSt при МП коронарного русла приводит к развитию значительного числа неблагоприятных кардиоваскулярных исхо-дов в данной группе пациентов. С учетом полученных в настоящем исследовании результатов становится понятным, что стратегия реваскуляризации для пациентов с ОКСбпSt должна осуществляться на основе комплексного применения стратификационных шкал (GRACE, SYNtAX, EuroScore II). При этом КШ (как самодостаточный метод) может являться оптимальной стратегией реваску-ляризации для значительно большего числа пациентов, чем применяется в настоящее время.Ключевые слова: острый коронарный синдром без подъема сегмента St, многососудистое поражение, чрескожное коро-нарное вмешательство, коронарное шунтирование, стратегия реваскуляризации. Aim. To evaluate the range of risk factors associated with adverse cardiovascular events onset in hospital period of follow-up in patients with non-ST elevation acute coronary syndrome (NSTEACS) with multivessel disease (MD).Material and methods. To the study, under the framework of prospective, single-center registry, the NSTEACS and MD patients were included. Depending on the strategy of revascularization, three groups were selected: staged PCI, coronary bypass (CBG) and PCI as the first with CBG as the second stage. As adverse cardiovascular events, the following were taken: death, myocardial infarction, stroke or transient cerebral ishemia, clinically significant bleeding by BARC, repeat nonscheduled revascularization of target vessel. Results. Analysis revealed the groups of factors increasing the probability of adverse cardiovascular events. Those are: (1) clinical and demographic — older age, diabetes, postinfarction cardiosclerosis, multifocal atherosclerosis, chronic renal failure; (2) coronary and surgical — left main stem lesion, severe atherosclerosis by SYNTAX Score, high surgical risk by EuroScore II, revascularization strategy; (3) predominance of low or moderate risk by GRACE comparing with high risk.Conclusion. The results have confirmed high predictional significance of clinical and demography and coronary factors for development of adverse cardiovascular diseases. A significant finding of the study was the fact that in a hospital with surgery and endovascular treatment available 24/7, the intermediate and low GRACE risk in NSTEACS and MD is a factor of adverse cardiovascular events development due to revascularization delays if the risk is not high.

Вопрос выбора оптимальной стратегии реваскуляризации для пациентов с острым коронарным синдромом без подъема сегмента ST (ОКСбпST) и многососудистым поражением (МП) коронарного русла остается открытым. Целью работы явилась оценка результатов реваскуляризации методом коронарного шунтирования (КШ), выполненного в ранние сроки ОКСбпST. Материал и методы. В исследование за период с 2016 по 2018гг было включено 87 последовательных пациентов с ОКСбпST и МП коронарного русла, которым было выполнено КШ. В зависимости от сроков выполнения реваскуляризации пациенты были разделены на 4 группы: первые 24 ч, 24-72 ч, 72 ч-7 сут., >7 сут.-в течение госпитального периода. Конечными точками исследования стали неблагоприятные кардиоваскулярные события: смерть, инфаркт миокарда (ИМ), острое нарушение мозгового кровообращения/транзиторная ишемическая атака, повторная реваскуляризация, кровотечение по шкале BARC (Bleeding Academic Research Consortium). Конечные точки оценивались в госпитальный период времени. Результаты. Пациенты исследуемой выборки характеризовались тяжелым клиническим и ангиографическим статусом. Средний показатель SYNTAX Score составил 36 (33;38) баллов, GRACE-136 (123;144) баллов. Гемодинамически значимое поражение ствола левой коронарной артерии выявили у 40% пациентов. В 44% случаев имело место течение ИМ, 48% пациентов страдали сахарным диабетом. В 8% случаев пациентов получили реваскуляризацию в течение 24 ч от момента поступления в клинику, в 9%-в интервал времени от 24 до 72 ч, в оставшихся случаях в течение госпитального периода. Девяносто четыре процента операций выполнялось в условиях искусственного кровообращения. Остаточное после КШ значение SYNTAX Score в группе не превысило 5 (3; 7) баллов. Средний объем кровопотери был на уровне 550±150 мл. Общее количество летальных исходов составило 4,6%. Заключение. Высокая потребность в КШ, как обоснованной стратегии реваскуляризации для пациентов с ОКСбпST и МП и удовлетворительные результаты операции демонстрируют необходимость в доступности КШ в режиме 24/7, по аналогии с чрескожным коронарным вмешательством.
