Оце-ниваемый с ее помощью показатель глобальной продольной деформации миокарда более чувствителен к ранним изменениям сократимо-сти левого желудочка, чем фракция выброса. Диагностические возможности speckle tracking эхокардиографии нашли отражение в клини-ческих рекомендациях и согласительных документах Европейского общества кардиологов (ESC), Европейской ассоциации специалистов по методам визуализации сердечно-сосудистой системы (EACVI) и Американского общества эхокардиографии (ASE). Целью данной статьи яв-ляется изложение основных принципов speckle tracking эхокардиографии и возможностей клинического применения этой новой техноло-гии. Уделено внимание использованию speckle tracking эхокардиографии при сердечной недостаточности, коронарной патологии и инфаркте миокарда, гипертрофии левого желудочка при артериальной гипертензии, гипертрофической кардиомиопатии и амилоидозе сердца, кла-панных пороках сердца, констриктивном перикардите и эффекте кардиотоксичности при лечении онкопатологии.Ключевые слова: speckle tracking, эхокардиография, деформация миокарда, дисфункция миокарда, заболевания сердца. Speckle-tracking echocardiography is promising modern technique for evaluation of structural and functional changes in the myocardium. It evaluates the indicator of global longitudinal myocardial deformation, which is more sensitive than ejection fraction to early changes of left ventricular contractility. The diagnostic capabilities of speckle tracking echocardiography are reflected in clinical recommendations and consensus statements of European Society of Cardiology (ESC), European Association of Cardiovascular Imaging (EACVI) and American Society of Echocardiography (ASE). The aim of this paper is describe basic principles of speckle tracking echocardiography and clinical applications of this new technology. Attention is paid to the use of speckle tracking echocardiography in such heart pathologies as heart failure, coronary heart disease and myocardial infarction, left ventricular hypertrophy in arterial hypertension, hypertrophic cardiomyopathy and amyloidosis of the heart, valvular heart disease, constrictive pericarditis and cancer therapy-induced cardiotoxicity.Keywords: speckle tracking, echocardiography, myocardial strain, myocardial dysfunction, heart diseases. Целью исследования было изучение влияния приверженности терапии на дисфункцию миокарда у лиц пожилого и старческого возрас-та с ишемической болезнью сердца и хронической сердечной недостаточностью. Материал и методы. В исследование были включены 86 больных ишемической болезнью сердца старших возрастных групп, поступивших на стационарное лечение в связи с прогрессированием хронической сердечной недостаточности: 21 пациент -от 65 до 74 лет, 65 пациентов -от 75 до 89 лет. Обследование пациентов включало стандартное клиническое исследование, эхокардиографию с оценкой глобальной продольной деформации миокарда, а также тест Мори-ски-Грина. Результаты. Установлено, что в группе лиц старческого возраста имеют место более низкая приверженность к лечению и более выраженные структурно-функциональные изменения миокарда. Выявлены корреляционные связи суммарного балла теста Мориски-Грина с конечно-диастолическим объемом левого желудочка (r=-0,33; p<0,05), с показателем Е/е' (r=-0,37; p<0,05), с ГПД (r=0,53). Заключение.Полученные данные свидетельствуют о более низкой приверженности лиц старческого возраста к терапии ИБС и ХСН, по сравнению с ли-цами пожилого возраста. Низкая приверженность к проводимой терапии, наряду с постинфарктным кардиосклерозом, по-видимому, может рассматриваться в качестве одного из факторов, способствующих прогрессированию дисфункции миокарда. В последнее годы отмечается рост продолжительности жизни населения большинства развитых стран, при этом ожидается, что число лиц старше 65 лет увеличится с 2010 по 2050 г. в два раза [1]. Возраст является одним из ключевых факторов сердечно-сосудистого риска, что обуславливает рост числа кардиологических заболеваний, включая коронарную патологию, на фоне постарения населения [2]. Расширяются современные возможности интервенционных вмешательств и операций хирургической реваскуляризации у лиц старших возрастных групп, страдающих ишемической болезнью сердца (ИБС) [3-5]. Вышесказанное диктует необходимость изучения структурно-функциональных изменений миокарда, что может быть полезно как для стратификации риска периоперационных кардиологических осложнений, так и выбора лечебной тактики. Мечникова Минздрава России, Санкт-Петербург 2 Госпиталь для ветеранов войн, Санкт-Петербург СТРУКТУРНО-ФУНКЦИОНАЛЬНЫЕ ИЗМЕНЕНИЯ МИОКАРДА И КЛАПАНОВ СЕРДЦА У БОЛЬНЫХ КОРОНАРНОЙ ПАТОЛОГИЕЙ СТАРШИХ ВОЗРАСТНЫХ ГРУПП Целью данной работы было изучение структурно-функциональных изменений миокарда и клапанов сердца у больных коронарной патологией пожилого и старческого возраста. Обследовано 128 лиц с ишемической болезнью сердца 82,4 ± 10,6 года (от 65 до 90 лет), которые, в соответствии с классификацией ВОЗ, были разделены на две возрастные группы: 1-я-лица пожилого возраста-от 65 до 74 лет (32%), 2-я-лица старческого возраста-от 75 до 90 лет (68%). Всем обследуемым выполнялись сбор анамнеза, физикальный осмотр, электрокардиография, эхокардиография с тканевой допплерографией, оценка глобальной продольной деформации с помощью методики 2D-speckle-tracking, коронароангиография. У больных коронарной патологией пожилого и старческого возраста выявлены структурно-функциональные изменения миокарда, связанные с возрастом, многососудистым поражением коронарных артерий, наличием в анамнезе перенесенного инфаркта миокарда, а также сопутствующей патологией (артериальная гипертензия). При этом у лиц старше 75 лет отмечается прогрессирование коронарной патологии, систолической и диастолической дисфункции, кальциноза аортального клапана, комбинированных пороков аортального и митрального клапанов, а также увеличение числа лиц с гипертрофией левого желудочка. Показана информативность эхокардиографической оценки глобальной продольной деформации миокарда для выявления субклинической систолической дисфункции миокарда у больных коронарной патологией пожилого и старческого возраста.
