Цель исследования -выявление факторов, позволяющих прогнозировать развитие и / или нарастание неврологического дефицита (НД) в послеоперационном периоде в случае преходящих критических изменений параметров (КИП) при регистрации транскраниальных моторных (ТКМВП) и соматосенсорных (ССВП) вызванных потенциалов во время клипирования интракраниальных артериальных аневризм.
ВВЕДЕНИЕОдной из общепринятых тактик лечения опухо-лей хиазмально-селлярной области является хирур-гическая. В связи с развитием современной эндоско-пической техники эндоскопическое эндоназальное транссфеноидальное удаление опухоли является наиболее популярным методом. Этот метод наиме-нее травматичен, лучше переносится больными и характеризуется более низкой частотой осложнений.Однако даже данный хирургический доступ не лишён определённых недостатков. Ещё со времён первой попытки удаления опухоли транссфенои-дальным доступом одной из важнейших задач стало создание эффективного восстановления селлярного дна для профилактики послеоперационной ликвореи и менингита. Назальная ликворея -довольно рас-пространённое состояние при транссфеноидальных вмешательствах. Оно является особенностью этого вида хирургических вмешательств и характеризует-ся достаточно высокой частотой как в интра-, так и в послеоперационном периоде. Ликворея считается крайне опасным и серьёзным состоянием из-за угро-зы инфицирования ликворной системы и развития вторичного менингита [5,6].Развитие менингита происходит в 9-25 % случаев назальной ликвореи, а риск повторного менингита составляет около 10 % [10]. Летальность при после-операционном менингите составляет 2-3,5 %.Для профилактики развития внутричерепной гипертензии и лечения назальной ликвореи в настоя-щее время используются дегидратационная терапия, люмбальное дренирование и повторная пластика де-фектов твёрдой мозговой оболочки. Стоит отметить, что приоритеты в мировой практике не расставлены.
