Вирусы герпеса (ВГ) широко распространены во всем мире и вызывают различные заболевания, в том числе герпетический герпес, генитальный герпес, герпетический стромальный кератит, менингит, энцефалит и др. Врожденные и адаптивные иммунные реакции являются ключевыми в борьбе с герпес-вирусом, и у вируса разработаны механизмы уклонения от них. Тот факт, что некоторые люди более других склонны к тяжелому заболеванию при инфицировании ВГ, можно частично объяснить наличием иммунных и генетических особенностей. ВГ имеет два цикла репликации: литический и латентный. Во время литической репликации вирусы герпеса продуцируют инфекционные вирусные частицы для заражения других клеток и организмов, в то время как во время латентного периода наблюдается ограниченная экспрессия генов и отсутствие инфекционных вирусных частиц. HV устанавливает латентный период в нейронах и может вызывать заболевание как при первичной инфекции, так и при реактивации. Механизмы, ведущие к латентности и реактивации, а также факторы вируса и хозяина, контролирующие эти процессы, до конца не изучены. В статье рассмотрен жизненный цикл ВГ, взаимодействие ВГ с иммунной системой и влияние инфекции SARS-CoV-2 на реактивацию ВГ. Herpesviruses (HV) are widespread throughout the world and cause various diseases, including herpes simplex, genital herpes, herpetic stromal keratitis, meningitis, encephalitis, and others. Innate and adaptive immune responses are key in the fight against herpesvirus, and the virus has developed mechanisms to avoid them. The fact that some people are more likely than others to become seriously ill when infected with VH can be partly explained by the presence of immune and genetic characteristics. VG has two cycles of replication: lytic and latent. During lytic replication, herpes viruses produce infectious viral particles to infect other cells and organisms, while during the latent period, there is limited gene expression and the absence of infectious viral particles. HV establishes a latent period in neurons and can cause disease in both primary infection and reactivation. The mechanisms leading to latency and reactivation, as well as the viral and host factors that control these processes, are not fully understood. The article discusses the life cycle of SH, the interaction of SH with the immune system, and the impact of SARS-CoV-2 infection on SH reactivation.
Инсектная аллергия – это совокупность аллергических реакций, возникающих при контактах с насекомыми и их метаболитами. Основным источником антигенов кровососущих насекомых является слюна, в состав которой входят вазоактивные вещества – ацетилхолин, гистамин, серотонин, эфедрин и др. Наиболее опасными в плане развития острых, жизнеугрожающих аллергических реакций считаются ужаления представителей отряда перепончатокрылых. Классическими симптомами инсектной аллергии у взрослых являются изолированные кожные проявления, крапивница / ангионевротический отек, головокружение/гипотензия, диспноэ / сухие хрипы, ларингоспазм / охриплость голоса, потеря сознания. Для диагностики инсектной аллергии почти всегда достаточно изучения анамнеза. Единственным лечением, которое может потенциально предотвратить развитие системных реакций от ужаления насекомых, является АСИТ. В Республике Беларусь до 2019 г. АСИТ аллергенами ядов насекомых не проводилась, клинический опыт отсутствует, поэтому считаем полезным представить описание 2 клинических случаев успешной иммунотерапии аллергеном яда пчел. An insect allergy is a set of allergic reactions occurring due to contact with insects and their metabolites. The main source of antigens of blood-sucking insects is saliva, which includes vasoactive substances - acetylcholine, histamine, serotonin, ephedrine, etc. The most dangerous, acute, life-threatening allergic reactions are happening as a result of bites of hymenoptera order representatives. The classic symptoms of insect allergy in adults are isolated skin reactions, urticaria / angioedema, dizziness / hypotension, dyspnea / dry wheezing, laryngospasm / hoarseness of the voice, loss of consciousness. For the diagnosis of insect allergy, it is almost always enough to study the present history of the patient. The only treatment that can potentially prevent the development of systemic reactions from insect bites is ASIT. In Belarus until 2019, ASIT was not carried out with allergens of insect poisons, there is no clinical experience, so we consider it useful to present a description of 2 clinical cases of successful immunotherapy with an allergen of bee venom.
