Использование допплерографического исследования у детей дает возможность визуализировать не только сосуды почек до кортикального слоя паренхимы, но и провести количественную оценку ренального кровотока в динамике. Целью исследования явилось изучение диагностической значимости динамической допплерографии внутрипочечного кровотока и состояния паренхимы у детей с обструктивной уропатией. Материал и методы исследования. Обследовано 128 пациентов с обструктивными уропатиями различного генеза (пузырно-мочеточниковый рефлюкс, гидронефроз, мегауретер, уролитиаз) в возрасте от 3 мес до 17 лет, которые были разделены на 3 группы. Контрольную группу составили 56 практически здоровых детей того же возраста, никогда не имевших патологии мочевыделительной системы. Всем детям проводились общеклиническое обследование, ультразвуковое исследование почек и их сосудов и мочевого пузыря, экскреторная урография и микционная цистобструктивная уропатияретерография. Все ультразвуковые исследования выполнялись с помощью аппаратов Aloka-SSD-3500 SV с применением конвексного (6,0-8,0 МГц) и линейного (9,0-14,0 МГц) датчиков. Результаты. В результате проведенных исследований у больных I группы отклонений со стороны допплерографических показателей не выявлено. У больных II группы установлены достоверное повышение индекса резистентности в междолевых и сегментарных артериях и нормализация его значений после лечения, что предполагает функционально припособительный характер изменений. Отсутствие положительной динамики в показателях допплерографии у детей III группы после лечения и выраженность их изменений позволяют говорить о наличии морфологических нарушений. Выводы. Комплексное ультразвуковое исследование почек с применением допплерографии позволяет оценить состояние ренального кровотока у детей и подростков с обструктивной уропатией. Возможность динамического наблюдения за состоянием сосудистого русла во время консервативного лечения, после оперативных вмешательств делает ультразвуковой метод востребованным в урологии, хирургии.

Доступно множество неонатальных наблюдательных инструментов для оценки боли. Однако их применение в клинических условиях ограничено. В сравнительном аспекте изучены две предиктивные шкалы оценки неонатальной боли: NFCS-R (Neonatal Facial Coding System – Revised) -система кодирования лица новорожденного и CHIPPS (Children and Infant’s Postoperative Pain Scale) - шкала послеоперационной боли у детей и младенцев с точки зрения их психометрических качеств. Относительная конвергентная валидность были высокими для обоих инструментов оценки. Следовательно, при выборе необходимо учитывать клиническую полезность, а в дальнейшем определить возможности улучшения данных инструментов.
Введение. При аноректальных аномалях (AРА), обычно требуются немедленные оперативные вмешательства, чтобы открыть задний проход для опорожнения, если только на фистулу нельзя положиться или пока не будет проведена коррегирующая операция. В зависимости от степени выраженности аномалии лечение осуществляется только анопластикой промежности или колостомой на первом этапе и определенным восстановлением позже Цель работы. оценка эффективности одноэтапной коррекции при лечении новорожденных с ректово-вестибулярным свищом и его сравнение с трехэтапным методом. Материал и методы. Исследование было проведено в период 2020 года по март 2022 года в Андижанском областном детском многопрофильном медицинском центре. В качестве рандомизированного одностореннего слепого клинического исследования в исследование были включены 40 новорожденных девочек с АРА и ректовисти-булярным свищом. Контрольную группу составили 20 новорожденных, которым выполнялась трехэтапная операция, основную группу составили 20 пациенток, которым была выполнена одноэтапная коррекция. Статистический анализ данных проводился с использованием программы SPSS 16.0. Уровень значимости в данном исследовании был принят за 0,05. Результаты. В обеих группах были зарегистрированы осложнении во время и после операции, проведена сравнительная оценка. В контрольной группе был замечен только один случай (5%) раневой инфекции и расхождения, тогда как при одноэтапной операции наблюдалось шесть случаев (30%). В отношении частоты других осложнений, между двумя группами не было выявлено значительных различий. Выводы. Одноэтапный метод коррекции ректовестибулярного свища является предпочтительным и надежным методом лечения пациентов с неперфо-рированным задним проходом и ректовистибулярным свищом.

Bu omillarni inobatga olinishi kelajakda ARM ni boshqarishda qo`l kelishi mumkin.
