В статье проведен анализ мотивационных аспектов донорства крови и ее компонентов. Мотивация доноров крови, ее компонентов в Республике Беларусь смешанная, включая альтруистические установки помощи другим и ожидания компенсации за выполнение донорской функции. Показано, что большинство доноров крови, ее компонентов считают важным сохранение и расширение гарантий и компенсаций донорам при введении принципов добровольного безвозмездного донорства. С целью эффективного рекрутинга должны быть учтены необходимость сохранения существующих гарантий (освобождение от работы с сохранением среднего заработка, питание в день донации) и возможные дополнительные гарантии для доноров в виде компенсации на питание в междонационный период, налоговые льготы и др. The article analyzes the motivational aspects of blood donors and its components. The motivation of blood donors and its components in the Republic of Belarus is mixed, including altruistic attitudes to help others and expectations of compensation for performing the donor function. It is shown that the majority of blood donors and its components consider it important to maintain and expand guarantees and compensations to donors while introducing the principles of voluntary donation. For the purpose of effective recruiting, the need to maintain existing guarantees (exemption from work with the preservation of average earnings, meals on the day of donation) and possible additional guarantees for donors in the form of compensation for food during the interdonation period, tax benefits, etc. should be taken into account.
По результатам динамического наблюдения и лабораторной и инструментальной оценки показателей здоровья 113 регулярных доноров крови и компонентов крови выявлены изменения гематологических и биохимических показателей. Согласно данным исследования, до 50% доноров крови и ее компонентов имеют дефицит железа. Установлено, что у доноров крови статистически значимо более низкие показатели, характеризующие обмен железа, чем у доноров компонентов крови. Данные изменения также имеют особенности в зависимости от пола. У доноров компонентов крови статистически значимо более низкие показатели содержания белка и более высокие показатели содержания лейкоцитов и кальция, чем у доноров цельной крови. Не установлено клинически значимых изменений показателей системы гемостаза, электрокардиографии. Сделан вывод о необходимости более расширенного и частого по сравнению с действующим скрининга на дефицит железа, белка и, с учетом полученных результатов, назначения в рамках междонационного периода медикаментозной коррекции и определенного режима питания, изменения видов и интервалов донаций. Summary to the results of dynamic monitoring, laboratory and instrumental assessment of health indicators of 113 regular blood donors and blood components, changes in hematological and biochemical parameters were revealed. Data have been obtained characterizing iron deficiency in up to 50% of blood donors and its components. It has been established that whole blood donors have statistically significantly lower rates characterizing iron metabolism than blood component donors. These changes also have features depending on gender. Blood component donors had statistically significantly lower protein levels and higher leukocyte and calcium levels than whole blood donors. There were no clinically significant changes in the parameters of the hemostasis system, electrocardiography. It was concluded that there is a need for a more extended and frequent screening for iron and protein deficiency compared to the current one and, taking into account the results obtained, within the inter donation period of prescribing drug correction and a certain diet, changing the types and intervals of donations.
В Республике Беларусь с 2013 г. организована система донорства крови за плату и на безвозмездной основе. За период 2013–2018 гг. представлены результаты определения динамических трендов донорства крови и ее компонентов, включая изменения общего числа доноров крови и ее компонентов, количества первичных доноров крови, безвозмездных донаций крови в целом и по регионам и взаимное влияние данных показателей. Установлено среднегодовое увеличение числа доноров крови и ее компонентов на 3,8%, числа первичных доноров крови на 7,16%, количества безвозмездных донаций крови на 4,5%. При тесте на параллельность выявлены зависимость трендов увеличения доноров и первичных доноров (p>0,05) и большее увеличение числа безвозмездных донаций по сравнению с увеличением числа доноров и безвозмездных доноров (p<0,05). Сделан вывод о необходимости дальнейшего изменения национального законодательства и развития программ добровольного без возмездного регулярного донорства, что должно привести к увеличению общего числа доноров и количества первичных доноров. Since 2013 in the Republic of Belarus, the blood donation system has been organized for a fee and free of charge. For the period from 2013 to 2018, there are presented the results of determination of dynamic trends of the national blood donation system, including changes of the number of blood donors and its components, the number of first-time blood donors, non-remunerated blood donation in all regions, and the influence of these indicators. It was revealed that theaverage annual number of blood donors and its components increased by 3.8%, the number of primary blood donors – by 7.16%, the number of free blood donations – by 4.5%. The parallelism test revealed the dependence of trends of increase of donors and first-time donors (p>0.05) and a larger increase of the number of free donations, if compared to increase of the number of donors and free donors (p<0.05). There was made the conclusion about the need for further changes in the national blood service program and the development of voluntary non-remunerated donation programs that should lead to the increase of the total number of donors and the number of first- time donors.
