Ревматические болезни сердца (острая ревматическая лихорадка и хроническая ревматическая болезнь сердца) – актуальная клиническая, эпидемиологическая и в ряде стран социальная проблема. Диагностика ревматических поражений основана на синдромологическом подходе, т. к. специфических критериев нет. Это определяет необходимость серьезного дифференциально-диагностического поиска в каждой клинической ситуации. В статье представлены клинические случаи постстрептококкового артрита и миокардита с позиций дифференциального диагноза с ревматическим поражением суставов и сердца. Обсуждаются современные особенности течения острой ревматической лихорадки, роль стрептококковой инфекции, возможности и проблемы диагностики и дифференциальной диагностики. Rheumatic heart disease (acute rheumatic fever and chronic rheumatic heart disease) is an urgent clinical, epidemiological and, in some countries, social problem. Diagnosis of rheumatic lesions is based on the syndromological approach, since there are no specific criteria. This determines the need for a serious differential diagnostic search in each clinical situation. The article presents clinical cases of post-streptococcal arthritis and myocarditis from the standpoint of differential diagnosis with rheumatic joint and heart disease. The current features of acute rheumatic fever, the role of streptococcal infection, the possibilities and problems of diagnosis and differential diagnosis are discussed.
