При использовании метотрексата и других иммунодепрес-сантов повышается риск развития не только гепатоток-сичности, нейтро-и/или лимфопении, но и пневмоний, особенно часто -в течение первого года лечения [10]. Ингибиторы фактора некроза опухоли (Тumor necrosis factor, TNF) ␣ повышают риск развития бактериаль-ных, грибковых и оппортунистических инфекций [10]. Приблизительно половина всех инфекционных нежела-тельных явлений у пациентов с ЮИА связана с пораже-нием дыхательных путей [11,12]. Пациенты с ювенильным идиопатическим артритом (ЮИА) имеют повышенный риск развития инфекций. При-близительно половина всех серьезных инфекций у детей с ЮИА связана с поражением дыхательных путей. Цель исследования: изучить эффективность и безопасность 13-валентной пневмококковой конъюгированной вакцины (ПКВ) у детей с ЮИА. Методы. В ходе проспективного когортного исследования было сформировано 5 групп: дети с ЮИА в фазе ремиссии на терапии метотрексатом (группа 1) или этанерцептом (группа 2), с ЮИА в активной фазе до назначения метотрексата (группа 3) или этанерцепта (группа 4), контрольная группа (условно здоровые дети 494Streptococcus pneumoniae (пневмококк) является наиболее частым этиологическим фактором пневмоний в детском возрасте, именно с ним связывают до 50% слу-чаев этого заболевания [13,14]. Кроме того, имеются данные о том, что более высокая смертность среди паци-ентов, получающих ингибиторы TNF ␣, является следстви-ем пневмококковых инфекций [2].Пневмококковая инфекция -это заболевание, пре-дотвращаемое вакцинацией. Полисахаридные капсуль-ные серотипы S. pneumoniae являются факторами виру-лентности, ответственными за инвазивные инфекции [15]. Эта бактерия имеет более 90 серотипов. У людей около 25-30 серотипов отвечают за 90% инвазивных инфекционных случаев [15]. Эффективность пневмокок-ковых вакцин зависит от охвата наиболее распростра-ненных серотипов, а также от их иммуногенности [15,16]. Распространенность серотипов варьирует в зависи-мости от возраста, хронических заболеваний, геогра-фического региона, введения пневмококковых вакцин и использования антибиотиков [16]. Учитывая пробле-мы, связанные с инфекцией, развивающейся на фоне терапии иммунодепрессантами, руководства по лечению ревматических болезней рекомендуют рутинное исполь-зование пневмококковых вакцин у пациентов с осла-бленным иммунитетом [17]. Однако вакцинация детей (например, против пневмококка), страдающих ЮИА, проводится относительно редко по сравнению с общей популяцией [18][19][20]. Background. To assign genetically engineered biologic drugs, we need data on the predictors for response to therapy. Prognostic factors for the response to tocilizumab in patients with juvenile idiopathic arthritis (JIA) without systemic symptoms are poorly studied.Objective. Our aim was to reveal early predictors for the response to tocilizumab therapy in patients with JIA without systemic symptoms.Methods. A retrospective cohort study enrolled patients with JIA without systemic symptoms who received tocilizumab therapy between July 2009 and August 2017. We assessed the association between the initial demographic, clinical, and laboratory parameters in patients and the best response (according to the ACR90 criteria) to treatment after a year.Results. The study included 95 (girls 85%) patients; the mean age was 10.3 (6.0; 13.6). During the first year of therapy, 71 (75%), 55 (58%), 38 (40%), and 22 (23%) patients achieved the improvement according to ACR30/50/70/90 criteria, respectively; 22 (23%) patients reached disease inactive stage according to the Wallace criteria. When performing multivariate analysis, the following improvement predictors were revealed based on the ACR90 criteria after a year of treatment: decrease in serum C-reactive protein level during the first month of therapy [odds ratio (OR) 1.024; 95% confidence interval (CI) 1.007–1.051], decrease in disease activity score on the visual analogue scale according to the parent/patient assessment (OR 1.048; 95% CI 1.005–1.105), early onset of the disease (OR 0.38; 95% CI 0.16–0.72), persistent oligoarthritis according to the ILAR (OR 9.9; 95% CI 1.5–109.3). During the first year of tocilizumab administration, neutropenia was registered in one patient, leukopenia — in three cases, and urticaria — in one case.Conclusion. The variant of JIA, the age at the disease onset, and the disease course pattern in the first month of tocilizumab therapy are the predictors of treatment efficacy throughout the year.
Background. Immunological potency of 13-valent pneumococcal vaccine (PCV-13) in children with systemic juvenile idiopathic arthritis (SJIA) is still unstudied. Estimates of the genetically engineered biologic drugs (GEBD) effects on pneumococcal vaccination results also remain controversial.Objective. The aim of the study was to explore the PCV-13 efficacy in patients with SJIA and who is on treatment with monoclonal antibodies against interleukin 6 receptor (tocilizumab) and interleukin 8 receptor beta (canakinumab).Methods. The study included patients under the age of 18 with SJIA in remission or active form of disease vaccinated with PCV-13. The vaccine was administered in single dose of 0.5 ml intramuscularly in patients on treatment with GEBD or 3 weeks before GEBD administration for the first time (for patients with active disease). Vaccination was considered effective at achievement of the minimum protective level of antibodies to capsular polysaccharide of pneumococcus (anti-SPP IgG; ≥ 7 U/ml) or increase of anti-SPP IgG level ≥ 2 times in 4 weeks after vaccination. The anti-SPP IgG levels were measured with enzyme immunoassay.Results. The study included 53 patients (27 girls) in remission of SJIA and 25 (16 girls) in active disease. Median age was 13.3 and 10.8 years respectively. Tocilizumab/canakinumab was administrated in 43/10 and 18/7 patients respectively. Minimum significant anti-SPP IgG level and two-fold increase in anti-SPP IgG level were recorded in 49/53 (92%) and 32/53 (60%) patients with SJIA in remission, as well as in 22/25 (88%) and 18/25 (72%) patients in active disease respectively. PCV-13 immunological potency in patients with SJIA in remission and in active disease (in those who were initially administrated and who did not receive GEBD) did not differ.Conclusion. PCV-13 vaccination allows to achieve protective antibodies level in most of the patients with SJIA in children population regardless of the disease stage and the history of GEBD administration.

The article presents modern ideas about the treatment of systemic lupus erythematosus (SLE). The details of the management of patients with SLE during immunosuppressive and genetically engineered therapy is given. The article also reflects the aspects of rehabilitation, prevention of exacerbations, and follow-up care of children with SLE. The criteria for assessing the quality of medical care for children with SLE are presented. The detailed information on systemic lupus erythematosus for patients with SLE and their parents is outlined specifically.
Background. An important goal of treating patients with juvenile idiopathic arthritis (JIA) is to achieve the best quality of life associated with health. Objective. Our aim was to assess the impact of methotrexate plus etanercept therapy on the quality of life of patients with early and late JIA. Methods. The prospective study included patients with early and late JIA without systemic manifestations. The patients’ quality of life was assessed with the help of questionnaires for children and parents: the Pediatric Quality of Life Inventory (PedsQL) Generic Core Scale, the Pediatric Quality of Life Inventory (PedsQL) Rheumatology Module, and the Health Utilities Index Mark 3 (HUI3). The quality of life was assessed prior to the therapy and after one, six, and 12 months of treatment. Results. 150 children with JIA aged 5.1 (2.0; 17.7) years; 50 children aged 4.0 (2.3–11.4) years in the group of etanercept monotherapy, 50 children aged 5.0 (3.2–9.0) years in the group of methotrexate monotherapy, and 50 children aged 9.9 (6.4–13.0) years in the group of methotrexate plus etanercept combination therapy. All groups showed low scores on all questionnaires before treatment, compared to healthy children. In the course of therapy, there was a tendency for score increase to almost 1.0 according to the HUI3 questionnaire in all groups. After one year of etanercept therapy, the parameters of the quality of life of children with early JIA did not differ from healthy children; the score increased from 56 to 90 p = 0.942 according to the physical functioning scale and from 60 to 85 p = 0.889 according to the emotional functioning scale. In the 2nd group, there was a tendency for score increase, but a statistically significant difference was found across all scales of the questionnaire. After 12 months of etanercept plus methotrexate combination therapy in patients with late JIA, the questionnaire analysis showed that the responses of healthy children and children with JIA differed with probability p = 0.001 for the physical functioning scale, p = 0.001 for the social functioning scale, p = 0.001 for role functioning, and p = 0.001 for the total score. The score increase from 60 to 85 p = 0.789 was noted for emotional functioning scales. Conclusion. The use of questionnaires to assess the quality of life in children with severe chronic diseases can significantly improve the efficacy of treatment and ensure its control.
