Главный принцип современной терапии -целенапра-вленный патогенетический подход, в соответствии с кото-рым основной целью лекарственного или немедикамен-тозного воздействия становятся ключевые элементы раз-вития болезни.Одной из наиболее перспективных «мишеней» фар-макотерапии представляется воспаление. Это связано с тем, что воспалительный процесс лежит в основе патоге-неза практически всех болезней и патологических состо-яний [1, 2]. Очевидно, что локальное и системное воспа-ление определяет развитие «воспалительной» патологии, связанной с инфекционной или аутоиммунной агресси-ей, оно сопровождает любые травмы и ранения, играет принципиальную роль в прогрессировании злокачествен-ных новообразований [3][4][5]. Не вызывает сомнения и важнейшее значение воспалительной реакции в развитии таких болезней, как остеоартрит (ОА), атеросклероз и нейродегенеративные заболевания, которые ранее рас-сматривались как «дегенеративные» или «обменные» [6][7][8].
The purpose of the overview is to analyze the role of apoptosis in the pathogenesis of critical illness and discuss specific features of contributed biochemical processes. The paper reviews 117 publications, 76 of which were published during the recent five years. Published data show that the ability to control endogenic apoptotic processes offers opportunities for the development of functional therapy approaches to various diseases.
Venous thromboembolism (VTE) is the third most frequent cause of death from cardiovascular diseases after myocardial infarction and stroke and the most preventable cause of mortality. VTE is common and potentially life-threatening in patients admitted to ICU, even in spite of preventive care.The purpose of the review is to justify the necessity of preventive care for venous thromboembolism in ICU patients.From over 300 initially selected sources of literature databases (Scopus, Web of science, RSCI, etc.), 99 sources were chosen including 69 that were published during the last five years (2015)(2016)(2017)(2018)(2019)(2020). The exclusion criteria included data of low informative value or disproven data.The review discusses VTE relevance, risk factors for its development, selection and scope of preventive care depending on the risk of thrombosis and hemorrhage, patient management in different clinical settings (impaired renal function, thrombocytopenia, heparin-induced thrombocytopenia, liver dysfunction, indications for installation of vena cava filter).In the world literature, however, there is yet no consensus on the matter under discussion that would have been based on meta-analyses or large randomized studies. No agreement has been reached either in respect of use of mechanical and combined prophylaxis of venous thromboses/PATE, application of ultrasound to detect asymptomatic thromboses. There are no studies on efficacy and safety of pharmacological prophylaxis of VTE in patients with significant hepatic impairment.The review describes that all patients admitted in ICU feature a high risk of VTE development. The scope of preventive care depends not only on VTE risk but also on the risk of a hemorrhage. To prevent the latter, low-molecular weight heparins should be used. For most cases, the choice of heparine dose depends on renal function. When there is a high risk of hemorrhage, mechanical preventive aids are applied.
Цель. Изучить кинетику мочевины в организме после резекции печени в экспериментеМатериал и методы. Опыты проведены на 45 белых крысах (самках) массой 180-220 г. Резекцию пече ни (РП) проводили под эфирным наркозом, удаляя 15-20% от массы органа. Исследовали содержание мо чевины в биологических жидкостях (артериальная кровь, кровь v. porta, v. hepatica, v. renalis, желчь холедо ха, моча) и тканях висцеральных органов (щитовидная железа, легкие, сердце, печень, почки, селезенка, желудок, кишечник) на 3 и, 7 е и 14 е сутки после РП.Результаты. РП, снижая концентрацию мочевины в крови v. hepatica, не приводит к аналогичным изме нениям в артериальной крови. Это сопровождается увеличением ее реабсорбции в почках и повышением концентрации в крови v. porta, которое в зависимости от сроков послеоперационного периода достигается либо снижением экскреции мочевины в просвет тонкого кишечника, либо увеличением ее образования эн тероцитами с дальнейшим поступлением метаболита в портальный кровоток. Если на 3 и сутки после РП поступление мочевины из гепатоцитов в желчевыводящую систему печени не изменялось, то на 7 е сутки оно увеличивалось, а на 14 е тормозилось. РП не вызывала изменения содержания мочевины в ткани же лудка, но приводило к увеличению ее концентрации в ткани двенадцатиперстной и толстой кишок. Не вли яя на содержание мочевины в сердечной мышце, РП вызывала увеличение ее концентрации в легких и щи товидной железе на 3 и, 7 е и 14 сутки послеоперационного периода. На фоне отсутствия аналогичных изменений в артериальной крови это указывает на стимуляцию образования мочевины клетками этих орга нов или на ретенционную задержку метаболита в них.Заключение. РП не только изменяет кинетику мочевины в органах портальной системы, но и активиру ет внепеченочные механизмы, направленные на предотвращение развития дефицита мочевины в артериаль ной крови в результате нарушения ее поступления из оперированного органа в центральный кровоток.Ключевые слова: резекция печени; мочевина; висцеральные органы; кровь; желчь; моча Purpose.
