Статья посвящена 100-летию со дня рождения видного отечественного интерниста, создателя оригинальной терапевтической школы в Сибири, члена-корреспондента АМН СССР и основателя терапевтической клиники Новосибирской областной клинической больницы (1955-1977), почетного профессора НГМУ Аристарха Александровича Дёмина (1918-1977). Приводятся краткие биографические данные об ученом, в том числе в годы Великой Отечественной войны. Ар.А. Дёмин возглавлял кафедру госпитальной терапии НГМИ (ныне-НГМУ) в 1953-1977 гг. Он известен исследованиями послевоенного септического эндокардита (СЭ), соавторством многотомного «Руководства по внутренним болезням». Ар.А. Дёмин отстаивал возможность развития СЭ на неизмененных клапанах сердца, а не только на фоне ревматизма, как тогда считалось (Москва, 1962). Ар.А. Дёминым впервые в отечественной литературе описаны множественные врожденные артерио-венозные аневризмы конечностей, распознана полная врожденная блокада сердца при болезни Толочинова-Роже, впервые в Сибири диагностирована и описана системная красная волчанка. Разработанные Ар.А. Дёминым положения учения о СЭ нашли отражение в 60 докторских и кандидатских диссертациях его учеников. Созданная им в НГМУ оригинальная терапевтическая школа в настоящее время успешно решает современные задачи теоретической и практической медицины. Среди учеников и последователей Ар.А. Дёмина-известные ученые, заведующие (в прошлом и в настоящее время) кафедрами НГМУ акад. Л.
Introduction. Despite the success of current therapeutic approaches and suffi cient supply of clotting factor concentrates, adult patients with hemophilia have a high frequency of developing joint pathologies. Furthermore, an increase in life expectancy has correlated with an increase in the share of patients with concomitant somatic diseases, which are characteristic for this population over the age of 40 years.Aim — to analyze the treatment results of adults with hemophilia living in the city of Novosibirsk and Novosibirsk region and evaluate their orthopedic status, types, frequency of bleeding, and comorbidity structure.Material and methods. A total of 93 patients were enrolled in the study (including 86 patients with hemophilia A and seven patents with hemophilia B). The analysis was carried out separately in subgroups of patients according to age: 18–24, 25–34, 45–44, and over 45 years old. Hemophilia type, severity of disease, strategy of therapy, inhibitor status, and features of concomitant pathology were analyzed.Results. A high frequency of spontaneous bleeding was found in all age groups of patients enrolled in the study (39.6 and 34.9 % during 6 and 12 months, respectively). The average frequency of joint bleeding was 4 cases per person per year in the group of patients aged ≥ 18 years.In evaluation of orthopedic status, it was remarkable that there was a signifi cant number of target joints (262, n = 93), which amounted to 2.8 defected joints per patient (the elbow, knee, and ankle joints were the most frequently involved in pathological process). All patients aged over 18 years with severe and moderate form of disease had a musculoskeletal complication with involvement of one or more target joints.The number of diseases per one patient prevailed in the age group over 45 years old and amounted to 4.12 comorbidities per patient. The prevalent diseases in the structure of concomitant pathology in adult patients with hemophilia were chronic viral hepatitis C (43.1 %, n = 42), stage I–III arterial hypertension (27.9 %, n = 26), stomach and duodenum diseases as well as gallbladder pathology (33.3 %, n = 31 and 26.9 %, n = 25, respectively), urolithiasis and chronic secondary pyelonephritis (21.5 %, n = 20).Conclusion. Despite the use of clotting factor concentrates in patients with severe and moderate hemophilia, the frequency of joint hemorrhages remains high, regardless of the age prophylactic replacement therapy was started. Additional measures must be taken to prevent the appearance of critical disease phenotype with frequent spontaneous bleeding.
Цель. Анализ основных эпидемиологических показателей множественной миеломы (регистрируемая заболеваемость, распространенность, летальность, выживаемость) в крупном мегаполисе-г. Новосибирске (Сибирский федеральный округ). Материалы и методы. Объектом исследования служила медицинская документация 335 пациентов с впервые диагностированной множественной миеломой (ММ), наблюдавшихся с 1 января 2006 г. по 31 декабря 2016 г. в Городском гематологическом центре Новосибирска. Медиана возраста больных составила 67 лет (диапазон 30-89 лет), женщин было 218 (65 %), мужчин-117 (35 %). Результаты. Средняя регистрируемая заболеваемость ММ в Новосибирске за последние 10 лет увеличила сь в 1,6 раза, а распространенность-в 4,9. Эти показатели составили 2,4 и 13,8 случая на 100 000 населения в год соответственно с линейным трендом роста, что указывает не только на повышение числа пациентов с впервые диагностированной ММ, но и на увеличение продолжительности их жизни. Показатели заболеваемости и распространенности ММ статистически значимо выше среди женщин, чем мужчин, что, вероятнее всего, объясняется особенностями административных факторов региона. Ежегодная летальность больных ММ уменьшилась с 28,3 до 8,2 % с отрицательным линейным трендом на протяжении всего анализируемого периода, что, вероятнее всего, связано с возможностью использовать новые лекарственные средства и трансплантационные технологии. Заключение. Полученные эпидемиологические данные позволят планировать оказание своевременной и эффективной помощи пациентам с ММ, а также разработать систему обоснованного распределения дорогостоящего оборудования и лекарственных препаратов.
Background. An increase in the number of patients with multiple myeloma (MM) necessitates the creation of reliable tools for assessing their somatic status (comorbidity) in order to personalize the optimal treatment regimen that helps to minimize its toxicity, improves survival and patients quality of life.The objective of this study was to modify the MM comorbidity index (MCI) by adding an additional variable reflecting the biological properties of the tumor, and to determine the informativeness of the new scale – a modified MM comorbidity index (M-MCI), to predict the outcome and select personalized therapy in patients with MM in real clinical practice.Materials and methods. From January 2012 to December 2017 the study included 369 patients with newly diagnosed MM (134 men and 235 women) who were hospitalized in the hematology department of the City Clinical Hospital No. 2, Novosibirsk. The median age of the patients was 67 (32–82) years. The prognostic value of concomitant diseases and individual prognostic factors in relation to the overall survival of patients with MM was evaluated.Results. Cox multivariate analysis showed that the most significant predictors of reduced overall survival of patients with MM are impaired renal function (glomerular filtration rate <30 ml / min / 1.73 m2 (according to the CKD-EPI formula), general condition according to the Karnowski scale ≤70 %, chronic obstructive pulmonary disease with moderate (50 % ≤ forced expiratory volume in 1 second <80 %) and severe (30 % ≤ forced expiratory volume in 1 second <50 %) severity of bronchial obstruction and the ratio κ / λ free light chains <0.04 or >65. These factors were combined into a weighted 5‑point scale M-MCI. A comparative analysis of survival depending on the value of the M-MCI allowed us to distribute patients with MM into groups of high (M-MCI 3–4 points) and standard (M-MCI 0–2 points) risk with significantly different indicators of overall survival (median overall survival amounted to 15.5 months in the high and 60 months in the standard risk group; χ2 = 58, p <0.016) and confirm the prognostic value of M-MCI in relation to the outcome of MM.Conclusion. In terms of its prognostic significance in predicting an adverse outcome, the proposed M-MCI scale is superior to its prototype – the MCI. The median overall survival in the high-risk group according to the M-MCI was 15.5 months compared to 20 months according to the MCI; the median overall survival in the group the standard risk was 60 and 50 months, respectively (χ2 = 58 (M-MCI) versus χ2 = 42 (MCI); p <0.001). The advantages of M-MCI are also its more accurate assessment of the physical condition of patients with MM and its simple clinical applicability.
