Резюме Качество эндодонтического лечения во многом зависит от механической очистки корневого канала, которая осуществляется в большинстве случаев машинными никель-титановыми инструментами. Несмотря на большое их разнообразие, универсальной системы, являющейся простой и эффективной, позволяющей работать в корневом канале со сложной анатомией, нет. Поэтому создаются все новые инструменты. Цель. Сравнить эффективность обработки корневых каналов зубов тремя разными системами (Reciproc Blue, Mtwo-file и Soco SC Pro). Провести анализ полученных результатов при помощи сканирующей электронной микроскопии, оценить качество механической очистки корневого канала при работе этими инструментами. Материалы и методы. В исследовании использовались 30 удаленных зубов (с кривизной корня более 25 градусов), разделенных на 3 группы (по 10 в каждой группе). В 1 группе была проведена механическая обработка с использованием Reciproc Blue. Во 2-й группе использовались файлы системы Soco SC Pro, а в третьей контрольной группе-Mtwo-file. Затем были изготовлены шлифы и произведена оценка результатов на электронном сканирующем микроскопе.
The aim of this study is to evaluate the efficiency of ultrasound in endodontic practice in various clinical cases. Objects and methods. Extracted teeth, divided into 2 experimental groups, were used to assess the degree of the tooth cavity cleaning. In the first group, the tooth cavity was treated mechanically and medically without ultrasound use. In the second group, the tooth cavity was treated mechanically and medically with ultrasound use. Ultrasound treatment was found to improve significantly the level of tooth cavity cleaning at the preparatory stage for endodontic treatment. Extracted teeth, divided into 3 experimental groups were used in our study to demonstrate the efficiency of passive ultrasonic irrigation. Results and discussion. The results of scanning electron microscopy of extracted teeth slices which show the level of purification of the root canal previously treated mechanically and medically are attached to the article. Our research leads to the conclusion that the activation of the irrigation solution with sound or ultrasound leads to significant increase in quality of root canal cleaning. The article presents 3 clinical cases of ultrasound application in procedure of extraction a silver post, a fiberglass post and a fragment of the canal filling instrument from the root canal. The research has found that use of ultrasound in modern endodontic practice facilitates the extraction of intracanal posts and instrument fragments, with duration of intervention being reduced and the possibility of saving the tooth in complicated clinical cases being increased. Conclusion. The results of the research lead to the conclusion that ultrasound should be used almost at every stage of endodontic treatment.

Use of ultrasound in the clinic of an endodontist offers solution to number of major problems, such as preparation of a dental cavity for further endodontic treatment, a proper irrigation of root canals, removal of different types of dental posts and parts of broken instruments from the root canal, while reducing duration of intervention and increasing the possibility of saving the tooth in complicated clinical cases.
Compliance with the algorithms of endodontic treatment in most cases ensures the quality of treatment. However, real, long-term and sometimes immediate results that occur in clinical practice are not always successful. In general, the main reason for the unfavorable outcome of dental treatment is an incomplete sanitation of the root canal system. Modern publications by J. F. Siqueira, I. N. Rocas, and D. Ricucci (2019) note that many issues of endodontic treatment are unresolved. It follows that improving the quality of endodontic treatment is still relevant.Aim. To compare two systems of instruments for root canal preparation with different types of rotation.Materials and methods. To identify a tool that will increase the efficiency of root canal preparation, taking into account their anatomy using CBCT, 24 teeth were removed, taken according to orthodontic or pristidintic indications, without previous endodontic treatment. Among them there were 2 incisors, 1 canine, 6 premolars, 15 molars. In total, 60 canals were examined.Results. The shape changes throughout the canal, turning into a round, oval, slit-like, complementing the configuration by the isthmuses between the canals in 13.3% of cases. WaveOne Gold showed a change in cross-sectional shape after preparation, while ProTaper Universal retained a rounded section of 6.67% after preparation of the slit-like channels.Conclusion. Identified priority technique of root canal preparation, allowing to reduce the number of instruments and, as a consequence, the possibility of technological failures in the root canal to reduce machining of the mouth of the channel, retaining dentine playing a major role in the further prosthesis and positive prediction of remote results.
