Aim.To evaluate the functioning of an implantable pediatric axial pump «DON-3» for bypassing the left ventricle of the heart in experiments on sheep.Materials and methods. Five sheeps at the age of 12–18 months performed paracorporeal installation of the children’s axial pump «DON-3» according to the scheme «left ventricle – aorta».Results.A technique was developed and 5 chronic experiments were conducted on sheep to assess the model of the children’s axial pump «DON-3». The duration of the experiments in this series averaged 9 ± 5 days. The main indicators of hemodynamics, acid-base balance were within the norm. Conducted morphological and histological studies of the kidneys, liver and lungs did not reveal the presence of zones of ischemia and thromboembolism.Conclusion.The results of this series of experiments showed satisfactory results, suggesting further research on the development of prototypes of a pediatric pump for clinical practice.
The aim:to analyze the waiting list for heart transplantation from 2010 to 2017 and to characterize recipients with chronic heart failure III–IV NYHA Class.Methods.The study comprised 997 patients (139 [14%] females and 858 [86%] males) included in the waiting list for heart transplantation the period from January 2010 to December 2017. The average age of patients on the waiting list was 49.0 ± 12.0 (from 10 to 78 years). Before making a decision on inclusion in the waiting list, all patients underwent clinical and instrumental examination, including general clinical studies, echocardiography, measurement of central hemodynamic parameters using a Swan–Gans catheter, computer and/or magnetic resonance imaging of the chest, abdominal and brain.Results.Heart transplantation was performed on 728 patients (99 females – 13.6% and 629 males – 86.4%) including 18 children aged 12 to 17 years (14.18 ± 2.07 years). Mortality in the waiting list in 2010 was 16.1%, compared with 3.2% in 2017.

Материалы и методы. В исследование были включены 17 пациентов, оперированных в период с 2012-го по октябрь 2017 г. в ФГБУ «Националь-ный медицинский исследовательский центр трансплантологии и искусственных органов имени акаде-мика В.И. Шумакова». С целью замещения насосной функции ЛЖ сердца использовалась миниатюрная имплантируемая система вспомогательного кровообращения «Аппарат вспомогательного кровообра-щения -носимый» (АВК-Н) (Россия). До имплантации системы длительной механической поддержки кровообращения все пациенты были обследованы по программе потенциальных реципиентов на транс-плантацию сердца. Среди оперированных пациентов было 16 (94,1%) мужчин и 1 (5,9%) женщина, сред-ний возраст составил 52,64 ± 10,56 (от 33 до 67 лет). Все пациенты имели застойную сердечную недо-статочность III-IV функционального класса по NYHA, рефрактерную к оптимальной медикаментозной терапии. Причиной развития сердечной недостаточности в 12 (70,58%) случаях явилась дилатационная кардиомиопатия, а в 5 (29,42%) -постинфарктная систолическая дисфункция левого желудочка. Имп-лантация системы АВК-Н проводилась потенциальным реципиентам донорского сердца с терминаль-ной стадией ХСН со снижением фракции выброса ЛЖ до 10%. Результаты. В результате настоящего исследования был разработан ряд технологических аспектов имплантации системы, облегчающих по-следующее выполнение ТС. Заключение. Наш опыт оптимизации хирургической тактики импланта-ции системы АВК-Н в качестве «моста» к трансплантации сердца продемонстрировал возможность и безопасность ее активного применения у пациентов с терминальной сердечной недостаточностью, как находящихся в листе ожидания трансплантации сердца, так и имеющих временные противопоказания к ее выполнению.Ключевые слова: критическая сердечная недостаточность, трансплантация сердца, механическая поддержка кровообращения.
