Аннотация. Последнее десятилетие исследования ученых направлены на коморбидную патологию. Заболеваемость сахарным диабетом и хронической обструктивной болезнью легких растет с каждым годом. В настоящее время активно изучаются цитокиновый статус у больных хронической обструктивной болезнью легких и коморбидной эндокринной патологией. Цель исследования: изучение цитокинового статуса у больных ХОБЛ и сахарным диабетом 2 типа в зависимости от целевого уровня гликемического контроля. Материалы и методы. Всего обследовано 124 больных в возрасте от 31 до 80 лет, которые были разделены на основную группу (n=64)-пациенты с ХОБЛ и СД 2 типа, группу сравнения 1 (n=29)-пациенты с изолированной ХОБЛ и группа сравнения 2 (n=31)-пациенты с изолированным СД 2 типа. Всем пациентам проводились общеклинические и инструментальные обследования, а также исследование цитокинового статуса (ИЛ-4, ИЛ-8, ФНО-α). Основная группа была поделена на три подгруппы в зависимости от целевого уровня гликированного гемоглобина. Первая подгруппа (n= 22) имела целевой уровень гликированного гемоглобина (HbA1c) менее 7%, вторая (n=20)-целевой HbA1c менее 7,5%, третья подгруппа (n= 22)-целевой HbA1c менее 8%. Результаты исследования цитокинового статуса были сравнены с аналогичными показателями в группе здоровых, которую составили 25 добровольцев. Результаты. У больных ХОБЛ в сочетании с сахарным диабетом 2 типа выявлен дисбаланс в системе про-и противовоспалительных цитокинов, который был достоверно более выражен у пациентов с целевым уровнем HbA1c менее 8,0%. Выводы. Выявленный в ходе исследования дисбаланс провоспалительных и противовоспалительных цитокинов у больных ХОБЛ в сочетании с СД 2 типа свидетельствует о прогрессировании системного воспаления у коморбидных больных. Показана зависимость цитокинового статуса от целевых показателей углеводного обмена.
Медико-социальное значение болезней органов дыхания в современных условиях велико и определяется, прежде всего, их крайне высокой частотой среди различных контингентов населения. Болезни органов дыхания по уровню заболеваемости и по распространенности занимают первое место. Согласно статистике, сегодня на долю органов дыхания приходится около 40 % всех случаев заболеваемости, которая превосходит уровни заболеваемости другими классами болезней. В структуре причин обращаемости за медицинской помощью их удельный вес на различных территориях составляет от 29,2 до 43,5 % среди взрослых и от 65,4 до 83,8 % - среди детей. Своевременная диагностика и лечение заболеваний легочных болезней на ранней стадии может сохранить здоровье человека и предотвратить возникновение хронических заболеваний, а также поможет начать незамедлительное их лечение. Поэтому разработка информационно-программного обеспечения, используя нейросетевое моделирование, является актуальной задачей. Выделение топологических групп по легочным заболеваниям проводилось с использованием самоорганизующихся карт Кохонена. По результатам классификации было проведено обучение нейронных сетей, используя алгоритм «многослойного персептрона» методом «обратного распространения», и получены математические модели. Разработанная система для диагностики легочных заболеваний на основе нейросетевого моделирования с высокой точностью способна определять такие болезни, как пневмония, COVID-19, туберкулез и рак легких. По данным, полученным в результате исследования, можно судить о перспективности внедрения инновационных технологий, таких, как нейронные сети, в область медицины. Программное обеспечение, полученное на основе нейросетей, способно ускорить процессы постановки диагноза, тем самым оно оптимизирует работу медицинских учреждений The medical and social significance of respiratory diseases in modern conditions is great and is determined, first of all, by their extremely high frequency among various contingents of the population. Therefore, the development of information software using neural network modeling is an urgent task. The selection of topological groups for pulmonary diseases was carried out using Kohonen's self-organizing maps. Based on the classification results, neural networks were trained using the “multilayer perceptron” algorithm using the “back propagation” method and mathematical models were obtained. The developed system for diagnosing lung diseases based on neural network modeling is capable of detecting diseases such as pneumonia, COVID-19, turbeculosis, and lungs' cancer. According to the data obtained as a result of the study, one can judge the prospects for the introduction of innovative technologies, such as neural networks, in the field of medicine. The software obtained on the basis of neural networks is able to speed up the process of making a diagnosis, thereby optimizing the work of medical institutions
В 2018 году по данным Всемирной организации здравоохранения было выявлено более 10 млн. человек, заболевших туберкулезной инфекцией, 1,6 млн. из которых погибли. Причинами высокой смертности могли послужить: низкая эффективность лечения, множественная лекарственная толерантность к препаратам как первой, так и второй линий, применяемых в лечении, неосведомленность о соматическом состоянии пациентов, развитие побочных действий и снижение реактивности организма [1]. За последние годы по данным статистических обзоров во всем мире, в том числе и РФ, отмечается значительный рост числа больных, инфицированных мультирезистентными штаммами M.tuberculosis. В режимы химиотерапии таких больных включают большое количество противотуберкулезных препаратов, удлиняются сроки лечения, что способствует увеличению побочных реакций токсического характера, как от самих лекарственных препаратов, так и от их метаболитов. Также одним из возможных и недооцененных триггеров, влияющих на течение туберкулеза, может быть функциональное состояние щитовидной железы. Именно поэтому тиреоидный статус пациентов с туберкулезной инфекцией может играть значительную роль в развитии, течении и исходе данного заболевания. Also, one of the possible and underestimated triggers that affect the course of tuberculosis may be the functional state of the thyroid gland. That is why the thyroid status of patients with tuberculosis infection can play a significant role in the development, course and outcome of this disease. Thus, the problem of modern medical society may be the lack of knowledge and the lack of comprehensive examinations of polymorbid patients, which are the basis of the group of patients with pulmonary tuberculosis
Цель исследования -выявить клинические особенности течения хронической обструктивной болезни легких у пациентов с раком гортани и их динамику при использовании в лечении витамина D в комплексной терапии. Было 2 этапа исследования. На первом этапе исследования изучали клинические особенности течения хронической обструктивной болезни легких у больных раком гортани. В основную группу вошли 59 пациентов с ХОБЛ и раком гортани. Группу сравнения составили 29 пациентов с изолированной ХОБЛ. На втором этапе изучали клинические проявления ХОБЛ у пациентов с раком гортани в динамике через 1 год: основную группу составили 30 больных ХОБЛ и раком гортани, которые в комплексном лечении получали витамин D (колекальциферол) 500 МЕ в сутки, группу сравнения -29 пациентов, которые не принимали витамин D. Для больных ХОБЛ и раком гортани характерны: высокая частота встречаемости курения и коморбидных заболеваний (ишемическая болезнь сердца, гипертоническая болезнь и патология желудочно-кишечного тракта), большее количество жалоб, связанных со специфическим процессом, и достоверно чаще выявление пневмофиброза при рентгенологическом исследовании по сравнению с изолированным течением ХОБЛ. У пациентов основной группы наблюдались достоверно более выраженные дисбаланс цитокинового статуса, частота встречаемости недостаточности и дефицита витамина D, анемии и лабораторного синдрома гиперкоагуляции и значимо высокие уровни С-реактивного белка, чем у пациентов группы сравнения. Применение витамина D в течение 1 года в комплексном лечении ХОБЛ и рака гортани не выявило нежелательных влияний препарата, способствовало снижению маркеров воспаления, уменьшению дисбаланса в системе цитокинов и повышению частоты встречаемости физиологической нормы витамина D.
