Мета дослідження – знизити частоту акушерських ускладнень, пов’язаних із порушеннями серцево-судинної діяльності та ритму серця в жінок з недиференційованою сполучнотканинною дисплазією (НДСТ) шляхом включення до складу протокольної програми лікування карнітин-аргінінового комплексу. Матеріали та методи. Обстежено 58 вагітних, в яких діагностовано від 2 до 8 окремих ознак НДСТ, у тому числі пролапс мітрального клапана (ПМК) із та без частої екстрасистолії, і 14 вагітних групи контролю. Для передпологової підготовки вагітних із НДСТ і профілактики акушерських ускладнень у дослідній групі застосовано метаболічну підтримку шляхом внутрішньовенного вливання 4,2 г L-аргініну та 2,0 г L-карнітину. Результати дослідження та їх обговорення. У вагітних із ПМК та екстрасистолічною аритмією на фоні НДСТ у вихідному стані спостерігали порушення ремоделювання серця з розвитком помірної систоло-діастолічної дисфункції міокарда. У них достовірно частіше діагностували невиношування вагітності, пізній гестоз, плацентарну дисфункцію, аномалії пологової діяльності, пологовий травматизм, післяпологові кровотечі тощо. Ехоструктурні аномалії серця виступали предикторами розвитку аритмій. Під впливом комплексного лікування з включенням L-аргініну та L-карнітину в групі пацієнток із ПМК відбувалось суттєве зменшення частоти шлуночкової або змішаної екстрасистолії (на 90,6 і 77,8 %), а при суправентрикулярній екстрасистолії антиаритмічний ефект спостерігали лише в 37,5 % пацієнток, що обґрунтовувало додаткове застосування небівололу. Висновок. У вагітних із ПМК та екстрасистолічною аритмією на фоні НДСТ у вихідному стані спостерігають порушення ремоделювання серця з розвитком помірної систоло-діастолічної дисфункції міокарда, що є тригером ускладненого перебігу вагітності й пологів, а застосування в них комплексної терапії з включенням L-аргініну і L-карнітину приводить до достовірного покращення параметрів центральної кардіогемодинаміки, що супроводжується суттєвим зменшенням частоти розвитку екстрасистолічної аритмії та ускладнень вагітності.
