BackgroundPatients with atopic dermatitis (AD) can develop autoantibodies against intracellular proteins. AD patients often suffer from herpes viruses (HV) infection which complicates the inflammatory process in the skin. The aim of the study was to reveal IgE and IgG antibodies (abs) specific to some skin antigens and to compare their levels with the severity of AD with HV infection in children.MethodsIgE and IgG abs specific to tissue antigens, total IgE, IgE-abs to environmental common allergens as well as IgG abs specific to HV were detected in serum samples by ELISA in 157 AD children.ResultsIgE and IgG antibody production to keratin and elastin was observed in children with AD and elevated proportionally to the severity of AD. IgG – abs to herpes simplex virus was increased in children and associated with the severity of clinical course of AD.ConclusionOur data shown that clinical course of severe AD is accompanied with autoimmune response to epidermal antigens (keratin and elastin). Elevated levels of the autoantibodies, especially against the background of HV infection may be useful serological parameter for monitoring of the disease activity.
BackgroundAtopic dermatitis (AD) is a chronic skin disease predominately with the beginning in childhood. One hundred autoantigens and more are capable of binding with autoantibodies in AD patients, including autoantigens with common epitopes of EBV. Aim of study was to reveal IgE- and IgG-autoreactivity to some tissue proteins and IgG-abs to some herpes viruses and to compare the received dates with the severity of AD.MethodsIn the sera of 157 children with AD (from 1 to 17 years old) the levels of IgG- and IgE-abs to keratin, collagen III and VI, elastin, myosin and basic myelin protein were determined in adapted ELISA. The levels of IgG-abs to HSV, CMV and EBV were determined using commercial kit of ELISA. The level of total IgE and IgE-antibodies to allergens were detected using an autoanalyzer.ResultsIn all age groups of patients (with light AD, middle AD and severe AD) elevated levels of total IgE were revealed, especially in children with severe AD (Me = 360 KU/L (80 – 1160); P < 0.05). An increased contents of IgE-abs to keratin (Me = 2.71 ME/mL (1.4–13.69); P < 0.05) and elastin (Me = 2.69 ME/mL (1.4–2.78); P < 0.05) and IgG-abs to keratin (Me = 296.21 μg/mL (127.88–342.01); P < 0.05) were revealed in children with severe AD in comparison with healthy children. Whereas in children with light AD the levels of these antibodies were not significantly increased (P > 0.05), but were being increased in proportion to a severity of AD. We revealed a correlation between the levels of total IgE and the levels of IgE-abs to keratin in children with severe AD (r = 0.35; P < 0.05). In addition we revealed that the levels of IgG-abs to HSV were being increased in proportion to a severity of AD. Maximal increased level of IgG-abs to HSV was in children with severe AD (Me = 2.93 units of OD (0.14–3.15); P < 0.05).ConclusionsThe detection of the elevated levels of the autoantibodies, especially against the background of herpetic infection, amplifies the phenotypic and diagnostic criteria of severity of AD and allows the clinician to choose the most adequate and effective treatment of AD patients.
Резюме. Изучались эффективность и безопасность включения мелатонина в комплексную терапию ато пического дерматита. Дизайн исследования: сравнительное рандомизированное открытое исследование в параллельных группах. Пациенты и вмешательство: обследованы 46 больных атопическим дерматитом, в стадии обострения, среднетяжелое течение, третий возрастной период. Больные были рандомизированы к приему комплекса стандартных методов лечения и мелатонина, 3 мг ежедневно в 21.00 в сочетании со стандартной терапией. Основная конечная точка: интенсивность и распространенность поражения кожи по шкале SCORАD через 3 недели. Фармакадинамическая оценка включала содержание и функциональные свойства Т и В лимфоцитов, фагоцитирующих клеток, уровня IgE, IL 4, IFNγ в крови в утренние и вечерние часы. Результаты: после завершения лечения клинический эффект зарегистрирован в опытной и контрольной группах, однако в опытной группе он был статистически значимо выше. Так, у пациентов, принимавших мелатонин, распространенность кожных проявлений уменьшилась на 84%, а в контрольной группе на 60%. Включение мелатонина в комплексную терапию снижало относительный риск развития неблагоприятного исхода (определяемого как отсутствие 20% ответа по SCORАD) на 63%. Клинический эффект в опытной группе ассоциировался с повышением содержания CD8 + клеток, IgM, показателя Fc зависимого фагоцитоза моноцитами и нейтрофилами. При этом достоверно снизился уровень IgE и IL 4 в сыворотке крови. В опытной группе выявлены достоверные различия в утреннем и вечернем содержании CD3 + , CD4 + , CD8 + , CD16 + клеток, сывороточной концентрации IFNγ. Результаты пилотных исследований обосновывают целе сообразность проведения клинических испытаний эффективности мелатонина у больных атопическим дерматитом на большей выборке.

Одним из механизмов, ограничивающих пролиферативный потенциал клеток, является укорочение теломер. Теломеры -это хроматиновые структуры, которые кэпируют и защищают кон-цы хромосом. У позвоночных они состоят из тандемных гексамерных повторов (TTAGGG), ассо-циированных с разнообразными специфичными белками. Иммунная система чрезвычайно чувстви-тельна к потери теломер, так как для Т-и В-лимфоцитов пролиферация является не только частью процесса самообновления, но и необходима для выполнения их биологических функций. Укороче-ние теломер в лимфоцитах может способствовать развитию иммунной недостаточности и предрас-полагать к аутоиммунным процессам в пожилом возрасте. В данной работе была исследована дли-на теломер, темпы укорочения и вариабельность теломер в В-лимфоцитах у пациентов с ревматоид-ным артритом и у здоровых доноров. Было выявлено, что длина теломер в В-клетках в среднем на 1,9 и 2,3 килобаз длиннее в сравнении с CD4 + и CD8 + Т-лимфоцитами соответственно у здоровых доно-ров. Эта же тенденция наблюдается у пациентов с ревматоидным артритом. Темп укорочения теломер в В-лимфоцитах у здоровых доноров составил 20 пар оснований в год, тогда как при ревматоидном артрите он был незначительно больше. Таким образом, укорочение теломер в В-лимфоцитах, так же, как и другие механизмы старения, может способствовать развитию аутоиммунных заболеваний.Ключевые слова: теломеры, В-лимфоциты, ревматоидный артрит. Shortening of telomeres represents an important mechanism that limits the proliferative potential of cells. Telomeres are chromatin structures that perform capping and protection of the chromosome ends. In vertebrates, they consist of tandem hexameric repeated sequences (TTAGGG) that are associated with various specific proteins. Immune system is extremely susceptible to telomere loss, since cellular proliferation underlies self-renewal of T and B lymphocytes, being also quite necessary for their biological functions. Telomere loss in lymphocytes may contribute to development of immune deficiency, and predispose for autoimmune responses in the elderly. In this study, we analyzed telomere length, the rates of telomere loss, and the variability of telomeres in B cells from rheumatoid arthritis patients, and in healthy donors. Average values of telomere length in healthy donors were found to be, respectively, 1.9 and 2.3 kb longer for B cells, as compared with CD4 + и CD8 + Т-lymphocytes. The same tendency was observed in rheumatoid arthritis patients. Estimated rate of telomere loss in B-cells was 20 bp per year in healthy donors, while being slightly faster in rheumatoid arthritis patients. Резюме. Резюме. В настоящей работе исследовали функциональную активность тимуса у доноров и пациентов с различной формой бронхиальной астмы с помощью методики определения количества TREC-позитивных Т-клеток. Несмотря на известные нарушения показателей Т-клеточного гомеоста-за, группа пациентов с бронхиальной астмой достоверно не отличались от группы доноров по уровню TREC в CD4 + и в CD8 + лимфоцитах. Также не было выявлено достоверных различий по количеству TREC-содержащих клеток в зависимости от формы астмы и уровня IgE. Однако группа пациентов, болеющих от нескольких дней до года, демонстрировала увеличение количества TREC в CD4 + и CD8 + лимфоцитах по сравнению с донорами и группой пациентов с длительностью заболевания больше года и находившихся на базисной терапии с применением глюкокортикостероидов. Возможно, уси-ление тимопоэза на начальных стадиях заболевания может быть связано с тимусным стромальным лимфопoэтином, который вносит вклад в развитие бронхиальной астмы, и в норме участвует в про-цессе образования Т-клеток. THYMOPOIESIS INTENSITY IN PATIENTS WITH BRONCHIAL ASTHMA, DEPENDING ON DURATION OF THE DISEASEAbstract. Functional activity of thymus in normal donors and patients with various clinical forms of bronchial asthma was evaluated by means of assessing TREC-positive T-cell numbers. In spite of well-known alterations in parameters of T-cell homeostasis, TREC levels in CD4 + and CD8 + lymphocytes from the patients with asthma did not differ significantly from those in donors. There were also no significant differences in TREC-containing cell numbers, dependent on clinical types of bronchial asthma and IgE levels. However, the patients with a recent disease onset (duration of several days to one year) showed increased TREC positivity in CD4 + and CD8 + lymphocytes, as compared with donors and those patients who underwent basal therapy with glucocorticoids, and during long-term disorder (> 1 year). Enhanced thymopoiesis at initial stages of the disease may be associated with thymic stromal lymphopoietin which contributes to development of bronchial asthma, and normally participates in the process of T cell production.
