Цель исследования-улучшение результатов лечения послеоперационной лимфореи, развивающейся после радикальных оперативных вмешательств на молочной железе посредством применения фотодинамической терапии. Материал и методы. В исследовании оценивается влияние фотодинамической терапии (ФДТ) на выраженность и длительность лимфореи у 8 пациенток, которые были оперированы по поводу рака молочных желез в объеме радикальной мастэктомии по Мадден. У этих пациентов длительность послеоперационной лимфореи варьировала от 7 мес до 1,2 года. Для проведения «лимфостатической фотодинамической терапии» мы использовали Фотосенс и диодный лазер с кварцевыми моноволоконными световодами длиной 1,5-3 м, диаметром 400-600 мкм. Раствор фотосенсибилизатора вводился внутривенно, струйно, медленно, из расчета 1-2 мг/кг. Для подведения света в полость лимфокисты мы предварительно устанавливали в нее дренажную трубку через которую проводили световод с торцевой микролинзой, дающей круговое рассеивание с четкой границей светового пятна. Результаты. Визуально у пациентов с лимфореей после мастэктомии через 4 ч после ФДТ определялся незначительный отек зоны облучения и окружающих мягких тканей, который достигал максимума через 24 ч и постепенно уменьшался в течение последующих 48 ч. Отметим, что к концу первых суток после ФДТ у всех пациентов объем отделяемого по дренажу из послеоперационной раны составил в среднем 185 мл по сравнению с предыдущим периодом-225 мл, на 7-е сутки объем лимфореи в среднем снизился до 154 мл. Через 2 нед она составила в среднем 102 мл, а через 3 нед-72 мл. Окончательная остановка лимфоистечения происходит в среднем через 20 сут. Вывод. Эффект фотодинамической терапии в виде повреждения эндотелия кровеностных и лимфатических сосудов был использован в качестве профилактики и лечения послеоперационной лимфореи. Выполнение фотодинамической терапии приводит только к локальному лимфостазу.
