Relevance. Hemorrhagic fever with renal syndrome (HFRS) is an acute viral zoonosis. Being widespread in Eurasia, it occupies a leading place in Russia among natural focal human diseases. The vast majority of HFRS cases in Russia, about 98%, are associated with Puumala virus. The disease characterized by a wide range of clinical manifestations. Early specific diagnostics appears to be of a great importance for starting timely pathogenic therapy.The aim of the study was to clarify the diagnostic value of hantavirus antibodies detection in the HFRS suspected patients urine. Materials and methods. Blood sera and urine samples from 68 patients of the Infectious Diseases Hospital in Ufa, obtained with a 2-day interval, as well as urine and blood serum samples of 15 convalescents after 1, 2, 3 and 6 months of disease onset were examined for hantavirus antibodies. 53 blood sera and urine samples from Moscow, Moscow and Samara regions patients taken at different times over the disease course were investigated in parallel. Antibodies were detected by the indirect immunofluorescence method.Results. On the 3rd, 4th, 5th and 6th days of illness, in the presence of antibodies in the blood serum, antibodies in the urine were detected in 85.7%, 89.4%, 93.1% and 100% of patients, respectively. The vast majority amount of antibodies was excreted in the urine from 5 to 11 days, which corresponds with the oliguric stage of the disease. In the convalescent period, antibodies were still detected in urine after 1, 2 and 3 months in 86.7%, 46% and 20% of cases, respectively, and were not detected after 6 months, which probably reflects the process of long-term restoration of the kidneys function. A moderate positive correlation of antibodies in serum and urine was observed only in the oliguric period of the disease.Conclusions: Detection of hantavirus antibodies simultaneously in blood serum and urine of febrile patients instead of paired blood sera investigation provides HFRS diagnosis in the very first days of hospitalization and prevent severe complications due to timely pathogenic therapy.

Уфы с подозрением на острые кишечные инфекции и переведенных после клинико-лабораторного обсле-дования либо из приемного отделения по экстренным показаниям, либо из кишеч-но-диагностического отделения после установления непрофильного характера за-болевания в другие стационары. Выделены наиболее информативные клинические симптомы и анамнестические данные при дифференциальной диагностике острых кишечных инфекций с острой хирургической патологией органов брюшной полос-ти (острым аппендицитом, кишечной непроходимостью, холециститом и желчнока-менной болезнью, ущемленной грыжей, дивертикулярной болезнью, перитонитом, панкреонекрозом), с опухолями кишечника и тромбозом мезентериальных сосудов. При дифференциальной диагностике большого круга непрофильных заболеваний были использованы результаты дополнительных исследований: общего анализа кро-ви и мочи, копроцитограммы, бактериологического и иммуноферментного анализов испражнений и рвотных масс на патогенные и условно-патогенные энтеробактерии, ротавирусы, а также данные УЗИ и рентгенографии органов брюшной полости, рек-тороманоскопии.Ключевые слова: острые кишечные инфекции, дифференциальная диагностика, осложнения, хирургия, брюшная полость.
Introduction. The polyetiology and diversity of clinical manifestations of acute intestinal infections determine the difficulties of the differential diagnosis with acute surgical pathology of the abdominal organs.This study aims to elucidate the causes of the erroneous diagnosis of surgical pathology of the abdominal organs which have symptoms similar to acute intestinal infections.Materials and methods. The paper presents an analysis of 2940 medical records of patients hospitalised in the intestinal infections department of the Infectious Diseases Hospital No. 4 of Ufa with suspected acute intestinal infections. Among these in 42 patients following the clinical and laboratory examinations, the nature of the disease was established as a disorder requiring surgical treatment, and they were transferred — urgently, more often than not — to appropriate hospitals.Results and discussion. The authors identified the earliest and most informative clinical, anamnestic and laboratory parameters aimed at making a timely differential diagnosis of acute intestinal infections with a variety of surgical pathology of the abdominal organs, primarily with acute appendicitis, intestinal tumours, somatic diseases of the gastrointestinal tract, haemorrhagic fever with renal syndrome and myocardial infarction.Conclusion. When a non-infectious disease was diagnosed the authors carefully examined the results of general clinical tests, fecal panel, results of bacteriological and enzyme-linked immunosorbent assays of feces and vomit for pathogenic and opportunistic enterobacteria, rotaviruses, ultrasound and X-ray images of the abdominal organs.
Очередной подъем заболеваемости новой коронавирусной инфекцией (COVID-19), обусловленный проникновением дельта-штамма SARS-CoV-2 в Россию во второй половине апреля 2021 г., характеризуется быстрым распространением, тяжелым течением у взрослых и ростом случаев заболевания среди детей.Цель работы -оценка клинико-лабораторных показателей новой коронавирусной инфекции (COVID-19) у детей.Материал и методы. Приведены результаты клинико-лабораторного обследования и комплексной терапии 95 детей со среднетяжелой и тяжелой формой течения COVID-19, госпитализированных в ГБУЗ РКИБ г. Уфы с мая по сентябрь 2021 г.Результаты. Изучены анамнез болезни и жизни, эпидемиологические, клинико-лабораторные, инструментальные данные, полученные при обследовании детей с COVID-19 в динамике болезни. Среди клинических проявлений у детей преобладали симптомы острой респираторной инфекции (37,9±4,98%), пневмонии без дыхательной недостаточности (26,3±4,52%), пневмонии с острой дыхательной недостаточностью (33,7±4,85%). У 2 (2,1±1,47%) пациентов первых месяцев жизни на фоне отягощенного преморбидного фона, иммунодефицита COVID-19 протекал в крайне тяжелой форме с летальным исходом.Заключение. Клинические проявления COVID-19 схожи у детей и взрослых, но различаются по частоте симптомов поражения дыхательных путей. Тяжелое течение с летальным исходом зарегистрировано у детей первых месяцев жизни с отягощенным преморбидным фоном.Финансирование. Исследование не имело финансовой поддержки.
