Делегатская, 20/1 У беременных женщин с кардиоваскулярной патологией (артериальная гипертония, аритмии, синдром Марфана, гипертрофическая кардиомиопатия) применяют высокоселективные β-адреноблокаторы (β-АБ). β-АБ по классификации безопасности пищевых продуктов и лекарственных препаратов (Food and Drug Administration) относятся к категории С, и их назначение в различных клинических ситуациях отвечает принципу «риск-польза». Кроме того, следует мониторировать состояние плода и новорожденного, так как препараты этой группы способны вызвать брадикардию, гипогликемию, апноэ и метаболические расстройства, но риск возникновения этих состояний крайне низок, а клиническая эффективность β-АБ-высокая.

Стрюк Московский государственный медико-стоматологический университет Артериальная гипертония (АГ) беременных встречается по данным ВОЗ у 15-20% женщин. В разных регионах России частота АГ у беременных колеблется от 7 до 29%. У женщин с АГ, имевшейся до беременности, потери плода составляют 3,8%, преждевременные роды-15,3%, а внутриутробная задержка развития плода-16,6%. Представлены данные о физиологических изменениях гемодинамики у беременных женщин. Выделены беременные с АГ высокого и низкого риска. Подробно описаны немедикаментозные методы ведения беременных с АГ. У пациенток низкого риска, к которым относят беременных с уровнем артериального давления (АД) 140-160/90-110 мм рт.ст. антигипертензивная терапия может не проводиться. При более высоком АД антигипертензивная терапия назначается незамедлительно. Антагонисты кальция (АК) из группы дигидропиридинов являются препаратами выбора для лечения АГ у беременных. Более «совершенными» препаратами этой группы являются АК третьего поколения. Их отличает более предсказуемая эффективность. Наряду с этим необходимо подчеркнуть, что для купирования гипертонического криза у беременных возможно использование нифедипина короткого действия. Striuk Moscow state medico-stomatological University According to WHO data, arterial hypertension (HT) is revealed in 15-20% of pregnant women. In different regions of Russia the incidence of pregnancy HT varies from 7 to 29%. In women with HT started before pregnancy, perinatal losses are observed in 3, 8%, premature births-in 15, 3% and intrauterine growth retardation-in 16, 6%. Physiological gestational changes of hemodynamics are described. Hypertensive pregnant patients of high and low risk are defined. Non-pharmacological treatment of pregnant women with HT is presented in details. Antihypertensive therapy may not be used in hypertensive patients of low risk with blood pressure (BP) of 140-160\90-110 mm Hg. If BP is higher antihypertensive therapy should be used immediately. Dihydropyridine calcium antagonists (CA) is drugs of choice for HT treatment during pregnancy, especially "advanced" CA of the third generation. They have predictable efficacy. It is possible to use short-acting nifedipine for treatment of acute HT in pregnant patients.
