Мета дослідження – дослідити клінічну ефектив- ність застосування пацієнтспецифічних імплан- татів (ПСІ), створених на основі автоматизова- ного алгоритму віртуального відновлення орбіти, при заміщенні дефектів стінок орбіти. Матеріали та методи. Проаналізовано результати лікування 58 пацієнтів з посттравматичними дефектами сті- нок орбіти, яким проводили реконструкцію орбіти з використанням ПСІ. В залежності від алгоритму виготовлення ПСІ усі пацієнти були розділені на дві групи – основну і контрольну. В основній групі, що включала 31 пацієнта, створення дизайну ПСІ ґрунту- валось на використанні автоматизованого алгоритму сегментації та віртуального відновлення цілісності орбіти, тоді як в контрольній – віртуальне заміщення дефектів проводили в напівавтоматичному режимі (в кожному зрізі КТ). Результати. Середня різниця об’єму між інтактною та неушкодженою орбітою до оперативного втручання становила 3,4 ± 2,5 в осно- вній групі та 2,8±1,1 в контрольній (р>0,05), після операції – 0,68±0,28 cм3 та 0,71±0,21cм3 (р>0,05). Безпосередньо перед хірургічним етапом лікування частка посттравматичного енофтальму в основній групі становила 70,1%, а в контрольній – 74,1%, тоді як у віддаленому післяопераційному періоді жодного випадку залишкового енофтальму виявлено не було. Різниця форми орбіти статистично достовірно не відрізнялася в обох групах і становила 3,3±3,5% та 3,25±2,5% відповідно (р>0,05). Середня тривалість етапу комп’ютерного моделювання в основній групі була 36,7±6,9 хвилин проти 72,9±7,7 хвилин в контр- олі (p<0,001). В основній групі оперативне втру- чання в середньому тривало 57,5±14,7 хвилин, тоді як у контрольній групі – 58.3±11,3 хвилин (р>0,05). Висновки. Відповідно до отриманих результатів ПСІ, створені на основі автоматизованого алгоритму комп’ютерного моделювання, мають співставну клі- нічне ефективність з традиційним цифровим про- токолом реконструкції орбіти і відповідно можуть бути рекомендовані як метод вибору при заміщенні дефектів орбіти. Дослідження клінічної широти їх практичного застосування становить собою задачу подальших наукових досліджень цього питання.
