Таким чином, необхідним є пошук новітніх ефективних стратегій зменшення операційного ризику хірургічного лікування пацієнтів з морбідним ожирінням, в тому числі суперожирінням. в світі накопичений досвід використання різних лікарських засобів фармакотерапії ожиріння. Так, управління з контролю якості продуктів харчування та лікарських засобів СШа (FDA, 2016) схвалило 5 препаратів для лікування ожиріння, а саме орлістат, лоркасерин, фентермін/топірамат, налтрексон/бупропіон, ліраглутид [10]. відомий ефект використання ендоскопічних методик, насамперед, встановлення внутрішньошлункового балону [9] та баріатричної хірургії, насамперед, шлункового шунтування, рукавної резекції та бандажування шлунку [7]. З нефармакологічних підходів в комплексній консервативній терапії таких хворих розглядають модифікацію способу життя -поєднання дієти, фізичного навантаження та поведінкової терапії. в цілому, за допомогою застосування дієти можна досягти втрати 5-7% маси тіла в більшості клінічних випадків. успішним результатом вважають втрату більш ніж 5% від початкової маси тіла [20]. при цьому використовують збалансовану низькокалорійну дієту, дієту з контролем порцій, дієту з низьким вмістом жиру, низьковуглеводну дієту, середземноморську дієту. при дотриманні будь-якої дієти, яка знижує споживання калорій нижче витрат, відбудеться втрата маси тіла, що ілюструється результатами мета-аналізу 48 рандомізованих досліджень із залученням 7286 осіб, коли автори порівнювали різні дієтичні програми [12].Оптимальною є стратегія, яка передбачає поєднання гіпокалорійної дієти з поступовим збільшенням фізичної активності. Цікаві дослідження, які оцінювали ефект виключно фізичних вправ задля зниження ваги [18]. Так, у мета-аналізах показано, що лікування ожиріння лише за допомогою фізичних вправ призводить до незначного зниження маси тіла у порівнянні із відсутністю будь-якого лікування (в середньому різниця досягла лише 1,6 кг) [8, 17]. на жаль, дієта, додана до фізичних вправ має також невеликий додатковий вплив на загальну втрату маси тіла.
