The role of various inflammatory mechanisms and oxidative stress in the development of atherosclerosis and arterial hypertension (AH) has been increasingly acknowledged during recent years. Hypertension per se or factors that cause hypertension along with other complications lead to infiltration of activated leukocytes in the vascular wall, where these cells contribute to the development of vascular injury by releasing cytokines, oxygen radicals, and other toxic mediators. However, molecular mechanisms underlying leukocyte activation at transcriptional level in AH are still far from being clear. To solve this problem we employed cDNA microarray technology to reveal the differences in gene expression in peripheral blood leukocytes from patients with AH compared with healthy individuals. The microarray data were verified by a semi-quantitative RT-PCR method. We found 25 genes with differential expression in leukocytes from AH patients among which 21 genes were upregulated and 4 genes were downregulated. These genes are implicated in apoptosis (CASP2, CASP4, and CASP8, p53, UBID4, NAT1, and Fte-1), inflammatory response (CAGC, CXCR4, and CX3CR1), control of MAP kinase function (PYST1, PAC1, RAF1, and RAFB1), vesicular trafficking of molecules among cellular organelles (GDI-1 and GDI-2), cell redox homeostasis (GLRX), cellular stress (HSPA8 and HSP40), and other processes. Gene expression pattern of the majority of genes was similar in AH patients independent of the disease stage and used hypotensive therapy, but was clearly different from that of normotensive subjects.

At the same time, measures aimed at preserving the health of the population lead to a considerable reduction in losses associated with an abnormal rise in air temperature.
Although collateral circulation is the essential means of perfusion for ischemized myocardium, its efficiency varies substantially within and between the species. There is both experimental and clinical evidence for association of glycemic disorders with inadequate col- lateral circulation. Current article reviews general mechanisms of collateral circulation failure due to such metabolic disturbances with regard for stages of arteriogenesis. We also highlight horizons of further studies in this field.
Цель обзора: освещение основных положений обновленных рекомендаций Европейского кардиологического общества по реваскуляризации миокарда, касающихся чрескожных коронарных вмешательств (ЧКВ) при стабильной ишемической болезни сердца (ИБС). Основные положения. Показана тенденция к расширению возможностей ЧКВ в плане улучшения прогноза у пациентов с ИБС, однако в случае реваскуляризации миокарда при поражении ствола левой коронарной артерии и многососудистом стенозировании у пациентов с сахарным диабетом и сложной коронарной анатомией методом выбора остается операция аортокоронарного шунтирования. Исключено применение биодеградируемых каркасов, в любых клинических ситуациях рекомендуется имплантация стентов с лекарственным покрытием. Заключение. Совершенствование технологии ЧКВ и появление новых фармакологических средств способствуют улучшению результатов эндоваскулярного лечения и прогноза у пациентов с ИБС. Цель. Оценить частоту сердечно-сосудистых событий и смертность в зависи-мости от исходного состояния коронарного коллатерального кровотока (ККК) в рамках 5-летнего наблюдения за пациентами со стабильной ишемической болезнью сердца (ИБС). Материал и методы. В исследование включено 579 пациентов со стабильной ИБС, у которых при плановой коронароангиографии (КАГ) был выявлен стеноз хотя бы в одной коронарной артерии (диаметром не менее 1,5 мм), сужающей ее просвет на 50% и более. ККК оценивали по модифицированной методике Rentrop. Через 5 лет после индексной КАГ оценивалась частота сердечно-сосудистых событий (возобновление или усугубление функционального класса (ФК) стенокардии, нефатальный инфаркт миокарда, нестабильная стенокардия, чрескожное коронарное вмешательство, коронарное шунтиро-вание, острое нарушение мозгового кровообращения) и смертность. Результаты. При многофакторном анализе (методом регрессии Кокса) выяв-лена ассоциация хорошо развитого ККК с меньшей смертностью от кардиаль-ных (ОР 0,5; р=0,02) и от всех причин (ОР 0,5; р=0,004). При многофакторном анализе (логистическая регрессия) выявлена ассоциация развитого ККК с меньшей частотой возобновления/усугубления ФК стенокардии в отдален-ном периоде - (ОШ 0,36, p=0,002). Взаимосвязь частоты нефатальных инфарк-тов миокарда, чрескожное коронарное вмешательство в отдаленном периоде со степенью развития ККК не выявлена. Заключение. По данным 5-летнего наблюдения, хорошо выраженный ККК связан с меньшей смертностью от кардиальных и от всех причин и частотой возобновления/усугубления ФК стенокардии напряжения у больных хрониче-ской ИБС, в том числе перенесших реваскуляризацию, независимо от тяжести заболевания, поражения коронарного русла и других факторов, ассоцииро-ванных с сердечно-сосудистыми осложнениями. Aim.To evaluate the prevalence of cardiovascular events and death depending on baseline coronary collateral blood flow (CBF) in five year follow-up of stable coronary heart disease (CHD) patients. Material and methods. To the study, 579 stable CHD patients included, who during scheduled coronary angiography (CAG) were diagnosed with at least one stenosis in one coronary artery (diameter no less than 1,5 mm), narrowing its lumen by ≥50%. CBF was assessed by Rentrop modified method. In 5 years post index CAG, the cardiovascular events were counted (relapse or worsening of functional class of angina, non-fatal myocardial infarction, coronary bypass surgery, acute stroke) and mortality. Results. In multifactorial analysis (Cox regression) the association revealed of the good CBF with lower cardiovascular mortality (HR 0,5; р=0,02) and all-cause mortality (HR 0,5; р=0,004). Fibromuscular dysplasia (FMD) is a rare disease that affects small and medium-sized arteries. Clinical manifestations of FMD depend on its localization. In many cases, FMD of renal arteries (RA) is associated with arterial hypertension. Young age, particularly of female patients, suspected RA dissection or kidney infarction, absence of atherosclerosis or presence of FMD in other arteries of such patients evidence for RA FMD. In invasive treatment of hemodynamically significant stenoses, transluminal balloon angioplasty (TBA) of renal arteries is preferrable. Taking into account initial alterations of the vascular wall and unevenness of the lumen of the affected blood vessel, stent implantation is associated with an increased risk of complications and is recommended only if ballooning complications develop. An open reconstructive surgery is indicated in complicated narrowing anatomy, a high risk of the endovascular treatment, or after failure of the endovascular intervention. This article presents a clinical case of a young female patient with RA FMD and renovascular arterial hypertension who successfully underwent renal TBA with a drug-coated balloon.
The paper describes a clinical case of a female patient with severe obstructive sleep apnea syndrome in the presence of congenital hemangioma of the face, soft palate, and tongue concurrent with paroxysmal atrial fibrillation and atrial flutter, paroxysmal supraventricular tachycardia, and sinoatrial block (maximally up to 3.9 sec). Continuous positive airway pressure therapy could reduce the number of paroxysms of atrial fibrillation and atrial flutter, supraventricular tachycardia and eliminate sinoatrial block.
