Favorable versus detrimental cardiovascular responses to intermittent hypoxia conditioning (IHC) are heavily dependent on experimental or pathological conditions, including the duration, frequency and intensity of the hypoxia exposures. Recently, we demonstrated that a program of moderate, normobaric IHC (FIO2 9.5-10% for 5-10 min/cycle, with intervening 4 min normoxia, 5-8 cycles/day for 20 days) in dogs afforded robust cardioprotection against infarction and arrhythmias induced by coronary artery occlusion-reperfusion, but this protection has not been verified in other species. Accordingly, in this investigation cardio- as well as vasoprotection were examined in male Wistar rats completing the normobaric IHC program or a sham program in which the rats continuously breathed atmospheric air. Myocardial ischemia and reperfusion (IR) was imposed by occlusion and reperfusion of the left anterior descending coronary artery in in situ experiments and by subjecting isolated, perfused hearts to global ischemia-reperfusion. Cardiac arrhythmias and myocardial infarct size were quantified in in situ experiments. Endothelial function was evaluated from the relaxation to acetylcholine of norepinephrine-precontracted aortic rings taken from in situ IR experiments, and from the increase in coronary flow produced by acetylcholine in isolated hearts. IHC sharply reduced cardiac arrhythmias during ischemia and decreased infarct size by 43% following IR. Endothelial dysfunction in aorta was marked after IR in sham rats, but not significant in IHC rats. Similar findings were found for the coronary circulations of isolated hearts. These findings support the hypothesis that moderate, normobaric IHC is cardio- and vasoprotective in a rat model of IR.
A course of transauricular electrostimulation (TES) consisting of 10 sessions was administered to rats before the induction of myocardial infarction by Selye's method and to rats that were left intact. In the latter animals, the electrostimulation did not influence cardiac contractile function at rest (as judged by heart rate, developed pressure, and Katz's index), but exerted beneficial chronotropic and inotropic effects during the maximum isometric tension produced by compression of the ascending aorta. In the TES-pretreated rats with a 2-day-old myocardial infarct, cardiac contractile function was depressed significantly less, both at rest and during isometric tension, than in infarcted rats not exposed to TES.

This review presented recent data on initiation, regulation, and execution of neutrophil apoptosis with participation of «death receptors», mitochondria, Bcl-2 family proteins, PI3-K (phosphatidylinositol 3-kinase), p38 MAPK (mitogen-activated protein kinase), ERK (extracellular signal regulated kinase) and JNK (c-Jun N-terminal kinase) cascades, protein kinases A, B and C, сAMP, heat shock proteins, NF-kB (nuclear factor-kB), calpains, caspases and theirs inhibitors, reactive oxygen species, and other factors. A speculative model of the apoptotic processes involvement in the regulation of neutrophil differentiation and reactivity was proposed.

Мы предположили, что подавлять кристалл-индуцированное воспаление мог бы М3 фенотип макрофагов, который увеличивает продукцию антивоспалительных цитокинов при действии воспалительных индукторов (АВ-М3 фенотип). Цель работы состояла в проверке этой гипотезы. Методы. Подагрическое воспаление моделировали с помощью добавления кристаллов MSU и СаРР к культуре макрофагов. Для программирования АВ-М3 фенотипа использовали ингибитор инфламмасомы MCC950 и IL-4. Воспалительную реакцию оценивали по продукции IL-1b и TGFb1. Результаты. Реакция нативных (M0) макрофагов и на кристаллы MSU, и на кристаллы CaPP воспроизвела острое подагрическое воспаление в форме 3-кратного увеличения продукции IL-1b (p < 0,05). При добавлении кристаллов MSU к АВ-М3 макрофагам содержание IL-1b в культуральной среде было меньше в 25 раз (p < 0,05), а содержание TGFb больше почти в 2 раза (p < 0,05) по сравнению со средой М0 макрофагов с кристаллами MSU. Аналогичный результат был получен при добавлении кристаллов CaPP к АВ-М3 макрофагам. Заключение. Таким образом, АВ-М3 фенотип отвечает на провоспалительное действие кристаллов MSU и CaPP снижением продукции IL-1b и усилением продукции TGFb1. Результаты работы подтвердили гипотезу о том, что АВ-М3 фенотип может ограничить кристалл-индуцированное воспаление. Соответственно, разработка клинической версии технологии АВ-М3 макрофагов представляется весьма перспективной. Gout is a common cause of inflammatory arthritis, joint damage, kidney stones, and kidney failure. Joint inflammation is induced by interaction of uric acid (monosodium urate, MSU) crystals and calcium pyrophosphate (CaPP) crystals with macrophages. We hypothesized that the crystal-induced inflammation could be suppressed by the M3 switch phenotype, which increases production of anti-inflammatory cytokines under the action of inflammation inducers (AB-M3 phenotype). The aim of the present study was to test this hypothesis. Methods. Gouty inflammation was modeled by adding MSU and CaPP crystals to cultured macrophages. The inflammasome inhibitor, MCC950, and IL-4 were used for programming the AB-M3 phenotype. The inflammatory response was evaluated by production of IL-1β and TGFβ1. Results. The response of native (M0) macrophages to either MSU crystals or CaPP crystals reproduced acute gouty inflammation in the form of a 3-fold increase in IL-1 β production (p < 0.05). When MSU crystals were added to AB-M3 macrophages, the IL-1β content in the culture medium became 25 times lower (p < 0.05), and the TGF-β1 content became almost twice higher (p < 0.05) than the respective values in the M0 macrophage medium with MSU crystals. A similar result was obtained when CaPP crystals were added to AB-M3 macrophages. Conclusion. The AB-M3 phenotype responds to the pro-inflammatory action of MSU and CaPP crystals by decreasing the IL-1β production and increasing the TGF-β1 production. These results confirmed the hypothesis that the AB-M3 phenotype restricts the crystal-induced production of inflammatory IL-1β and increases the production of anti-inflammatory TGF-β1. Therefore, development of a clinical version of the AB-M3 macrophage technology is very promising.

В контексте этой концепции высокая выживаемость опухолевой клетки может быть обусловлена адаптацией к лекарствам и агрессивным факторам иммунитета, таким как свободные радикалы и воспалительные цитокины. В онкологической клинике врач с помощью иммунотерапии пытается усилить иммунитет больного. Эта тактика оправдана, если удается увеличить силу иммунной атаки до уровня, который убивает опухоль. Но если стимулирование иммунитета недостаточно сильное, чтобы убить опухолевую клетку, это приводит к дополнительной адаптации опухоли и повышению ее устойчивости. Отсюда следует важная для клиники гипотеза: снизить выживаемость опухолевых клеток можно с помощью нарушения ее механизмов адаптации. Концепция адаптации предлагает два способа решения: 1) блокировать активацию генов и синтез белков, необходимых для формирования адаптивного системного структурного следа (ССС); и 2) прекратить действие адаптирующих факторов, и благодаря этому вызвать исчезновение ССС, дезадаптацию и утрату опухолью приобретенной устойчивости. Применительно к иммунотерапии рака второй способ порождает, на первый взгляд абсурдную идею, «отключить» иммунную систему на период до стадии дезадаптации опухоли, и только затем применить иммунотерапию к утратившим устойчивость опухолевых клеток. Эта гипотеза нуждается в проверке, но уже сейчас с ней согласуются косвенные данные. Снижение адаптивной устойчивости опухолевых клеток могло бы существенно увеличить антиопухолевый потенциал иммунотерапии рака. Immunotherapy provides significant progress in treatment of cancer although not for all and not always. In this review, we briefly analyzed major immunotherapies for cancer and outlined a new approach to improve their effectiveness. We examined the issue of cancer immunotherapy through the prism of the concept of adaptation to environmental factors. In the context of this concept, the high survival rate of tumor cells may be due to cell adaptation to drugs and aggressive immunity factors, such as free radicals and inflammatory cytokines. In the oncological clinic, physicians try to enhance the patient’s immunity with immunotherapy. This tactic is warranted if the physician would manage to increase the potency of immune attack to a level that kills the tumor. However, if the enhanced power of immunity attack is insufficient to kill a tumor cell, the tumor may additionally adjust and stabilize. Therefore, a clinically important hypothesis ensues: the tumor cell survival may be impaired by disrupting its adaptive mechanisms. The concept of adaptation offers two options: 1) to block activated synthesis of the genes and proteins that are required for formation of an adaptive systemic structural trace (SST) and 2) to stop the action of adapting factors, and, thereby, to erase the SST and induce maladaptation with loss of the acquired resistance of the tumor. With regard to cancer immunotherapy, the second option creates, at the first glance, an absurd idea, to “turn off” the immune system for a period preceding the stage of tumor maladaptation, and only then to apply immunotherapy to the tumor cells that have become less resistant. This hypothesis needs to be verified but some indirect data are already consistent with it. Reducing the adaptive resistance of tumor cells could significantly increase the antitumor potential of cancer immunotherapy.
В развитии рака предстательной железы (РПЖ) макрофаги играют важную роль. Многие опухоли выделяют противовоспалительные цитокины, которые перепрограммируют М1 фенотип макрофагов на проопухолевый М2 фенотип. М2 макрофаги подавляют противоопухолевый иммунитет, способствуют делению и метастазированию опухолевых клеток. Предыдущие исследования позволили нам обосновать предположение, что деление опухолевых клеток РПЖ человека может быть ограничено особым М3 фенотипом макрофагов. Фенотип М3, в отличие от М1 фенотипа, реагирует на противовоспалительные цитокины увеличением продукции провоспалительных противоопухолевых цитокинов, что способствует сохранению их противоопухолевых свойств в зоне опухоли. Цель исследования - проверка гипотезы о способности М3 макрофагов останавливать деление клеток предстательной железы больного РПЖ. Методика. В работе использовали макрофаги мышей, выделенные из перитонеального лаважа и макрофаги человека, полученные из моноцитов крови больных РПЖ. Фенотип М3 макрофагов получали добавлением в среду культивирования IFN-γ, ингибиторов STAT3, STAT6 и SMAD3 с последующей стимуляцией липополисахаридом. Результаты. Показано, что М3 макрофаги мышей и человека ограничивали деление клеток предстательной железы больных РПЖ в условиях 2D (на плоскости) культивирования на 43% и 93%, соответственно. При 3D (в объеме) культивировании М3 макрофаги мышей не ограничивали, а М3 макрофаги человека лишь незначительно ограничивали деление клеток предстательной железы у больных РПЖ. Заключение. Результаты работы делают обоснованными дальнейшие исследования и разработку клинической версии биотехнологии лечения рака предстательной железы с использованием М3 макрофагов.Macrophages play an important role in the development of prostate cancer (PCa). Many tumors, including PCa, secrete anti-inflammatory cytokines that reprogram the M1 macrophage phenotype into the pro-tumor M2 phenotype. M2 macrophages suppress antitumor immunity and promote division and metastasis of tumor cells. We hypothesized that the division of human PCa cells may be restricted by a specific M3 macrophage phenotype. The M3 phenotype, in contrast to the M1 phenotype, responds to anti-inflammatory cytokines by increasing the production of inflammatory anti-tumor cytokines and retains its anti-tumor properties in the tumor area. The aim of the study was to test the hypothesis on the ability of M3 macrophages to stop division of prostate cells from patients with PCa. Methods. This study used murine macrophages isolated from the peritoneal lavage and human macrophages obtained from blood monocytes of patients with PCa. The M3 macrophage phenotype was obtained by adding IFN-γ, STAT3, STAT6, and SMAD3 inhibitors to the cultural medium followed by lipopolysaccharide (LPS) stimulation. Results. Murine and human M3 macrophages restricted the division of patients’ PCa cells in the conditions of 2D cultivation by 43% and 93%, respectively. In 3D cultivation, murine M3 macrophages did not restrict whereas human M3 macrophages only slightly limited the division of prostate cells from PCa patients. The results of the study warrant further research and development of a clinical biotechnology for PCa treatment with reprogrammed M3 macrophages.
В обзоре проанализированы механизмы подагрического воспаления и разрешения острого воспаления при подагре, и то, как их понимание способствует созданию новых технологий лечения подагры. В первой части обзора анализируется кристалл-индуцированное воспаление при подагре, которое связано с активацией инфламмасомы NLRP3 в макрофагах, состоящее из двух этапов: 1) синтез компонентов инфламмасомы; и 2) сборка инфламмасомы. Проанализированы сигналы, активирующие синтез компонентов инфламмасомы, которые генерируются при взаимодействии кристаллов урата мононатрия с макрофагами. Описаны этапы инициации воспаления при подагре: синтез компонентов инфламмасомы, про-каспаз и про-IL-1β. IL-1β - основной эффекторный цитокин, который запускает не только воспалительный ответ, но и вызывает вазодилатацию сосудов, привлечение нейтрофилов к месту отложения кристаллов и еще большее усиление острого воспалительного эпизода через TLR-зависимую активацию NF-κB (фактор транскрипции). Рассматривается роль растворимой мочевой кислоты в активации инфламмасомы NLRP3. Подчёркнуто, что в разрешении острого воспаления при подагре основную роль играют те же клетки, что и в развитии классической воспалительной реакции: макрофаги и нейтрофилы. В обзоре рассмотрены сигнальные пути, которые переключают воспалительный ответ на разрешение и завершение воспаления: AMPK- (AMP-активируемая киназа) зависимый путь; антивоспалительные цитокины TGFβ1, IL-10 и IL-1Ra; белок аннексин А1 (AnxA1) и α1-антитрипсин (ААТ). Описаны способы терапевтического блокирования воспаления при подагре с помощью ингибиторов инфламмасомы и ингибиторов IL-1β. Изучение механизмов подагрического воспаления и его разрешения привело к идее о разработке методов репрограммирования иммунных клеток и адаптивной иммунотерапии подагры, методов, которые при других заболеваниях, таких как рак, уже показали выдающиеся возможности лечения больных.The review analyzes mechanisms of gouty inflammation and resolution of acute inflammation in gout and shows how their understanding contributes to creation of new technologies for the treatment of gout. The first part of the review addresses crystal-induced inflammation in gout, which is associated with activation of the NLRP3 inflammasome in macrophages. The crystal-induced inflammation consists of two stages, 1) synthesis of inflammasome components and 2) inflammasome assembly. The review analyzes signals activating the synthesis of inflammasome components, which are generated by the interaction of MSU (monosodium urate crystals) and macrophages. Stages of inflammation initiation are described, including the synthesis of inflammasome components, pro-caspases and pro-IL-1β. IL-1β is the main effector cytokine of gouty inflammation, which not only triggers the inflammatory response but also induces vasodilation, attraction of neutrophils to the place of crystal deposition, and even a greater potentiation of the acute inflammatory episode via the TLR-dependent activation of NF-κB (transcription factor). The role of soluble uric acid in the activation of NLRP3 inflammasomes is addressed. The main role in resolving acute gouty inflammation belongs to the same cells, macrophages and neutrophils, that contribute to the development of inflammation. The review focuses on signaling pathways switching the inflammatory response to inflammation resolution and termination, including the AMPK (AMP-activated kinase) signaling pathway, the TGFβ1, IL-10 and IL-1Ra anti-inflammatory cytokines, the protein annexin A1 (AnxA1), and α1-antitrypsin (AAT). The authors described in detail the techniques for therapeutic suppression of inflammation in gout using inflammasome and IL-1β inhibitors. Studying the mechanisms of gouty inflammation and its resolution has resulted in development of methods for reprogramming immune cells and adaptive immunotherapy of gout. These methods have already shown outstanding therapeutic potential for patients with other diseases such as cancer.
