Принципы, изложенные в его диссертации, предвосхитили появление работ, изучающих юкстамедуллярный кровоток и почечную гемомикроциркуляцию в норме и при патологии При острых сердечно-сосудистых заболеваниях возрастает значение юкстамедуллярного пути кровотока, наблюдается юкстамедуллярное шунтирование. При хронических сердечно-сосудистых заболеваниях вначале усиливается юкстамедуллярное шунтирование, а на поздних стадиях заболевания склероз мозгового вещества почек вызывает редукцию и блок юкстамедуллярного пути кровотока, ухудшается как юкстамедуллярный, так и кортикальный путь кровотока, что приводит к срыву адаптации интраорганного артериального русла почек. Современные сведения о путях почечной гемоциркуляции, подготовленные исследованиями В.З. Голубева, позволяют объяснить механизм развития различных патологических состояний: острой кровопотери, гидронефроза, вазоренальной гипертензии, гломерулонефрита, пиелонефрита, нефроурологических заболеваниях, кардиогенного шока, мочекаменной болезни, кардиоренального синдрома при острой ишемической болезни сердца, артериальной гипертензии, внезапной коронарной смерти и др. ВО «Ростовский государственный медицинский университет» Минздрава России, г. Ростов-на-Дону, Россия Цель исследования -изучить особенности и асимметрию кровеносного русла матки. Материал и методы. Исследование выполнено на 140 препаратах матки женщин от 16 до 80 лет, не имевших на вскрытии патологии внутренних половых органов. Использованы рентгеноанатомический и морфометрические методы исследования.Результаты. Установлено что, как у нерожавших, так и у рожавших женщин диаметр маточных артерий в 43% случаев больше справа, в 30% -слева, и почти одинаков в 27 % случаев. Артериоматочный индекс в каждой возрастной группе значительно больше у рожавших женщин, чем у нерожавших; в 46.4% случаев он больше в правой половине матки, в 30.8% -в левой половине и в 22.8% случаев почти одина-ков с обеих сторон. Отток из венозной сети правых отделов матки имеет преимущества, так как осуществ-ляется в подвздошную вену, а из левых -в левую почечную вену; диаметр маточной вены преобладает с правой стороны.Выводы. Морфологическая асимметрия кровеносного русла матки проявляется различиями диа-метра правой и левой маточных артерии и вен, артериоматочного индекса правой и левой половин органа. Можно предположить, что морфологическая асимметрия сосудистого русла матки связана с развитием органа из парных Мюллеровых протоков. Asymmetry of the Uterine VesselsThe aim of the study was to study the features and asymmetry of the uterus blood channel. Material and methods.The study was performed on 140 uterine preparations of women aged 16 to 80 years who did not have an internal genital pathology at the autopsy. X-ray anatomical and morphometric methods of investigation were used.Results. It was established that, in both nulliparous and congenital women, the diameter of the uterine arteries was 43% larger in the right, 30% in the left, and almost the same in 27% of cases. The relative content of arteries of the uterus in each age group is much larger in women giving birth than in nulliparous women; in 46.4% of cases it is more in the right half of the uterus, in 30.8% in the left half and in 22.8% of cases it is almost identical on both sides. Outflow from the venous network of the right divisions of the uterus has advantages, since it is carried into the iliac vein, and from the left -to the left renal vein; the diameter of the uterine vein predominates on the right side.Conclusion. The morphological asymmetry of the uterine blood channel is manifested by differences in the diameter of the right and left uterine arteries and veins, as well as the relative content of arteries of the uterus of the right and left halves of the uterus. It can be assumed that the morphological asymmetry of the vascular bed of the uterus is associated with the development of an organ from paired Mueller ducts.
Combined anomalies of the urinary system and axial skeleton, including an L-shaped kidney, obstruction of the ureteropelvic junction (CAKUT syndrome, congenital anomalies of the kidney and urinary tract syndrome) and spinal anomalies, are extremely rare. The clinical case gives a description of a similar combined developmental anomaly.
Objective: to study the structural transformations of the architectonics of intra-organ renal arterial vessels in the age aspect.Materials and methods: 150 kidneys of people of diff erent age who died from the reasons which are not connected with diseases of cardiovascular and urinary systems are investigated. The studies were carried out using a set of methods: angiographic, macromicroscopic and morphometry.Results: with increasing age, a decrease in the number of vascular glomeruli in the kidney, the proportion of glomerular mass in the cortical substance of the kidney was found. In old age and in centenarians, a rare capillary network in the cortical substance of the kidney, tortuosity, narrowing and expansion of direct arterioles and capillaries in the cerebral substance was revealed. In old age and in centenarians, compared with adolescence, the relative content of arterial vessels in the cortical substance decreases by 6 times, in the juxtamedullary zone — by 4 and in the cortical substance — by 2 times.Conclusions: the large diameters of the juxtamedullary glomeruli and a large index of the relative content of arterial vessels in the juxtamedullary zone create prerequisites for possible juxtamedullary shunting with urgent adaptation in the norm. The decrease in these indicators in old age, the elderly and centenarians, obviously, explains the age-related decline in the adaptive capacity of the arterial bed of the kidneys. With increasing age, the range of adaptive capabilities of the renal vascular bed of aging people decreases compared to those of mature age.
