В статье представлена информация о наиболее часто встречающихся болевых синдромах, ассоциированных с COVID-19. Механизмы возникновения боли связаны с особенностями взаимодействия вируса с рецептором ангиотензинпревращающего фермента 2, вовлеченного в процессы ноцицепции, высвобождением провоспалительных медиаторов, повышающих чувствительность ноцицептивных рецепторов к медиаторам боли, а также активацией тригеминоваскулярной системы и менингеальных ноцицепторов. Наиболее рациональным является всесторонний подход к ведению пациентов с COVID-19-ассоциированными болевыми синдромами с использованием как фармакологических, так и немедикаментозных методов лечения. The article provides information on the most common pain syndromes associated with COVID-19. The mechanisms of pain occurrence are associated with the specific effects of the virus with the angiotensin-converting enzyme 2 receptor involved in the processes of nociception, the release of pro-inflammatory mediators that increase the sensitivity of nociceptive receptors to pain mediators, as well as the activation of the trigeminovascular system and meningeal nociceptors. The most rational is a comprehensive approach to the management of patients with COVID-19-associated pain syndromes using both pharmacological and non-pharmacological methods of treatment.
Немоторные симптомы при болезни Паркинсона (БП) имеют недофаминергические и дофаминергические механизмы и широко варьируют по своей природе, диапазону и клиническим проявлениям, приводя к значительному ухудшению качества жизни и социальной дезадаптации. В связи с этим немоторные проявления БП всегда должны оцениваться параллельно с моторными симптомами заболевания. В данной статье рассматриваются клинические особенности когнитивных, эмоционально-аффективных, вегетативных и поведенческих нарушений, а также расстройств сна и бодрствования у пациентов с БП. Также представлены основные подходы к их диагностике и лечению, которое должно проводиться с пониманием патофизиологических механизмов немоторных симптомов с учетом клинической картины заболевания в целом, побочных эффектов принимаемых пациентом противопаркинсонических препаратов и других факторов. Non-motor symptoms in Parkinson’s disease have non-dopaminergic and dopaminergic mechanisms and vary widely in nature, range and clinical manifestations leading to a significant deterioration in life quality and social maladaptation. In this regard, non-motor manifestations of PD should always be evaluated in parallel with the motor symptoms of the disease. This article discusses the clinical features of cognitive, emotional-affective, autonomic and behavioral disorders, as well as sleep and wakefulness disorders in patients with PD. The main approaches to their diagnosis and treatment are also presented, which should be carried out with an understanding of the pathophysiological mechanisms of non-motor symptoms, taking into account the clinical picture of the disease as a whole, side effects of antiparkinsonian drugs taken by the patient, and other factors.
