Мета дослідження – проведення аналізу психоемоційного стану жінок із доброякісними пухлинами яєчників на тлі акушерських та перинатальних ускладнень. Матеріали та методи. Ми комплексно обстежили 67 вагітних із доброякісними пухлинами та пухлиноподібними утвореннями яєчників, які було виявлено до вагітності або у І триместрі гестації (основна група), та 50 вагітних без новоутворень яєчників та іншої тяжкої гінекологічної чи соматичної патології, які склали контрольну групу. Визначали вегето-судинні характеристики, особливості психоемоційного стану, особистісної та реактивної тривожності, наявність ознак депресії та якість життя у вагітних. За допомогою ліцензованої програми SPSS v.21 проведено статистичну обробку даних, вписаних в електронні таблиці Excel-2010. Паралельно зa пошуковими системами Pubmed, Google Scholar, Sciencedirect, Webcardio проведено аналіз літературних джерел із даної тематики за останні 5 років. Результати дослідження та їх обговорення. Депресивні стани часто виникають як реакція на хвороби, постійні стреси, а з іншого боку, самі можуть бути причиною соматичних проявів та різних ускладнень. За нашими даними, згідно зі шкалою Е. Бека, прояви депресії практично не відмічаються у здорових жінок із фізіологічним перебігом вагітності (90,0 %), тоді як серед пацієнток із доброякісними новоутвореннями яєчників ознаки депресії різної тяжкості вже відмічають більше третини жінок (37,3 %, р<0,05), а серед пацієнток із гестаційними ускладненнями таких переважна більшість: 72,1 % проти 24,5 % у підгрупі 2 (р<0,05). Ще більший відсоток жінок основної групи показав високий рівень ситуативної тривожності, яка відповідає рівню стресу і реакції на нього у даний момент: 40,3 % пацієнток основної групи проти 22,0 % жінок контрольної групи (р<0,05). Висновки. Отже, нами встановлено у вагітних із доброякісними новоутвореннями яєчників та гестаційними порушеннями високу частоту виражених проявів синдрому вегето-судинної дисфункції.
Резюме. Більшість гайдлайнів, клінічних протоколів, методичних рекомендації присвячені веденню утворень придатків поза вагітністю, а рекомендації щодо вагітних жінок залишаються мало систематизованими і не чіткими, хоча необхідність враховувати материнські і плодові наслідки ускладнює процес ведення таких новоутворень. Менеджмент пухлин придатків, виявлених під час вагітності, є суперечливим. Гінекологічна патологія у жінок завжди тією чи іншою мірою пов’язана з психологічним стресом, змінами психоемоційного стану, які можуть при тривалому стресогенному навантаженні призводити до психосоматичної патології. Крім того, з психо-соціального аспекту вагітність може розглядатися як специфічний надзвичайно емоційний стан, який може бути потужним стресором. Перинатальний материнський стрес може призвести до різних ускладнень, які можуть мати далекосяжні наслідки як для соматичного, так і для психічного функціонування новонародженого. У науковій літературі наявна достатньо велика кількість публікацій з проблеми «пухлини яєчника і вагітність», однак переважна більшість з них присвячена гінекологічним аспектам, ризику малігнізації, диференціальній діагностиці та менеджменту таких новоутворень лише в напрямку вибору між очікувальною та хірургічною тактикою та визначення термінів і обсягів операції. Незважаючи на емпіричні докази численних наслідків, пов'язаних з високим рівнем стресу та проблемами психічного здоров'я під час вагітності, оцінка психо-соціального стресу у вагітних проводиться рідко.
