Prevention of femoral head dislocation is becoming one of the most important tasks in view of increased number of total hip replacement procedures throughout the world. The purpose of the present work is to acquaint the surgeons with possibilities and the first experience of dual mobility acetabular component application as the most up-to-date solution to prevent dislocation of the femoral head in primary or revision arthroplasties. The paper is based on the literature data and presents a critical analysis of the causes of femoral head dislocation during hip replacement. The authors considered historical aspects and the first outcomes of dual mobility use in the European and Western countries, as well as own positive clinical experience with dual mobility system during a study of 36 patients with an increased risk of femoral head dislocation.
Статья посвящена проблеме кросс-культурной адаптации и валидации русскоязычной версии анкеты OxfordKneeScore. Англоязычная версия анкеты OKS была переведена на русский язык носителем русского языка. Затем эта версия была обратно переведена на английский язык носителем английского языка. Далее, на основе этих переводов была сформирована окончательная версия, с помощью который были опрошены 55 пациентов. Русскоязычная версия анкеты OxfordKneeScore была разработана успешно, показала хорошую валидность и внутреннее соответствие. Высокая частота гонартроза в Российской Федерации и растущий интерес к тотальному эндопротезированию коленного сустава отражают потребность в специализированных методах оценки результатов оценки данной методики на русском языке. OxfordKneeScore (OKS) -это короткий вопросник, который измеряет боль и физическую активность пациента относительно коленного сустава. До сегодняшнего момента она была валидирована и культурно адаптирована на несколько языков, кроме русского. Одним из наиболее часто используемых хирургических доступов при эндопротезировании тазобедренного сустава является прямой боковой по Хардингу. При этом нередко возникают сложности с восстановлением отводящей группы мышц бедра в интра-и послеоперационном периоде. Цель исследования -определить частоту возникновения недостаточности отводящего аппарата бедра после эндопротезирования тазобедренного сустава из чрезъягодичного доступа и эффективность ее лечения. Проспективно было изучено 883 случая эндопротезирования тазобедренного сустава из чрезъягодичного доступа у 883 пациентов. Плановый осмотр пациентов в послеоперационном периоде проводился через 3 месяца. Осмотр включал определение симптома Тренделенбурга, оценку функционального статуса по шкале Харриса, наличие болевого синдрома и его интенсивность по ВАШ. Пациентам, которым диагностировали дисфункцию средней и малой ягодичных мышц, назначалось консервативное лечение, наблюдение в следующие 6 и 12 месяцев после операции, дополнительное обследование. Пяти пациентам, у которых через год после операции определялась слабость отводящего аппарата мышц бедра, выполнялась операция: рефиксация средней и малой ягодичных мышц. Недостаточность функции отводящего аппарата бедра после ЭПТБС из чрезъягодичного доступа встречается часто, но может разрешаться на фоне консервативного лечения в срок до одного года после операции. В случае неэффективности проводимого восстановительного лечения мы рекомендуем выполнение УЗИ ТБС. При наличии сонографических признаков отрыва сухожилий средней и малой ягодичных мышц оперативное лечение в объеме рефиксации может обеспечивать восстановление ходьбы без хромоты и дополнительной опоры. Introduction.Acetabular component malposition at total hip arthroplasty is a common situation that may affect the frequency of dislocation and the rate of implant friction unit wear. The purpose of the study was to determine the influence of different factors on the variability of acetabular component orientation and evaluate the role of orientation in dislocation development.Patients and methods.Total number of patients made up 1408. Out of them 695 patients were operated on at RSRITO named after R.R. Vreden using standard approaches (group 1), 184 - using low invasive approach (group 2) and 55 patients were admitted with implant head dislocation (group 4). At City St. Petersburg hospitals 474 patients (group 3) were operated on. Orientation of acetabular component (inclination and anteversion angles) was evaluated on digital pelvis and plain hip roentgenograms. Dispersion unifactorial analysis was used to evaluate the dependence of the precision of acetabular component positioning upon the surgeon’s experience, patient’s body mass index, type of surgical approach and the use of guide for acetabular component implantation.Results.Within the first postoperative year the rate of femoral component dislocation in group 1 made up 0.9%. Within the Lewinnek safe zone 76.6% of acetabular components were implanted with the use of a guide and 71.8% without. The rate of dislocation in group 2 made up 71.8% and 63.4% of components were implanted within the safe zone. In group 3 the intra-hospitalization dislocation developed in 1.9% of patients and satisfactory positioning relative to Lewinnek safe zone was achieved in 68.2% of cases. The risk factors for acetabular component malposition included high body mass indices, use of low invasive approach and insufficient experience of the operating surgeon. No direct influence of acetabular component orientation upon the femoral component dislocation was detected.Conclusion. Further studies directed to the more detailed analysis of additional factors that either directly or indirectly affect the implant function and to the optimization of surgical technique that would enable the reproducibility of total hip arthroplasty results are required.
Purpose.Instability is a challenging complication of revision hip arthroplasty and a frequent cause of repeatrevisions. Constrained liners and dual mobility systems have gained major attention among the options of dislocation prophylaxis. The aimof this study is to compare the outcomes of revision hip arthroplasy with use of constrained liners and dual mobility systems. Materials and Methods.We used DePuy Duraloc (inner diameter 28 mm) and Zimmer Trilogy (inner diameter32 mm) systems in the constrained liners group (N 78, mean follow-up — 66.2 month, 54-82), Serf Novae and Biomet Avantage systems in the dual mobility group (N 58, mean follow-up — 17.8 month, 10-41). The two groups were comparable in age, sex and different potential dislocation risk factors, however, dual mobility cups were used more frequently in revisions due to unreduced and recurrent dislocations and in patients with the history of instability following total hip arthroplasty. There were also differences in the structure of primary diagnosis. Results.We observed 14 (17.9%) dislocations, 10 (12.8%) of which occurred within 2 years after surgery, and also3 (3.8%) cases of aseptic loosening of the acetabular component, 4 (5.1%) cases of locking mechanism damage without dislocation and 8 (10.3%) cases of deep infection in the constrained liners group. In the dual mobility group we observed 3 (5.17%) large articulation dislocations, 1 (1.7%) case of aseptic loosening of the acetabular component and 4 (6.9%) cases of deep infection. The difference in dislocation rates in two groups was significant (p<0.05). The analysis of the constrained liners group revealed an increased risk of dislocation in cases when a constrained system was implanted into a retained acetabular component compared to cases with acetabular shell revision (p<0.01; RR = 7.2, 95% CI: 2.05-25.26), as well as a trend for increased risk of dislocation in cases when DePuy Duraloc liners (inner diameter 28 mm) were used compared to Zimmer Trilogy (inner diameter 32 mm) (p= 0.07; RR = 4.97, 95% CI: 1.03-24.04). Conclusion. Dual mobility systems proved to be more effective than constrained liners in revision hip arthroplastyalthough being used more frequently as a treatment rather than prophylaxis of instability. Constrained liners bear a higher risk of dislocations when implanted into retained acetabular components and when used with heads of lesser diameter.
Петербург Статья посвящена сравнению мышечной травматизации хирургических доступов при ревизионном эндопротезировании тазобедренного сустава. Был проведен лабораторный анализ биохимических маркеров тканевой травматизации у 56 пациентов, которым выполнялось ревизионное эндопротезирование тазобедренного сустава. Пациенты были разделены на три группы по типу используемого доступа-мини-инвазивный переднелатеральный (n=11), стандартный прямой латеральный (n=33), стандартный задний доступ (n=12). До операции и на 3-и, 5-е, 7-е сутки после нее измерялось содержание таких маркеров, как креатининфосфаткиназа, лактатдегидрогеназа, аспартатаминотрансфераза, креатинин, с-реактивный белок. Проведенный анализ маркеров тканевой травматизации продемонстрировал, что статистически значимые различия между хирургическими доступами были обнаружены только у одного показателя-с-реактивного белка (стандартный прямой латеральный доступ оказался связан с его максимальным увеличением), а разницы в КФК, ЛДГ, АСТ, КРЕ обнаружено не было. В результате проведенной работы установлено, что мышечная травма присутствует при выполнении любых доступов. Это демонстрирует, что основной урон, который влияет на изменение биохимических показателей, наносится не путем отсечения мышц (как при выполнении только стандартного латерального и стандартного заднего доступов), а при давлении хирургических инструментов на мышцы, которое присутствует при выполнении как стандартных, так и малоинвазивных доступов, при которых отсечения мышц от мест прикрепления не производится.
