Введение. Пандемия COVID-19 в 2020 году внесла свой вклад в эпидемиологию респираторных инфекций. Важным является своевременное проведение дифференциальной диагностики COVID-19 и сезонных острых респираторных заболеваний. У пациентов с новой коронавирусной инфекцией возрастает риск развития госпитальной пневмонии. Актуальным является анализ особенностей циркуляции резистентных к антибактериальным химиопрепаратам штаммов возбудителей внутрибольничных инфекций. Цель – изучение этиологической структуры внебольничных пневмоний в период эпидемического распространения COVID-19, оценка рисков присоединения возбудителей пневмоний, связанных с оказанием медицинской помощи. Материалы и методы: Исследовали биологический материал от 446 пациентов с диагнозом «внебольничная пневмония», находившихся на амбулаторном лечении или в стационарах г. Ростова-на-Дону. Верификация респираторных вирусов, включая РНК SARS-CoV-2, а также M. pneumoniae, C. pneumoniae, L. pneumophila выполнена методом полимеразной цепной реакции в мазках носоглотки. Бактериологический анализ мокроты проводили с использованием дифференциально-диагностических сред, идентификацию выделенных патогенов осуществляли с помощью времяпролетной масс-спектрометрии на приборе Autoflex (Bruker Daltonics) c программным обеспечением BioTyper 3,0. Результаты и обсуждение. В период распространения новой коронавирусной инфекции в Ростовской области доля положительных результатов на SARS-CoV-2 среди пациентов с диагнозом внебольничная пневмония составляет 35,6 %. Частота микстинфекций вирусной природы достоверно не отличалась среди пациентов с лабораторно подтвержденным диагнозом COVID-19 и пациентов с отрицательным результатом на SARS-CoV-2 (25,9 % и 26,2 %, соответственно). В структуре микробиоты превалировали грибы рода Candida и плазмокоагулирущие стафилококки. Достоверно чаще от пациентов с лабораторно подтвержденным COVID-19 изолировали культуры неферментирующих грамотрицательных бактерий. У 51,6% пациентов, проходивших лечение в стационаре, отмечено вторичное коинфицирование, вероятно связанное с объектами внешней среды или с передачей инфекции от персонала. Передача ИСМП между пациентами не установлена.
The aim of study was to conduct epizootiological monitoring of natural tularemia foci of the steppe type and investigate epizootic activity in the south-east of the Rostov Region.Materials and methods. An epizootiological survey was carried out on the territory of Remontnensky, Sal’sky and Peschanokopsky districts of the Rostov Region in 2019–2021. To capture and collect mammals, Ixodidae ticks and to study the samples of field material conventional methods were used.Results and discussion. Habitation of 16 species of small mammals, 6 species of Ixodidae ticks has been found. Molecular-genetic analysis of the voles has revealed the presence of the species Microtus arvalis obscurus in the studied area of the region. The circulation of the tularemia agent has been established in the population of common and social voles, forest mouse, hare, rook, Dermacentor marginatus, Hyalomma marginatum, removed from rooks. In May 2020, a high increase in the number of the social vole in the Remontnensky district (up to 21 %), in July 2020 – the common vole in the agrocenoses of the Sal’sky district (up to 33 %) was observed. An extensive epizooty of tularemia was detected in the population of common vole in the south-east of the Rostov Region and in adjacent territories in the Republic of Kalmykia and the Stavropol territory. Two cultures of the pathogen were isolated from the fallen and captured social voles, and four cultures – from the common vole. The isolated strains belong to the Holarctic subspecies of Francisella tularensis EryR. The results obtained attest to the activation of the natural tularemia focus in the south-east of the Rostov Region and its high epizootic activity.
The review provides concise information on the innate ability of cells of the tularemia pathogen, Francisella tularensis subsp. Holarctica, to resist antimicrobials through a variety of mechanisms, leading to its multi-resistance. In total, taking into account new territories, 120 cases of human infection were registered in the Russian Federation in 2022. Epizootic manifestations of the infection of varying degrees of intensity were detected in 58 constituent entities. Against this background, sporadic cases of tularemia in humans were reported in 18 regions of the country. An outbreak of tularemia occurred in the Stavropol Territory; the disease of mild and moderate severity was found in 76 people. The increased incidence of tularemia persists in the Republic of Karelia with severe cases of the disease in the absence of immunoprophylaxis of this infection in the region. A total of 61 cultures of the tularemia pathogen F. tularensis subsp. holarctica, out of which 20 erythromycin-resistant strains were isolated in the Stavropol Territory. In addition, 8 cultures of F. tularensis subsp. mediasiatica from a silt sample and mites Dermacentor silvarum and Haemaphysalis concinna caught in the Republic of Altai were isolated. On the territory of the Russian Federation in 2022, 930 999 people were vaccinated and revaccinated against tularemia. Based on the analysis of the data obtained in 2022, epidemic complications in 2023 in the form of sporadic cases of the disease among the unvaccinated population are most likely to occur in the territories of the Central Federal District – in the Vladimir, Ryazan and Smolensk Regions; Northwestern Federal District – in the Arkhangelsk Region and the Republic of Karelia; Southern Federal District – in the Volgograd and Rostov Regions. The situation in the North Caucasian Federal District will remain tense in the Stavropol Territory; in the Volga Federal District – in the territories of the Saratov Region, as well as in the Kirov Region and the Republic of Mordovia; Ural Federal District – in Khanty-Mansi and Yamalo-Nenets Autonomous Districts; Siberian Federal District – in certain territories of Omsk, Kemerovo, Tomsk, Novosibirsk, Irkutsk Regions, Altai, Krasnoyarsk Territories; in the Far Eastern Federal District, the most intense epizootic activity of natural tularemia foci is in Primorsky Krai.

The upward tendency in the morbidity rate dynamics around the world (compared to 2009) with an average annual growth rate of 5,352 % was revealed. For the first time during the 7th pandemic caused by V. cholerae O1 El Tor, WHO reported 1227391 cases of cholera world-wide in 2017, out of which 1032481 (84.1%) were registered in Yemen, where the war continues and one of the largest epidemics in the world. Представлены данные собственных исследований биоценотической структуры природного очага иксодового клещевого боррелиоза в биотопах Ростовской области. Обнаружена ДНК боррелий комплекса Borrelia burgdorferi sensu lato (s.l.) в пробах от мелких млекопитающих и рукокрылых, птиц, иксодовых клещей и кровососущих насекомых. Полученные данные характеризуют биоценотическую структуру, как поли векторную и полигастальную, обеспечивающую стабильное функционирование природного очага. The data of our own studies of the biocenotic structure of the natural focus of Ixodes tick-borne borreliosis in biotopes of the Rostov region are presented. Borrelia burgdorferi sensu lato (s.l.) Borrelia DNA complex was detected in samples from small mammals and bats, birds, ixodid ticks and bloodsucking insects. The obtained data characterize the biocenotic structure as poly vector and poly pastal, ensuring stable functioning of the natural focus. Введение Иксодовые клещевые боррелиозы (ИКБ) (синонимы: Лайм-боррелиоз, боррелиоз Лайма, болезнь Лайма)-группа природно-очаговых зоонозов с трансмиссивным механизмом передачи возбудителей. Возбудители ИКБ относятся к роду Borrelia семейства Spirochaetaceae. Патогенные для человека и животных виды боррелий объединяют в комплекс Borrelia burgdorferi sensu lato (s.l.) [11, 12]. Природные очаги иксодового клещевого боррелиоза приурочены, главным образом, к лесным ландшафтам умеренного климатического пояса. В пределах России находится значительная или даже большая часть мирового ареала ИКБ. Его границы совпадают с ареалом основных переносчиков-клещей рода Ixodes (Latreille, 1795): Ixodes persulcatus (Schulze, 1930) и Ixodes ricinus (Linneus, 1758) и расположены в таёжной и лесной зонах [5]. Большая часть Ростовской области находится в степной и сухостепной зонах с засушливым климатом и территория, занимаемая лесами, незначительна, ландшафтно-географические особенности региона не являются благоприятными для широкого распространения I. ricinus, вследствие чего, ареал этого вида мозаичен. Carried out is approbation of the new nutrient media set for cholera diagnostics on the basis of bakers' yeast pancreatic digest within the frames of the special tactical training exercises for specialized anti-epidemic team (SAET). It is demonstrated that the developed nutrient media set compare favourably with its analogues, commonly used for practical procedures; and its constituent media have a potential to be included into the SAET mobilization reserve.
