The renal functional reserve (RFR) is the ability of the kidneys to increase renal plasma flow and glomerular filtration rate (GFR) in response to protein intake. It is a measure of functional and anatomic integrity of nephrons. It is not known what relation between RFR and kidney Doppler parameters. We aimed to study the relation between the RFR and renal hemodynamic parameters in hypertensive patients with and without nephropathy who had normal kidney function. Twenty-four hypertensive subjects with nephropathy (HTN-n, n ¼ 10) and hypertension without nephropathy (HTN, n ¼ 14) were included in the study. Control group included 11 healthy subjects. Baseline GFR (GFR1) and GFR after intake of egg protein 1 mg/kg of body weight were determined (GFR2). RFR was calculated by the following formula: (GFR2-GFR1)/GFR1 Â 100%. Doppler ultrasonography was performed. Arterial blood pressure (BP), body mass index (BMI), and estimated GFR were also recorded. HTN and HTN-n groups had impaired levels of RFR compared with controls (p < 0.05), significantly decreased value of flow velocity parameters (Vmax, Vmin), and increased RRI compared with controls. There was significant negative correlation of RFR with blood pressure levels (sBP, r ¼ À0.435, p ¼ 0.009; dBP, r ¼ À0.504, p ¼ 0.002), RRI (r ¼ À0.456, p ¼ 0.008), micro albuminuria (MAU, r ¼ À0.366, p ¼ 0.031) and positive correlation with Vmax and Vmin (r ¼ 0.556, p ¼ 0.001 and r ¼ 0.643, respectively, p < 0.001). Linear regression showed that RRI and MAU were independent predictors of decreased RFR. RFR is lower in hypertensive patients despite near-normal level of kidney function and is related to particular level of BP. RRI and MAU were independent predictors of decreased RFR. ARTICLE HISTORY
Changing of media consumption in the digital age is the object of focus attention of both Russian and foreign researchers. Modern cultural studies note the increasing role of video content in the media environment. Taking into account current trends, libraries more often create their own video materials to implement various goals and objectives. Thus, creation of own video content is rapidly becoming an integral part of the work of modern library. However, despite some established experience of libraries in this area, there is still not enough research of general and theoretical nature on the content, guidelines and prospects for the development of activities related to the creation and use of library video content, and there is no its classification.The purpose of this work is to conduct analysis of the video content in libraries, identify the most common materials, as well as to determine the prospects for using this tool to reach their own goals. The article discusses definitions of the term “video content”. The author presents the main types of videos produced by libraries, their characteristics and features. Special attention is paid to video projects of libraries in Russia. The article reveals the experience of the Astrakhan Library for Youth named after B. Shakhovsky in using its own video content. The paper discusses the issue of classification of video materials produced by libraries. The author proposes the following classification of library videos: video review, virtual book exhibition, webinar (online seminar or web conference), interview, humorous video, webcast, event announcement, video report and booktrailer. The author indicates the main reasons hindering the demand for library video content among the wide range of Internet users and gives the rationale for the necessity and importance of this type of activity and proposes possible prospects for using own video content of libraries. Thanks to its presence, the library ceases to be a closed institution storing knowledge within itself that produces positive impact on its image. Using means of communication that are understandable to a person of visual culture, it changes stereotypes and demonstrates its modern capabilities.
The efficacy of SPECT-CT with 99mTc-MIBI for detecting axillary lymph node (LN) involvement was evaluated in 184 patients with primary breast cancer. All patients were operated with histological examination of axillary LNs. During statistical analysis we determined correlation between LN metastases and various diagnostic and clinical characteristics. SPECT-CT signs of LN involvement were as follows: short axis more than 10 mm, cortical thickness more than 4mm, solid structure, round shape, intensive tracer uptake. More than 2 metastatic LNs were considered as plural metastatic damage. The complex model for evaluation axillary LN involvement was elaborated. The use this data in our study showed that SPECT-CT 99mTc-MIBI could correctly exclude 2 or more metastatic LNs in 96% breast cancer patients.
Objectives: To determine the efficacy of fetal stem cell transplant for treating patients with diabetes mellitus types 1 and 2. Materials and Methods: Five patients with diabetes mellitus type 1 and 5 patients with diabetes mellitus type 2 (aged 18-56 years) received a fetal pancreatic stem-cell transplant (cells were 16-18 wk gestation) performed by intravenous infusion at 50 mL/hour. The quantity of fetal stem cells infused was ≥ 5-8*10⁶. We analyzed the patients' C-peptide and glycated hemoglobin levels both before and 3 months after fetal stem cell transplant. Results: In patients with diabetes mellitus type 1, fetal stem-cell transplant led to a significant increase in C-peptide levels, from 0.09 ± 0.01 ng/mL to 0.20 ± 0.07 ng/mL, after 3 months (P < .008). Conclusions: Treatment with fetal pancreatic stem cells may be beneficial for treating patients with type 1 or type 2 diabetes.
